Botulinum toxin injection is an effective treatment for spasmodic dysphonia. There are ethical difficulties in performing a controlled placebo trial to assess the effectiveness of this treatment. This paper shows a significantly decreased clinical response following technically poor injections given to patients who respond well to good quality injections.
Adenotonsillectomies are performed increasingly as day cases in North America and Canada. This paper analyses reports where such tonsillectomies were performed. It is possible to successfully select a group with lower risk of complications. Haemorrhage occurs either early in the post-operative period or later than 24 hours after operation. With good organization tonsillectomies can be safely performed as day cases.
Tonsillectomies have been performed on a same-day basis in Coventry for the past three years. We report our experience with this technique which has gradually evolved over the last ten years. The procedure is described in detail and the results of the first two years have been analysed. In common with the experience of others, it is possible to perform tonsillectomies safely on a same-day basis.
Parents of children undergoing (adeno) tonsillectomy were questioned about their preferences for a period of hospitalization. The proportion wanting early discharge increased as their children recovered after surgery.
Velopharyngeal function, voice and speech changes following uvulopalatopharyngoplasty (UPPP) were studied prospectively and compared with those in an age and sex matched group undergoing tonsillectomy. The fundamental frequency of voice was significantly lower in those undergoing UPPP. All the other changes studied were not significantly different in the two groups.
Leiomyosarcoma of the larynx is extremely rare. A patient with respiratory obstruction due to such a tumour is presented. This was originally misdiagnosed as hysteria. The treatment of choice was an emergency laryngectomy.
