Altered functional activity and anoxic tolerance in diabetic rat isolated atria.

Functional activities of isolated atria from 4-week diabetic rats and their anoxic tolerance and recovery were compared to those of control rats. The present study showed a two-fold increase in maximum developed contraction force (CF), the rate of force development, and the rate of relaxation in isolated atria from diabetic rat heart over that of the control. The rate of the sinoatrial node was lower in the diabetic. However, this lower rate was not responsible for higher CF of the diabetic isolated atria since the twofold difference was still present when both diabetic and control atria were electrically paced at identical rates. Although the high energy phosphate content of the diabetic isolated atria was lower than that of control (50% lower phosphocreatine and 15% lower ATP), diabetic atria exhibited improved short term anoxic tolerance and recovery compared to that of the control. The recovery from long term anoxia, however, was better in control than in diabetic atria, considering the higher original CF in diabetic atria. The present observations are compatible with better ischemic tolerance and higher basal force development observed by some investigators on diabetic isolated heart preparations but not with impaired contractility reported by others. Possible mechanisms responsible for the observed changes are discussed.

Variations in occlusal forces with a resilient internal implant shock absorber.

The importance of an internal shock absorber for implant prostheses has concerned many, primarily because the efficacy of a shock absorber to reduce stress has not been demonstrated in vivo. This study examined 15 subjects with unilateral distal extension single implant abutments attached to a single natural tooth abutment to assess the effect of a shock-absorbing element within the implant on occlusal force levels. Occlusal forces and bilateral occlusal similarity were examined using specially adapted research software for the T-Scan occlusal analysis system. The results indicate that there is a significant difference (P less than 0.011) between occlusal forces using an internal shock absorber (18 N) and a titanium internal element (30 N).

The IMZ osteointegrated implant system.

From September 1978 to September 1988, a total of 3088 IMZ implants have been placed in 1401 consecutive cases in our clinic to restore partially or totally edentulous arches. Of the total number of implants placed, 334 implants, or 10.8 per cent, were lost to follow-up (patient death, relocation, so forth). If these implants are excluded from the implant population database, 2754 IMZ implants continue to be followed in routine recall examination. During the reporting period, a total of 61 IMZ implants were removed, primarily because of soft-tissue complications. Therefore, the overall implant rate of success of the implants placed and routinely followed over this 10-year period is 97.8 per cent.

[Psychological and social behavior of females following radical operation of breast cancer].

55 women having had radical mastectomy were interviewed. In 40 of them a continuing anxiety could be shown to exist. Grief, consternation about the loss of attractivity and disfigurement of the body could be found in 50 of them. The loss of self confidence may be followed by disturbances of sexuality, marital and interpersonal relationships. Reintegration into social live is often difficult, relations with the social environment being interrupted. In the course of the patient's preoperative information these emotional disturbances must be discussed by the surgeon. 17 of these patients were interviewed again after successful breast reconstruction. In all of them psychological reactions could be found to be lowered and there was a rise of self confidence, too. Therefore rehabilitation of women after mastectomy must be considered to be an important part of treatment. "Today and in future surgery will include more than mere operations."

[Titanium spray-layered human tooth root implantation under physiological stress].

Titanium-plasma-flame-spray coated endodontic stabilizers have been implanted in six patients after apicectomy. These endodontic stabilized teeth have been postoperatively under physiological stress. Histologically in one case there was six month post-operatively bone in direct contact to the implant surface and in certain areas the structure of a new periodontal membrane was formed.

[Proteinase inhibiting agents and glucagon in acute pancreatitis (author's transl)].

In two prospective studies the effect of proteinase inhibiting agents and glucagon in acute pancreatitis have been studied by a randomised series. Moreover the effect of the basic therapy was examined in additional 103 patients. Symptoms, clinical and laboratory chemical findings correspond to each other in all collectives. The results seem to be the best ones in those patients having been treated by basic therapy only.

[Duration of disablement after general-surgical operations (author's transl)].

As a result of cooperation existing between Hospital and out-Patient Department the offwork time required before hospitalisation for routine operations can be limited to two days. A necessary preparation of the patients by remedies and physiotherapeutic methods should be done without interrupting their professional duties. The offwork time after the operation will depend on the kind of the surgical intervention, on the age and the condition of the patient and his profession. Data, obtained from a large number of patients treated in the Surgical Department of the Friedrichshain-Hospital in Berlin are discussed.
