Unusual complication of the Sengstaken-Blakemore tube.

Reported here is the first description of innominate vein obstruction by esophageal tamponade. The syndrome is characterized by local edema of the area drained by the vein. The diagnosis can be made by phlebography. Deflation of the balloon corrects the problem.

The surgical treatment of coarctation of the aorta.

Patients with coarctation of the aorta have a significantly decreased life expectancy. Surgical correction is curative and involves a low mortality rate which increases with coexisting cardiovascular abnormalities. In our series, there were no operative deaths in patients with pure coarctation of the aorta. Age is an important factor and postoperative complications were more frequent in older patients. Re section of the narrowed area with end-to-end anastomosis is the technique of choice. If theis is not possible, a Dacron tube is a good alternative. Early diagnosis and early intervention are of utmost importance, even in asymptomatic patients.

CT and MR findings in a calcified myocardial tuberculoma of the left ventricle.

Tuberculosis can involve a multitude of organ tissues but generally affects the respiratory tract. Heart tuberculosis is rare, and the pericardium is the most common location reported in the literature, accounting for 0.5% of extrapulmonary tuberculosis. Isolated myocardial tuberculosis is a very unusual occurrence, with only a few case reports in the literature, usually diagnosed at necropsy. Findings of myocardial tuberculoma on cross-sectional images previously have not been reported. We present computed tomography and magnetic resonance imaging findings of an isolated calcified myocardial tuberculoma.

A new surgical technique for aneurysmal right atrium.

A new surgical technique for reduction in size of aneurysmal right atrium (ARA) is described. The extracorporeal perfusion is based on extrapleural single stage caval return and profound hypothermia, with circulatory arrest. This method enabled us to diminish the cavity dimensions in the presence of a giant friable ARA, by excising the thinned portion of the free atrial wall and double-layer closure of the remaining edges (close to the AV groove) to the crista terminalis.

[Infected atrial myxoma simulating infective endocarditis].

BACKGROUND: We present a case of infected atrial myxoma simulating bacterial endocarditis. MATERIAL AND METHODS: A 75 year old male, without any antecedents of interest, with fever an evolution a month, and a cardiac murmur. Blood cultures were practiced by BACTEC N.R. 730 system. The diagnosis was performed by the bidimensional and transesophageal echo, and confirmed with the resection from surgery. The method of bibliographical review used, has been the data base of comprehensive Medline, cvc since 1987 up June 1992 and the Oncodise's Concerlit archive from the year 1985 up to june 1992 the date base of the IME (Indice Médico Español) was also review up to April 1992. RESULTS: The blood cultures were positive, 7/7 Streptococcus viridans and 3/7 a Staphylococcus epidermidis resistant to meticiline, we them with the same antibiogram. Echocardiography and angiography show a lef atrial mass, surgical resection of the mass, confirmed the hystological diagnostic of the myxoma and signs of infection. CONCLUSIONS: This is the first case in our country, with shows the rarity of it. We analyze the differential diagnosis with the non infected myxoma and the bacterial endocarditis. We think that there should be an early surgical treatment, together with antibiotic therapy, due to the high incidence of embolism in theses patients.
