[Prevention of asphyxia in tracheomalacia after operations of the thyroid gland].

A new technique for prevention of asphyxia in tracheomalacia has been developed. The technique consists in suturing the softened portion of the trachea to the dura mater graft which had been previously fixed to its healthy portions. Such a technique allows performing reliable fixation of the tracheomalacia portion in an extended state and thereby to prevent asphyxia, The technique was used in 11 operated patients and gave good results.

[Novocaine blocks in the overall treatment of acute cholecystitis, cholecystopancreatitis and pancreatitis].

The authors used paranephric, vagosympathetic and vagoganglionic blockade in 253 patients with acute cholecystitis, cholecystopancreatitis and pancreatitis. The universally adopted methods were used for the estimation of the results with the recording of electrogastrogram before the blockade, immediately after novocaine injection and on hour later. The arrest or subsiding of pain syndrome occurred after paranephric blockade in 78.8% of patients, after vagosympathetic blockade in 95.2% and after vagoganglionic blockade in 92.6%. 20 patients were operated upon.

[Surgical treatment of acute pancreatitis].

The authors studied 939 patients with acute pancreatitis; 144 of them were operated upon. Total postoperative mortality was 18%; of them 1.05% mortality was due to a mild form and 51.0% mortality occurred in pancreonecrosis and purulent pancreatitis. In extensive necrosis and abscess formation the drainage of the abdominal cavity and retroperitoneal space is recommended. The authors believe that omentopancreatopexy should be applied in oedematous and hemorrhagic forms of pancreatitis and in focal necrosis, as well.

[Enzymatic cholecystitis and cholecystopancreatitis].

Taking into consideration the role of pancreatic enzymes in the etiology of cholecystitis and cholecystopancreatits, the authors examined amylase in 92 patients, bile lipase--in 39 patients during cholecystectomy and also amylase in the choledochus bile during external drainage of the duct in 15 patients in the early postoperative period. The investigations inducated different levels of pancreatic enzymes in bile. Their level is found to depend on the occurrence of hypertension in the bile tract both pre- and postoperatively.

[Blood insulin levels in surgical treatment of peptic ulcer].

Blood plasma insulin has been studied in 74 patients with gastroduodenal peptic ulcer (GDU) with the aid of the radioimmunological method before the operation, in the early and remote postoperative periods. Prior to surgery, the level of insulin gets increased on an empty stomach and after stimulation both in gastric and in duodenal peptic ulcer. The hormone content has been found to be reduced postoperatively, both in the immediate and remote periods but pyloroplasty in selective proximal vagotomy and especially in selective vagotomy resulted in a greater increase in insulin following its stimulation with test breakfast. Rehabilitative measures before and after surgery have been shown to be associated with augmentation of the blood level of the hormone, which fact promotes increase of the trophic effect of insulin on the gastrointestinal system.

[Several kinin system indices in diseases of the thyroid gland].

Content of free bradikinin, kininogen and kininase activity were studied using biological methods in blood serum of 112 patients with goiter, of 20 patients with instrangulated hernia and of 21 healthy persons. The patterns studied were similar to normal value in patients with euthyroid goiter and instrangulated hernia. Thyrotoxicosis (63 patients) was accompanied by increase in all the patterns studied and the alterations were most pronounced in severe form of the impairment. Content of kininogen and kininase activity were increased about 2-fold and bradikinin content--5-fold. Content of free bradikinin was increased and kininase activity was decreased in all groups of patient within 1--3 days after the operation. Content of kininogen was increased in euthyroid goiter and decreased in thyrotoxic one. Patterns of kinine system trended to normalization within 6 days after the operation.
