Hypomanic behaviour associated with familial spastic paraplegia.

Psychiatric manifestations of familial spastic paraplegia are rare and have been described only infrequently. A 35-year-old male is reported, who presented both hypomanic behaviour and gait disturbances as features of a previously undiagnosed familial spastic paraplegia. This association implies that the CNS manifestations of familial spastic paraplegia may overlap with the neurochemical or neuroanatomic substrata regulating mood.

Comparison of brachiocephalic angiography and IVDSA in the same group of patients.

In this investigation for the first time conventional angiography (CA) and intravenous digital subtraction angiography (IVDSA) of the aortic arch and brachiocephalic vascular territory is compared in the same group of 25 patients. The quality of the image derived from IVDSA is inferior to that of CA and, moreover, varies with the location. If sufficiently good, IVDSA has an accuracy of 81 to 91% in comparison to CA for stenotic lesions depending on the site. Ulcerations are missed more frequently than with CA. Discrepancies between IVDSA and CA are caused mainly by misregistration. According to our investigation there are multiple stenotic lesions in the brachiocephalic territory in 84% of the patients. Adequate assessment of the need for surgery in symptomatic patients can therefore only be made after complete visualization of the aortic and brachiocephalic vascular territory. If non-invasive methods, like duplex-scanning, cannot fulfill this requirement, IVDSA is the only semi-invasive method that can provide the information. It is therefore desirable to improve IVDSA technique in the near future instead changing to intra-arterial DSA (IADSA). Now already the advantages outweight the drawbacks.

Multiple subcortical haemorrhages following lumbar metrizamide myelography.

The case is presented of a patient showing multiple subcortical haemorrhages after lumbar metrizamide myelography. This complication after intrathecally administered metrizamide contrast medium appears not to have been reported before. Several different possible explanations are proposed for the phenomena observed in this case.

Bilateral direct carotid-cavernous fistulas of traumatic and spontaneous origin: two case reports.

In the present paper we discuss two cases with bilaterally located carotid-cavernous fistulas: one of traumatic origin and the other occurring spontaneously. Most traumatic fistulas are direct, unilateral and of a high flow type. In our first case we deal with traumatic bilateral direct fistulas with a low flow, a combination which must be considered rare and is not reported in the literature so far. In our second case there are bilaterally located fistulas, occurring spontaneously. Peculiarly they are direct, instead of indirect as is to be expected in spontaneously developing fistulas, and of a low flow type. This combination too must be considered extremely rare. In both cases conservative therapy was successful, despite their difference in etiology. It appears that neither the anatomy nor the manner of onset determines the success of conservative therapy, but the fact that a fistula is of a high- or low flow type.

Paragrammatic speech without a comprehension deficit? A case report.

In this article we present a case report of a patient, who had a peculiar pattern of language use following a stroke. The two most prominent features were language switches during spontaneous speech in the first months postonset, when the patient mixed English, German, and French words and utterances into his mother tongue (Dutch); clearly disturbed speech output, showing signs of word-finding problems and paragrammatism but hardly any paraphasias, while at the same time no evidence of a language disturbance could be obtained with standard clinical aphasia tests.

Phase I study of recombinant human interferon alpha-2C in patients with chemotherapy-refractory malignancies.

Twenty-two patients with advanced haemopoietic and other malignancies were treated intramuscularly with recombinant interferon-alpha 2C in a daily escalating dose. The most common side-effects were flu-like symptoms. Two patients showed severe neurotoxicity, which was completely reversible in 1 case. Doses above 30 X 10(6) IU/day were poorly tolerated and could only be achieved in a minority of the patients. Objective tumour responses were documented in malignant lymphomas, hairy cell leukaemia, and renal cell carcinoma.

Basic aspects of development and maturation of the brain: embryological contributions to neuroendocrinology.

