Multiple thyroid involvement (intraglandular metastasis) in papillary thyroid carcinoma. A clinicopathologic study of 105 consecutive patients.

Multiple thyroid involvement (MTI) in papillary thyroid carcinoma was clinicopathologically studied in 105 non-selected, consecutive patients. Whole thyroids resected by total thyroidectomy were sectioned at intervals of 2-3 mm (mean number of slices per gland, 19.2) and histologically reviewed. The intraglandular cancer foci, other than the tumor regarded as the primary focus, were demonstrated in 82 (78.1%) of 105 patients. The foci were usually small (less than 4 mm) and were not accompanied by sclerotic fibrous stroma or by a fibrous capsule. These small foci were distributed around the primary lesion and also were found frequently (61.0%) in the opposite lobe as bilateral disease. In the opposite lobe, a similar incidence (approximately 30%) of disease was obtained in each of the three parts (upper, middle, and lower). The mean number of foci in patients with MTI was statistically correlated to age, the presence of lymph node metastases, and the presence of solid areas or psammoma bodies in the primary tumor. It was concluded that MTI could be regarded as one of the most striking and important biologic characteristics of papillary thyroid carcinoma.

Proliferating cell nuclear antigen (PCNA) immunostaining as an alternative to bromodeoxyuridine (BrdU) immunostaining for brain tumours in paraffin embedded sections.

Immunohistochemical staining for proliferating cell nuclear antigen (PCNA) and BrdU using anti-PCNA and anti-BrdU monoclonal antibodies, respectively, was performed in 16 human brain tumours, including 3 glioblastomas multiforme, 2 anaplastic astrocytomas, 1 cerebellar astrocytoma, 2 recurrent meningiomas, 4 non-recurrent meningiomas, 3 neurinomas and 1 medulloblastoma. Patients with brain tumours received an injection of bromodeoxyuridine (BrdU) intravenously during surgery, and tumour specimens were fixed in 70% ethanol and embedded in paraffin. The percentage of positive cells for PCNA was compared with a BrdU labelling index using adjacent paraffin-embedded sections. The percentage of PCNA-positive cells was correlated with the BrdU labelling index and the histological malignancy of the brain tumours. The correlation coefficient was 0.84. This suggests that the immunohistochemical staining for PCNA in paraffin sections is a good alternative to the BrdU labelling index.

Immunohistochemical localization of superoxide dismutases in fetal and neonatal rat tissues.

We investigated the developmental profile of copper-zinc and manganese superoxide dismutase (CuZnSOD and MnSOD) in tissue sections obtained from fetal (Day 12 to 21 of gestation) and neonatal (Day 0 and 6) rats. Tissues were stained immunohistochemically with specific antisera against the respective rat SODs. There was a general trend towards richness of SODs in the epithelial linings and metabolically active sites, although differential distribution between the two SODs also existed. At Day 12 of gestation, immunoreactivity for both SODs was detected in the cardiomyocytes but not in other tissues. Hepatocytes expressed CuZnSOD at Day 14 and MnSOD at Day 17. By Day 18 CuZnSOD was detected in the epithelial cells of the gastrointestinal tract, respiratory tract, pancreatic islets, kidneys, and adrenals. These tissues exhibited MnSOD staining at Day 19. CuZnSOD occurred in the epithelia of the thyroid, thymus, and salivary glands at Day 19, while MnSOD was seen at Day 21. The increase in intensity of the staining for SODs occurred no later than postnatal Day 0, indicating that most tissues accumulated SODs during late gestation. Breathing atmospheric oxygen during early extrauterine life did not appreciably intensify the SOD staining. These results suggest that perinatal increase in SODs occurs as a general mechanism of preparation for birth.

Histogenesis of diisopropanolnitrosamine (DIPN)-induced tumors of the rat thyroid gland.

In an attempt to eludicate the histogenesis of thyroid carcinoma, histopathological and immunohistological studies were conducted on rat thyroid tumors induced by diisopropanolnitrosamine (DIPN). A total of 235 nodular lesions detected on serial sections of whole thyroid glands were classified into three types as follows: foci of cellular alteration at the single follicle level (type 1); multifollicular nodules considered to be malignant or potentially malignant (type 3); and multifollicular, proliferative nodules other than type 3, including both hyperplastic and neoplastic nodules (type 2). Analysis of the frequency of nodular lesions after various periods of time following exposure to DIPN revealed that the size of the nodules, their pattern of development, and their growth activity, as measured by the labeling index for bromodeoxyuridine (BrdU), showed sequential changes. Type 1 lesions appeared early; these lesions were the smallest and were considered to indicate the initial morphological change. Subsequently, type 2 nodules of intermediate size and labeling index, and finally type 3 nodules with the largest size and the highest labeling index developed; the latter type 3 nodules become more numerous during the latest phase following DIPN exposure. These observations support a multistage hypothesis of tumorigenesis.

