Unusual pathological condition in the lower extremities of a skeleton from ancient Israel.

We report here an unusual pathology in a 1,500-year-old skeleton recovered from Bet Guvrin, Israel. The pathological changes in the lower extremities and the diagnostic difficulties are presented. We attribute this condition to Madura foot, found primarily in tropical and subtropical regions, although other diagnoses are possible and are noted. We discuss its appearance in the region in light of the historical context.

Total femoral replacement.

Between 1973 and 1983, 19 patients with sarcoma of the femur were treated by adjuvant chemotherapy, excision of the entire femur, and replacement by a total femoral prosthesis. Five patients had excellent and nine had good functional results. Twelve patients died an average of 23 months after the procedure and seven are at present disease free. This limb-saving procedure permits rapid rehabilitation, prevents severe psychological problems, and improves the quality of life.

Stimulation of bone formation by electrical current on spinal fusion.

An experimental double-blind study to evaluate the effectiveness of a constant current stimulator was performed on 30 young pigs that underwent a spinal fusion at the level of L5-L6. The group of animals with an active bone-growth stimulator showed a statistically significant increase of osteoblastic activity with bone formation.

The cardiac pacemaker patient with a femoral neck fracture.

More than half the patients with permanent cardiac pacemakers (PCPMs) are over the age of 70 years. Patients in this age group are also more susceptible to fractures of the femoral neck requiring surgical intervention. The authors studied five patients with PCPMs who underwent surgery for femoral neck fracture. They discuss the special precautions necessary during surgery to prevent electromagnetic interference with the cardiac pacemaker as well as the special management of these patients.

Ruptures of the ligaments about the tibio-fibular syndesmosis.

The clinical picture of ruptures of the ligaments about the tibio-fibular syndesmosis are presented. The diagnosis should be based on the history and clinical examination. Arthrography of the ankle confirms the diagnosis. We describe a surgical method of stabilizing this affected joint, wherein local bone is used to arthrodese the tibio-fibular joint.

Tension band wiring for fixation of comminuted fractures of the distal radius.

In the treatment of unstable comminuted and intra-articular fractures of the lower end of the radius, we have found that is is essential to reconstitute the normal anatomical relationship of the radius to the carpus and to the ulna. This is possible by performing an open reduction and by maintaining the fractured fragments by Kirschner wires and adding a tension band. Patients treated thus are reported. There have been no complications and the patients regained full function of the wrist without unsightly deformity.

Effect of venous stasis on the consolidation of fractures.

In an experimental study of 60 rabbits (120 preparations) in which a bilateral fibular osteotomy was performed, a group of 40 rabbits had ligation of the vena cava, and a group of 20 rabbits were used as controls. The results were evaluated by microscopic study by H&E, PAS abd Alcian Blue staining and showed that venous stasis accelerates the process of fracture healing by increasing fibroblastic and osteoblastic proliferation and bone production. The effects of changes in pH, and concentrations of oxygen and carbon dioxide are suggested as the factors that modify the mechanism of osteogenesis, and greater quantities of mucopolysaccharide substances were observed at the fracture site in the group with venous stasis.

Total femur replacement in sarcoma of the distal end of the femur.

Five patients with sarcoma of the distal end of the femur were treated by a total resection of the femur and its replacement by an endoprosthesis. This was to preserve the body image, prevent mutilation and permit independent ambulation. A chemotherapy program was established with administration of a high dose of methotrexate plus citovorum factor, adriamycin and vincristine. surgical removal of pulmonary metastases and other local recurrences was performed where and when possible. The functional results were excellent in four and poor in the remaining patient. All of the patients rapidly achieved physical rehabilitation and developed a stable and painless gait. This approach to the treatment of osteogenic sarcoma attempts to eliminate the primary tumor, the micrometastases and the overt metastatic disease, leaving the patient to follow as normal a life a possible, which is not the case when a high thigh amputation or a hip disarticulation is performed.

Synovial osteochondromatosis of the shoulder joint.

A case of synovial osteochondromatosis in a synovial recess of the shoulder joint, which is considered to be a very rare condition, is presented. Different clinical and pathologic aspects of this condition are reviewed and discussed. Treatment consists of a surgical removal of the free loose osteocartilaginous bodies with a partial excision of the affected synovia. The condition did not recur.

Osteogenesis by periosteal transplant. Experimental study of spinal fusion in rats.

Autogenous transplantation of periosteum was performed in 60 rats. Periosteum was transferred from the femur to the spinal column in the region of thoracic vertebra 8 to lumbar vertebra 2. Follow-up included histological examination and microradiography. New bone formation occurred late and the bones were of a heterotopic nature; a solid arthrodesis was not achieved. Free periosteal grafts do not exhibit the relatively predictable behavior of periosteal flaps. Greater understanding at the biomolecular level may be required before free periosteal grafts can be introduced into routine clinical practice.