The interpretation of studies on the development of neuroendocrine function presupposes a thorough knowledge of the complementary phenomena of morphogenesis and histogenesis of the brain. A short analysis of the morphogenesis of the diencephalic floor is given. The pituitary Anlage can be identified early in the neural plate stage. The hypophysis cerebri appears to be a key structure in the morphogenesis of both the head and the brain. The spatiotemporal pattern of histogenesis within the brain can be analysed by a study of the proliferative activity of the neuroepithelial matrix cell layer; a heterochrony of matrix (ventricular) layer mitotic activity and of mantle (intermediate) layer differentiation can be demonstrated. The process of neuron differentiation shows an articulate sequence of phenomena, among them migration, axon growth, dendrite growth, synapse formation and myelination. Dendritogenesis and the development of synapses in a particular area are strongly influenced by the ingrowth of axon nerve terminals from elsewhere. A number of structures observed in the developing central nervous system are only temporary phenomena that go into regression during subsequent stages; dendrites and synapses especially show a high degree of plasticity. Cell death occurs as a normal concomitant of development. Monoaminergic neuron systems originate early and show a positive histofluorescence shortly afterwards; their target areas are retarded as far as differentiation is concerned. The development of these target regions probably is influenced by the monoaminergic cells. The monoaminergic neurons are not subject to feedback regulation for some time, because of the typical late development of the dendritic receptive apparatus in these cells. Steroid receptors may play a role in the development of intersexual dimorphism of the brain. Probably a modulation of neurotransmitter synthesis is the intermediate between steroid receptor stimulation and a change in synaptogenesis in the target area of the neuron. Neuropeptide systems appear to possess a distribution beyond the limits of the hypophysiotropic area. The early presence of some of the neuropeptides within the embryonic brain suggests a role in histogenesis that is different from the usually presumed neurotransmitter or neuromodulator function of the neuropeptides.

Morphology of the cervical spinal cord on computed myelography.

To ensure adequate use of the technique of computed myelography (CM) it is necessary to have an exact picture of the morphology of the normal spinal cord as demonstrated by this technique. This has been obtained by studying the morphology and measuring the frontal and sagittal diameter of the cervical cord in 20 patients. The normal values are presented. The changes of this morphology in one patient with a tumour, one patient with atrophy of the spinal cord and in some patients with congenital malformations are also reported.

Some effects, in baboons, of chronic ingestion of glycerol with sucrose or glucose.

Six mature male and six mature female baboons were given fat-free diets high in sucrose or glucose together with glycerol, a chow diet with glycerol, and a fat-free high sucrose diet without glycerol. In the male animals, the concentration of triglyceride, cholesterol, glucose, and glycerol in fasting serum was significantly greater when sucrose and glycerol were given together compared with the other diets. The fasting serum triglyceride and cholesterol were significantly greater in the females on the sucrose + glycerol diet compared to the glucose + glycerol diet. It is suggested that glycerol and fructose (in sucrose) share a metabolic pathway that leads to an enhancement of serum lipid concentrations.

Madopar versus sinemet. A clinical study on their effectiveness.

Fifteen well-documented Parkinson patients treated with Madopar were switched to Sinemet for 3 months. The results were measured with an objective test procedure and compared. The effects of Madopar and Sinemet were equal provided th amount of levodopa was the same. There was one drop-out, because of ventricular extrasystoles. Attention is paid to the occurrence of involuntary hyperkinesias and the on-off-effect.

An investigation in rats, into the metabolic consequences of early ingestion of dietary carbohydrates.

Weanling rats were given diets containing a high proportion of either glucose, sucrose or fructose until they reached 200 g weight, when they were either transferred to a control (chow) diet for 4 weeks or killed. The results were compared with rats receiving a control diet throughout. The triglyceride (TG) and cholesterol levels in liver and in serum had returned to values similar to the control animals 4 weeks after being given the control diet. The fatty acid profile of liver TG and adipose lipid tended to take longer to return to normal.