Effect of diabetes mellitus induced by streptozotocin on renal superoxide dismutases in the rat. A radioimmunoassay and immunohistochemical study.

Two forms of superoxide dismutase, CuZn-SOD and MnSOD, have been investigated in the kidneys of streptozotocin-induced diabetic rats using both radio-immunoassay and immunoenzyme staining. The rats were killed 2, 8 and 12 weeks after the induction of diabetes mellitus and the kidneys excised. Two weeks after the induction of diabetes, the kidneys were hypertrophied because of the proliferation of renal tubular epithelium. However, the total CuZnSOD content of the kidneys did not increase and, because of the epithelial proliferation, the CuZnSOD concentration in each proximal tubular cell was decreased. Armanni-Ebstein lesions were found in the distal tubules 8 and 12 weeks after the induction of diabetes. The cells in these lesions were intensely stained for CuZnSOD, suggesting an adaptive response to the enhanced oxidative stress. The MnSOD staining in the thick ascending limbs of Henle's loops was enhanced in the diabetic kidneys, while that in the cortical tubules was unaltered. MnSOD was assumed to increase in response to hypermetabolism associated with the proliferation of renal tubules. This was most marked in the cells which were rich in mitochondria, again suggesting an adaptive response to enhanced oxidative stress induced by diabetes mellitus. The glomeruli of both the diabetic and control groups were not stained for SODs, and no significant microscopic change was found even 12 weeks after the induction of diabetes mellitus.

Genomic DNA analysis of rat retinal tumor induced by adenovirus type 12.

To elucidate the pathogenesis of human retinoblastoma, we investigated the genomic expression in retinal tumors induced by human adenovirus type 12 in rats, using various DNA probes. Seven rats received a single intraocular inoculation of concentrated virus fluid within 24 hours after birth. Intravitreous tumors were induced in two out of seven animals (28.5%) within 30 to 64 days after the inoculation. A remarkably uniform histologic feature, i.e., neuroblastic cells in association with Homer-Wright pseudorosettes, was present in all cases. The adenovirus-related oncoprotein gene E1A and human retinoblastoma susceptibility gene were detected in the tumors by Southern blot hybridization. In situ hybridization analysis demonstrated expression of adenovirus type 12 E1A gene in the inner granular layer of the retina. It was suggested that integration of adenovirus type 12 E1A fragment with the host genome and expression of the gene were required for induction of this tumor.

A case of lymphoma-type alpha-chain disease.

A 69-year-old man with a rare case of lymphoma-type alpha-chain disease was admitted to the hospital with marked cervical and inguinal lymph node swelling. Lymph node biopsy showed marked infiltration of plasma cells, plasmacytoid cells and immunoblastoid cells, alone or in combination. Immunoelectrophoresis and immunoselection identified alpha-chain in the serum and urine. The site of alpha-chain synthesis was extensively studied in the whole body, and the immunoperoxidase technique eventually demonstrated the presence of alpha-chains in the cytoplasm of infiltrating malignant cells in the lymph nodes. No infiltrating malignant cells were found in other organs or tissues, including those of the digestive and respiratory tracts.

Immunohistochemical study on perinatal development of rat superoxide dismutases in lungs and kidneys.

It has been reported that levels of antioxidant enzymes are low in fetal rat lungs and kidneys, and that they increase rapidly during late gestation. Among the antioxidant enzymes, both copper-zinc and manganese superoxide dismutases (CuZnSOD and MnSOD) are assumed to play a key role in protection against oxidative tissue injury. To determine the nature of the rapid perinatal increase in CuZnSOD and MnSOD, immunoenzyme staining was performed in the lungs and kidneys of fetal (d 18 and 20 of gestation) and neonatal (d 22) rats. The CuZnSOD and MnSOD in the homogenates were assayed by RIA, and they were found to be higher in the neonatal organs than in the respective fetal organs. The neonatal bronchiolar epithelium was stained for both CuZnSOD and MnSOD more intensely than the fetal one. The CuZnSOD staining in the neonatal alveolar wall was more intense than that in the fetal one. There was a significant reactivity for MnSOD in the neonatal, but not in the fetal, alveolar walls. In the kidneys, the reactivities for CuZnSOD and MnSOD were confined to the undifferentiated tubules. Although the tubules were increased in numbers in the neonatal kidneys, the intensity of the staining for both CuZnSOD and MnSOD was unchanged. The histochemical study disclosed that CuZnSOD and MnSOD increased in the kidneys in a manner different from that in the lungs. The low concentration of both CuZnSOD and MnSOD in the fetal lung tissues may contribute to the vulnerability to oxygen toxicity. Such changes in the concentrations in specific tissues were not delineated in the kidneys.

Age Factors

Localization of urinary procoagulant in the human kidney.

