The effects of increased penetration of betamethasone dipropionate in a propylene glycol base (Diprolene) for psoriasis.

The efficacy and safety of betamethasone dipropionate, 0.05% in a propylene glycol base (Diprolene), was investigated in an open, fixed-dose study of thirty-one patients with resistant psoriasis. Clearing of lesions or marked improvement occurred in 97% (thirty out of thirty-one) of patients. Increased penetration of corticosteroid did not cause adrenal suppression or other adverse reactions.

Is palmoplantar keratoderma of Greither's type a separate nosologic entity?

17 patients with palmoplantar keratoderma (PPK) were detected among 48 investigated members of the clan O. 5 of these 17 patients had a well-expressed hyperkeratosis on their knees. In one family hyperkeratosis of the knees was inherited as a dominant feature, but in two other families it was not. The observation was made that the expressivity of the disease was fading: while there were 15 PPK patients among the 25 investigated members in the generations II and III, there were only 2 patients among 22 members in the generations IV and V. In addition to PPK incontinentia pigmenti was diagnosed in two instances and pollex duplex in one. The question of the identity of PPK of Greither's type is shortly discussed.

Oral contraceptives and their influence on porphyrin concentrations in erythrocytes and urine.

In 15 females who have never before taken oral contraceptives, the porphyrin concentration in erythrocytes and in urine were investigated. The laboratory assays were performed before and after being on the oral contraceptive Stediril during 5 months (2 women took Stediril during only 3 months). The mean PP concentration in the erythrocytes increased from 16.4 microgram (before) to 24.1 microgram/100 ml of erythrocytes after taking Stediril regularly for 5 months. A statistical evaluation with the Student's test showed that at the level of 2%, the t(exp) = 2.58 was larger than the theoretical t(0.02) = 2.47. The difference of the mean CP concentration in the erythrocytes before and after taking Stediril was not statistically significant at the level of 5%. The mean concentration of CP in urine increased from 119.2 to 137.1 microgram in 1,000 ml. This difference was, however, not statistically significant at the level of 5% when assayed with the same test. There was no increase in UP concentration in urine.

[Juveline xanthogranuloma. A case report].

A case of xanthogranuloma juvenile (the small papular form) in a six month-old boy is presented. Only cutanous lesions are seen and the child is of good health. On the scalp, trunk and limbs there are 17 yellowish pea-sized papules. Histopathology reveals a dense infiltrate in the dermis which is composed of histiocytes, a number of giant cells of the foreign-body type is also present. With Sudan III staining in some of the histiocytes small droplets of lipids are seen. Other laboratory investigations are within normal limits.

Fluorescence microscopy test in porphyrias, photodermatoses and lead exposed persons.

Fluorescence microscopy tests were carried out in different groups of patients Peripheral blood diluted with saline was used and 200 high power fields were inspected in every case. The results were presented as the number of fluorescing erythrocytes (FE) per 100000 red blood cells (or 200 fields). In the controls, porphyria cutanea tarda patients and patients with photodermatoses other than erythopoietic protoporphyria and pellagra almost no FE were detected. In erythropoietic protoporphyria the mean value was 10600, in lead poisoning 1032, in patients exposed to lead 48.2, in sideropenic anaemia 123 and in patients with pellagra 8.1 FE/100000 red blood cells. The conclusion is made that one has to take care, when using this test for detection of latent carriers in genetic studies of the relatives of patients with erythropoietic protoporphyria. The test is useful for the confirmation of the diagnosis of erythropoietic protoporphyria.

[Disseminated granuloma annulare].

A case of generalized granuloma annulare in a 55 year old man is reported. The disease appeared five years before the first admission to the hospital. A large number of bluish-red or skin-colour papules were scattered mainly around the earlobes, buttocks and on the extremities. Some of the lesions were lined up in rings or plaques. Small depigmented and brownish scars were present. Two biopsies revealed characteristic foci of complete collagen degeneration accompanied by a palisading infiltrate in the upper dermis. Treatment with tuberculostatics and antimalarics was without improvement. The lesions cleared after a course of prednison, but reappeared when the drug was discontinued.
