Potential value of digital radiography. Preliminary observations on the use of dual-energy subtraction in the evaluation of pulmonary nodules.

The radiographic demonstration of calcification in a solitary pulmonary nodule renders the possibility of malignancy extremely unlikely, although rare exceptions have been reported. Conventional roentgenograms and tomograms sometimes provide inconclusive evidence although CT can be highly accurate in both identifying and quantifying calcium content. An alternative method is dual-energy subtraction utilizing scanned projection digital radiography. Forty-one patients with solitary (occasionally multiple) pulmonary nodules were examined with the technique, employing second-generation fan-beam equipment: 28 nodules or masses were noncalcified and 13 calcified. Of the former, 20 were pathologically proved, 16 being malignant and 4 benign (2 granulomas, 2 bronchiectasis); in 3 of the remaining 8, a presumptive diagnosis was reasonably certain (1 granuloma, 2 metastases), while in 5 the diagnosis was not made. In 8 of the 13 calcified lesions, the diagnosis can reasonably be regarded as confirmed as granulomas; 5 are being followed up with that presumptive diagnosis.

[The historical achievement and the limits of the work of Sigmund Freud (1855-1939)].

In this paper written on the occasion of Sigmund Freud's 125th birthday, an attempt is made to strike a balance of earlier marxist assessments of Freud's work. After a short presentation of the more important biographical and work-historical data, mainly Freud's antropological basic conceptions and their problematic consequences for his personality-theory are dealt with. Freud's historically important achievement is seen in the opening of a psychological access to the understanding of neuroses as well as the discovery of dynamic processes in the psychotherapeutic encounter.

The role of psychological set in psychosocial stress.

Objectively equal social stress may produce very different changes in different personalities, depending upon the subjective interpretation of the external situation. On investigation of the factors responsible for this subjective evaluation we found the psychological set to be of particular importance. We therefore compared some psychological sets of hypertensives, neurotics and adjusted normotensives. Following psychotherapy aiming at changing some of these psychological attitudes we were able to see improvement of the high blood-pressure.

[Developmental trends in psychotherapeutic concepts and methods].

In the course of the development from symptom-centered to personality-centered, psychoanalytic, behavior- and "client-centered" therapeutic conceptions, integration of psychotherapy within the realm of comprehensive medicine was temporarily disturbed. In this process psychotherapy restricted itself more and more to treatment of neuroses. At present due to the growing recognition of socio-and psychogenic components of somatic illness there exists the chance of integrating psychotherapy as a cross-sectional specialty into comprehensive medicine. In the USSR this state of affairs already exists.
