Reported symptoms from the masticatory system and general well-being in rheumatoid arthritis.

Eighty-one patients with rheumatoid arthritis (RA) and 41 patients with temporomandibular disorders (TMD) were evaluated with questionnaires regarding subjective symptoms from the masticatory system. The general well-being was assessed using the Mood Adjective Check List (MACL) and the Body Symptom Scale (BSS). Patients with rheumatoid arthritis in general had less symptoms from the masticatory system than TMD patients. The RA patients reported that their symptoms were related to acute phases of the general disease, while the TMD patients reported that mental stress, anxiety, bruxism and chewing aggravated their symptoms. The RA patients had more general physical discomfort than TMD patients according to the BSS. The RA patients rated their mental well-being (MACL) close to normal, except that they were less active than TMD patients. The TMD patients, on the other hand, showed higher values for mental tension. In conclusion, many patients with RA will develop TMD symptoms but there is a great variation in time relationship between the onset of RA and involvement of the masticatory system.

Craniomandibular disorders in rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis. A clinical study.

Sixty-one subjects with rheumatoid arthritis, 61 with psoriatic arthritis, 61 with ankylosing spondylitis, and 61 healthy controls were examined with regard to subjective symptoms and clinical signs of craniomandibular disorders (CMD). The frequencies of most subjective and clinical variables were higher in all three disease groups than in the control group. Subjects with rheumatoid arthritis and psoriatic arthritis showed more frequent and severe signs and symptoms than subjects with ankylosing spondylitis. It is concluded that subjective symptoms and clinical signs of CMD are common in rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis and are mainly caused by the respective general joint disease. None of the signs and symptoms is pathognomonic for rheumatoid arthritis, psoriatic arthritis, or ankylosing spondylitis.

Radiographic changes in the temporomandibular joint of patients with rheumatoid arthritis, psoriatic, arthritis, and ankylosing spondylitis.

Sixty-one subjects with rheumatoid arthritis, 61 with psoriatic arthritis, 61 with ankylosing spondylitis, and 77 healthy controls were examined using orthopantomography to determine the frequency of radiographic changes in the condyle of the temporomandibular joint. Radiographic changes were found significantly more often in subjects with rheumatoid arthritis (66%), psoriatic arthritis (38%), and ankylosing spondylitis (30%) than in controls (12%). Subjects with rheumatoid arthritis also had significantly more radiographic changes, especially cortical erosions and subcortical cysts, than subjects with psoriatic arthritis or ankylosing spondylitis. It may be concluded that rheumatoid arthritis is a more severe disease than psoriatic arthritis or ankylosing arthritis regarding temporomandibular joint involvement.

Endogenous as well as exogenous hormonal modulation of lectin binding sites in normal and neoplastic tissue of rat mammary gland.

Free and sialic acid conjugated binding sites for the lectin from Arachis hypogaea (peanut agglutinin, PNA) have been histochemically demonstrated in normal rat mammary tissue and in N-nitrosomethylurea (NMU) induced mammary tumors of rats. The lectin binding sites were predominantly associated with secretory activities of the normal and neoplastic breast tissue (secretory PNA binding sites). In normal breast tissue and in NMU induced mammary tumors the expression of secretory PNA binding sites was reduced after ovariectomy and by the antiestrogen tamoxifen. Estrogen stimulated the formation of free and sialic acid conjugated PNA binding sites in the presence of prolactin. In rat mammary tumors the estrogen induced formation of PNA binding sites was accompanied by an increase of the progesterone receptor concentration in the tumors. Comparative lectinhistochemical, morphological, and biochemical studies on NMU induced rat mammary tumors revealed that the expression of secretory PNA binding sites was associated with a good histologic differentiation and the presence of steroidhormone receptors. Endocrine therapeutic studies showed that tumors responding to therapy possessed higher amounts of secretory PNA binding sites than unresponsive tumors. Therefore secretory PNA binding sites represent a useful histochemical marker for hormone dependence in NMU induced mammary tumors.

[Catheter angiography in the aged patient: techniques and results (author's transl)].

We report on 133 suprarenal translumbar, 74 transfemoral and 1 transaxillary, together 208 catheter angiographies in patients aged over 70 years. The most frequent indication was arterial occlusive disease (74 per cent). For imaging of the abdominal aorta, the iliac and lower limb arteries and of the aortic arch we prefer the suprarenal translumbar catheter angiography in local anesthesia. Selective imaging of abdominal and supraaortic arteries is the main indication for the transfemoral catheter angiograhy. Side-effects and complications are limited to 2.4 per cent each. Puncture problems were present in this age group twice as often in the transfemoral approach (11%) than in the translumbar approach (6%). In 2 patients both angiographic methods failed.

[Surgical consequences of anomalous arterial blood supply and aneurysms in the epigastric region (authors' translation)].

Abdominal angiography was performed in patients with occlusive disease of the iliofemoral arteries. In 85 of 389 patients, the anomalous arterial blood supply of the upper abdomen was detected accidentally: 81% of the variations were related to the hepatic artery, 19% to the celiac artery. The possible surgical consequences are discussed if these variations are disregarded. Four patients with aneurysms of the visceral arteries (three aneurysms of the hepatic artery, one aneurysm of the superior mesenteric artery) and one patient with a celiac artery compression syndrome were also detected. The latter diseases represent clear indication for operation with high chances of success if treated in time.

[Asymptomatic ablatio placentae].

A 34 years old woman was admitted to our hospital with a retroplacental hematoma of more than 50% of placental area. It was diagnosed by routine pregnancy screening. The patient had no pain, bleeding or other clinical symptoms. 24 hours after inducing fetal lung maturation by betamethasone delivery was terminated by caesarean section in the 32nd week of gestation. Though the diagnose was confirmed by operation there was no influence of fetal outcome. The management of symptomless ablatio placentae is discussed by literature. Termination of pregnancy should be related the gestational age.
