Clinical pharmacy on-call program in the emergency department.

This is a descriptive and evaluative report regarding a clinical pharmacy residency in-house on-call program that facilitates the provision of 24-hour clinical pharmacy consultative services within the emergency department. During the two-year evaluation period, clinical pharmacy residents completed 3.1 consultations per 14-hour call period. Additionally, greater than 90% of the clinical pharmacy recommendations were accepted and completely followed by physician cohorts. These consultations were usually solicited by internal medicine physicians within the emergency department, involved patient-specific drug therapy recommendations, and required about 100 minutes (mean per consultation) of the clinical pharmacist's time. Thus, clinical pharmacy consultative services can be a useful addition to health care within the emergency department in teaching hospitals.

Studies on macrocyclic lactone antibiotics. VIII. Absolute structures of rhizoxin and a related compound.

The absolute structure of rhizoxin, a potent antifungal and antitumor antibiotic, was determined by interrelation with compound 2 whose structure was established by X-ray analysis. Since a 18OH group was introduced at C-3 on a hydrolytic cleavage of C-2, C-3 epoxy group with alkaline H2(18)O, the original epoxy oxygen should be retained at C-2. The stereo-chemistry at C-2 and C-3 positions in rhizoxin was, therefore, determined as 2R,3S.

Pharmacologic approach to ischemic stroke management.

Currently, pharmacologic intervention is directed toward preventing further spread of damage in the patient with cerebrovascular disease. Anti-platelet agents decrease platelet aggregation responses and anticoagulants decrease formation of the fibrin plug. In addition to the potential clinical benefits of these agents, risks must be considered. Patient education and compliance are major factors for consideration when assessing and monitoring treatment plans. Therefore, it is beneficial to the patient for health care teams to engage in multidisciplinary rounds which foster a consistent approach to treatment and allow each team member to make his unique contribution to the overall care of the patient.

Endotracheal drug administration in the critical care setting.

Administration of drugs through an endotracheal tube has been evaluated in a number of animal models. In addition, the technique has been utilized in humans as reported in several published cases. A review of endotracheal drug administration with emphasis on application to the critical care setting is presented.