By the indirect immunofluorescent and immunoenzymatic techniques with monospecific antiserum against urinary procoagulant (a tissue factor which accelerates blood coagulation), we found the urinary procoagulant in the kidney distributed to the loop of Henle and distal convoluted tubules. In these areas urinary procoagulant was found in association with the luminal and intercellular borders as well as in the cytoplasm of epithelial cells. Both the descending and ascending limbs of Henle were equally stained. The cytoplasmic staining was patchy in distribution among cells of distal tubules and was predominantly localized in the supranuclear areas. Glomeruli, the proximal tubular cells, the vascular wall, and the interstitium were not stained. There was, however, fluorescent staining along the epithelial layers of the Bowman's capsule, which was observed only in the frozen sections. Casts in the distal tubules were also positively stained. These findings suggest that urinary procoagulant is synthesized in the epithelial cells of these particular parts of nephron and is secreted into urine, although its physiologic roles and pathologic significance are not entirely known.

Glomerular lesions in multiple myeloma.

An autopsy case of multiple myeloma (IgG, lambda type), clinically characterized by decreased glomerular filtration rate, is reported with particular emphasis on changes in the glomeruli of kidneys. Histologically, the glomeruli revealed slight increase in mesangial matrix and focal thickening of tuft capillary wall. Electron-microscopically, deposits were observed in a subendothelial location in the glomerular capillary walls, and inclusions were noted in the cytoplasm of the visceral epithelial cells. Histoimmunofluorescent study of the kidney demonstrated intense focal and slight diffuse positivity against labelled antisera of anti-IgG and anti-lambda type of light chain on the capillary wall of the glomerular tufts. Other immunoglobulins were not demonstrable in capillary walls. These findings represent the intraglomerular deposition of paraprotein of multiple myeloma without amyloidosis.

Immunohistological study on the ACTH producing islet cell carcinoma of the pancreas.

ACTH producing cells in islet cell carcinoma associated with Cushing's syndrome were studied by an improved immunoperoxidase as well as immunofluorescence technique. The cells which contained ACTH antigen were distributed throughout the tumor tissue of both primary and metastatic lesions in rather small numbers. In the small cancer nests they were present in an irregular or mosaic pattern and, on the contrary, they tend to line along the margin in the larger nests. The intracytoplasmic localization of the ACTH antigen was characteristically demonstrated in the infranuclear area of the tumor cells, closely simulating that of the argentaffin granules of the enterochromaffin cells. The electron microscopic observation identified membrane enclosed, cored granules, 200-300 nm in diameter, present in clusters in the infranuclear area of some tumor cells, which were considered corresponding well to the ACTH positive cells in immune staining.

Transmission and scanning electron microscopic observations of mouse adrenocortical adenoma cells (Y-1) in non-stimulated and stimulated states.

ACTH sensitive murine adrenocortical adenoma cells (Y-1) were examined by transmission and scanning electron microscopes under non-stimulated and stimulated conditions. They showed the ultrastructural characteristics which were fairly different from those of the original adrenocortical cells of LAF1 mice. The smooth-surfaced endoplasmic reticulum (sER) were poorly developed, and the mitochondrial cristae showed lamellar rather than tubular or tubulo-vesicular type. Administration of ACTH induced rounding-up of the Y-1 cells with slight increase in sER, BUt no mitochondria with vesicular inner structure appeared. The effects of dbcAMP on the morphology of the Y-1 cells were similar to those of ACTH except that the rounding-up occurred more rapidly and uniformly.

Development of an enzyme-linked receptor assay (ERA) for hCG.

An enzyme-linked receptor assay (ERA) for hCG was developed using horseradish peroxidase. The 1,500 g pellets, interstitial cell fraction, and solubilized homogenate from the rat testis linked to tanned sheep red blood cells (SRBC) were used as the binding fractions for hCG. In these ERA systems, the limit of detection for hCG was 30 IU/l (3-12 mIU/tube), which was almost equal to that of radio receptor assay (RRA). The ERA using SRBC linked solubilized receptor fraction showed the most satisfactory result in accuracy, reproducibility and easiness.

Primary choriocarcinoma of the lung: case report of two male subjects.

Extragenital choriocarcinoma in the male is known to occur in retroperitoneum, mediastinum and pineal body in a high proportion, and there have been no certain reports of lung origin. We studied two cases of choriocarcinoma arising from the lung in the male by histological, electronmicroscopical, immunofluorescent, and immunoenzyme methods. Histologically, characteristic changes of choriocarcinoma, composed of irregular complex of cytotrophoblasts and syncytiotrophoblasts with massive necrosis and hemorrhage were demonstrated. These two cell patterns, characteristic of choriocarcinoma, were also demonstrated electronmicroscopically and differed from giant cell carcinoma of the lung. By immunofluorescent and immunoenzyme methods, anti-HCG reactive particles were seen in the cytoplasm mainly of syncytiotrophoblasts and proved HCG production function of the tumor.
