[Chemotherapy for metastatic or recurrent laryngeal cancer: tolerance and early results].

In the Head and Neck Cancer Department of Cancer Centre in Warsaw between 1994 and 1998 fifty three patients with recurrent or metastatic laryngeal cancer were treated with methotrexate-based chemotherapy. Chemotherapy protocol consist of methotrexate, vinblastine, 5-fluorouracil, bleomycin, cyclophosphamide and steroids used every two weeks. Before treatment, unresectable local recurrence was observed in 35 patient and distant secondaries in 18 others. Tolerance of treatment was acceptable. Partial regression of the tumor was obtained in 30% of patients. Improvement of the quality of life was observed in 66%. Presented program is an effective alternative to supportive treatment in patients with recurrent or metastatic laryngeal cancer.

[Combined modality treatment with chemotherapy followed by surgery with immediate reconstruction and irradiation for primary unresectable oral cavity cancer].

In the Head and Neck Cancer Department of Cancer in Warsaw between August 1997 and September 1998 twenty patients with primary unresectable oral cavity cancer were referred for combined modality treatment with cisplatin based chemotherapy followed by wide resection (in the case of tumor regression) with immediate reconstruction and adjuvant radiotherapy. Tumor regression after chemotherapy was obtained in 15 patients and all of them were referred to surgery; wide resection and reconstruction using pectoralis major (PM) flap. 14 patients completed all treatment protocol, also with adjuvant radiotherapy. Tolerance of treatment was excellent and there was no influence of chemo- and radiotherapy for adaptation of PM flaps. During observation local recurrence was observed in two cases. Twelve patients are alive without evidence of disease. Study is continued.

[Concomitant radiochemotherapy for locally advanced oro- and hypopharyngeal cancer: tolerance and early results].

Combination of radio- and chemotherapy is one of the methods which may improve results of treatment in patients with locally advanced head and neck cancer. Currently, the most promising sequence of radio- and chemotherapy is concomitant. In Head and Neck Cancer Department of Cancer Center in Warsaw since 1995 is continued study estimated tolerance and effectiveness of the concomitant radiochemotherapy for patients with locally advanced oro- and hypopharyngeal cancer. Chemotherapy consist of cisplatinum and 5-fluorouracil in continuous infusion during first week of irradiation followed by 24-hours infusion of 5-fluorouracil one weekly until radiotherapy is finished. Radiotherapy is used with conventional fractionation 2 Gy per fraction, 5 fractions weekly to total dose 66 Gy. Between October 1995 and September 1998 fifty seven patients with oropharyngeal cancer were entered to study. Tolerance of treatment was acceptable. Complete regression of the tumor was obtained in 41/57 patients (72%). Five other patients after radiotherapy were referred for successful radical neck dissection. Including this group, local control was obtained in 81%. At this moment, 60% of patients are alive without evidence of disease. In 12 patients with hypopharyngeal cancer tolerance of treatment and early results were poor, so the study was stopped in this group.

[The effects of anti-inflammatory treatment in patients treated with chemotherapy for advanced head and neck cancer].

The aim of the study is to analyse the effect of anti-inflammatory treatment in patients treated with chemotherapy for advanced head and neck cancer. 110 patients treated in the Head and Neck Department in CO in Warsaw were entered to study. Pain, dyspnoea, inflammatory reaction foetor ex ore, fever were assessed. Clinical response was positive in 67% of patients, microbiological improvement was positive in 44% and subjective response in 68% of patients.

[Olfactory neuroblastoma: own experiences].

The authors present material of 12 cases olfactory neuroblastoma observed in Head and Neck Department of Cancer Center in Warsaw from 1961 to 1998. The tumors were seen in 8 women and 4 men. According to Kadish staging there were one A, three B, and eight C stages tumors. Patient in stage A was treated with surgery and radiotherapy. One patient in stage B was treated with surgery and radiotherapy. Two patients in stage B were treated with radiotherapy alone. Six patients in stage C were treated also with surgery, radiotherapy and chemotherapy. Two patients in stage C were treated with surgery and radiotherapy. Complete regression was observed in 9 of 12 patients. One patient died from olfactory neuroblastoma, two other were died from other reasons.

[Evaluation of the results of radiotherapy of non-small cell lung cancer by the method of high fractionated doses with planned intervals].

The number of new cases of cancer of the lung in Poland increases systematically in connection prevalence of the tobacco smoking and increase of the natural environment pollution. However the percentage of patients qualified for radical surgical treatment is not increasing since the diagnosis of pulmonary cancer is extremely late. Such situation causes a dynamically increasing demand for palliative treatment. Radiotherapy is a method of choice for such treatment. In order to provide the treatment for possibly large group of patients in the situation of restricted availability of adequate therapeutic equipment it is necessary to propose such method of radiotherapy as fulfils the requirement of palliative therapy, with short period of hospitalization, not overburdening the therapeutic equipment. The method of irradiation with high fractionated doses and planned interval in shortened time is a method fulfilling those demands applied in the Radiotherapy Clinic of the Oncology Center in Warsaw. Results of treatment in our Clinic and in other centers are at least comparable with other radiotherapeutic programmes, and the method is worth recommendation.

[Radiotherapy of metastases to the brain by the method of high fractionated doses with planned intervals].

Results of treatment by irradiation with CO60 of metastases to the brain applying the high fractionated doses with planned interval method in the group of 32 patients, are presented. The treatment has been given in two stages; at the first stage 20 Gy has been given to the total brain in 5 fractions; after three weeks interval the response to treatment has been evaluated, and patients with positive response received second course of irradiation analogous to the first. Patients received as a total 40 Gy in 10 fractions with three weeks interval. The positive response to treatment has been obtained in 25 (78%) cases. Medium survival in the analysed group amounted to 7.5 months, 6 patients survived one year following the treatment (18.7%). Tolerance of the treatment has been good. No serious complications have been observed following the treatment.

[Elective (prophylactic) brain irradiation in small cell lung cancer patients treated with chemotherapy combined with surgery].

The incidence of brain metastases in small cell lung cancer patients in the group without prophylactic cranial irradiation (PCI) was compared with the incidence of brain secondaries after PCI. Out of 29 patients without brain irradiation 13 patients experienced brain metastases at 19 months (13/29). On the contrary, no one out of 17 patients who received PCI was found to have brain metastases at 4 years. The justification of prophylactic cranial irradiation is presented and criteria of patients selection are given. Elective brain irradiation is indicated exclusively in patients with "limited disease" achieving complete response.

[Analysis of early treatment results in patients with locally advanced breast cancer].

Results of treatment of 119 patients with locally advanced breast cancer in the III-rd and IV-th stage of clinical advancement previously treated with conventional radiotherapy (250 kV, HVL-3.0 mm Cu) have been retrospectively analysed. In 39 patients radical mastectomy has been performed following irradiation. In the total group 36.6% of complete 5 year survivals and 18.5% of symptomless survivals (median of survivals 28.2 months) has been obtained. Operated patients presented significantly better prognosis (56.0% s. 26.9% of 5 year of total survivals). Main cause of treatment failures has been the appearance of remote metastases (76.5%). Unfavourable prognostic factors appeared to be major enlargement of lymphatic nodes (over 3.0 cm), their immobilization and absence of significant regression of primary tumor following irradiation. Size of the primary tumor and age of patients did not significantly influence the result of treatment.
