Effects of adenosine administration on spike-wave discharge frequency in genetically epileptic rats.

1. In the present study, the effects of the administration of adenosine on absence seizures were investigated in the Wistar Albino Glaxo/Rijswijk (WAG/Rij) strain of rats, which are an adequate model for human absence epilepsy. 2. After baseline electroencephalogram (EEG) recording, adenosine was injected intraperitoneally (i.p.) at doses of 6, 30 and 120 mg/kg and EEG recordings were continued for 1 h postinjection. In addition, to enable evaluation of the contribution of peripheral factors to the occurrence of spike-wave discharges (SWD) after adenosine injection, arterial blood pressure and rectal temperature were also recorded. 3. Injection of adenosine induced a dose-dependent increase in the appearance of SWD. The number and total duration of SWD were found to be significantly increased after 30 and 120 mg/kg adenosine (P < 0.05). Adenosine also induced a fall in both blood pressure and body temperature in all experimental groups. 4. These results show that peripheral administration of adenosine promotes absence seizures. It is likely that the lowering of blood pressure and body temperature and the activation of sensory afferents after adenosine injection may contribute to the facilitation of SWD observed in the present study.

A process to maintain the quality of a computerized knowledge base.

As part of a project to develop knowledge-based reminders for the outpatient setting, we developed a process to help maintain the quality of the knowledge base. The knowledge engineering process involved many parties, including several domain experts, a knowledge engineer, and a programmer and a process was necessary to assure that information transfer among individuals did not become confused. An MS Access database was created to store, among other data, textual versions of the rules as they evolved over time. In a 9-month period 36 rules were entered into the database. Of those, 17 are still active in their original form. The remaining 19 underwent various types of modifications; these changes were tracked in the database. Processes and tools to maintain knowledge bases are necessary if the benefits of clinical decision support systems are to be realized and investments in knowledge engineering are to be protected.

[Electrolyte disorders, EEG changes and epileptic seizures in alcohol withdrawal delirium].

For 180 patients suffering alcohol-withdrawal induced delirium, electrolytic concentration in the serum of Na, K, Ca, and Mg was determined in the early withdrawal phase, and the electroencephalograms of 95 delirium patients evaluated in respect of local and diffuse changes and epileptic activity, and compared in delirium patients with and without initial seizures. Delirium patients who had initial seizures suffered significantly longer-lasting periods of delirium and significantly more frequent electrolytic changes in the form of hypomagnesemia and hypopotassemia (hypokalemia). There was no significant difference in the EEG changes. A temporary metabolic disorder in the initial phase of the two-phase withdrawal process should be assumed to be the cause of seizures during alcohol withdrawal, and the pathogenetic significance of hypomagnesemia and hypopotassemia should be taken into consideration.
