Thoracic duct and hepatic lymph in idiopathic portal hypertension.

Lymph dynamics in idiopathic portal hypertension has been studied in two phases. In the first phase thoracic duct lymph transport was studied in 11 patients with idiopathic portal hypertension by cannulating the duct. This revealed altered lymph transport in the form of a distended thoracic duct, raised pressure in the duct, and haemorrhagic lymph with an increased flow rate. The lymph flow rate was analysed in relation to various hepatic haemodynamic and biochemical parameters. In the second phase of the study hepatic lymphatics were studied by percutaneous hepatography in 16 patients with idiopathic portal hypertension. By this technique hepatic lymphatics were opacified in patients with idiopathic portal hypertension and cirrhosis with equal frequency. The significance of this finding in relation to the altered hepatic haemodynamics and thoracic duct lymph transport is discussed.

Primary biliary cirrhosis in India.

In a prospective search to study the occurrence of primary biliary cirrhosis, only one patient with this disease was encountered during the last six years. This is also the first documented case of primary biliary cirrhosis from the Indian subcontinent.

Homogeneous enzyme immunoassay of estradiol using estradiol-3-O-carboxymethyl ether as hapten.

A homogeneous enzyme immunoassay for estradiol estimation has been developed, which can be extended to other steroids. A new procedure for the preparation of estradiol -3-0- carboxymethyl ether by a simple one step reaction in high yield (90%) has been described. This hapten has been used for raising highly specific anti-estradiol antibody in rabbits and for preparation of enzyme conjugates. Two different enzymes, lysozyme and glucose -6- phosphate dehydrogenase have been studied for their suitability as enzyme labels. Our results indicate that lysozyme-conjugate meets the essential requirement for a practical enzyme immunoassay. The advantage of the present nonradioactive procedure is the overall simplicity, low cost and high stability of the reagents.

Screening for diabetes in pregnancy: is a random blood glucose enough?

It has been suggested that a single random blood glucose measurement, timed in relation to food, can be used to determine those women needing a formal oral glucose tolerance test for the detection of gestational diabetes. One hundred and ten pregnancies have been screened to compare the results of a formal oral glucose tolerance test with timed venous plasma glucose measurements taken throughout the day. At the suggested thresholds of 6.1 mmol/l (within 2 h of eating) and 5.6 mmol/l (at greater than 2 h of food), random blood glucose testing is specific and excludes most normal women. However, the sensitivity of the test is low, and the majority of those with impaired glucose tolerance would be missed. Random blood glucose measurement is not a sufficiently sensitive method for detecting gestational diabetes as presently defined.

An aminopeptidase regulates LPS stimulated interleukin-8 receptor on the surface of human neutrophils.

A large number of inflammatory diseases are mediated by interleukin-8, an inflammatory neutrophil chemotactic agent. Since the cytokine acts through a cell surface receptor, detailed knowledge about the regulation of receptor expression is very important. We found that LPS in serum became activated and triggered the expression of IL-8 receptor by more than two folds within 30 min. After that period, the receptor attained normal level within 2 hr of SA-LPS stimulation. EDTA and bestatin could block this downregulation of IL-8 receptor. Intracellular Ca2+ level was increased till 45 min of SA-LPS stimulation and then the level was reduced. Addition of CaCl2 accelerated and depletion of Ca2+ inhibited the downregulation of the IL-8 receptor. The ligand could fully protect the loss of receptor from downregulation. It suggests that during SA-LPS stimulation, increase in intracellular Ca2+ level activates an aminopeptidase which presumably cleaves the N-terminal region of the receptor, critically essential for the function of IL-8. Thus the activated aminopeptidase regulates the functions of IL-8. The study is important for understanding the regulation of IL-8 receptor expression by LPS during bacterial infection.
