[Atypical entrapment syndrome of the distal suprascapular nerve of the incisura scapulae. Case report and overview of the literature].

Entrapment neuropathy of the suprascapular nerve is a rare disease of the peripheral nerve system. Of Mumenthaler's 3465 patients with peripheral nerve lesions, only 15 had suprascapular nerve damage. Since Thompson's and Kopell's first description of suprascapular entrapment neuropathy in 1959, some reports have been published. In the majority of cases the lesion is normally located in the suprascapular notch with clinical involvement of the supra- and infraspinatus muscle. Here we report on an uncommon lesion of the suprascapular nerve with sparing of the supraspinatus muscle in a volleyball player. The current literature will be reviewed. The anatomical basis, examination, differential diagnosis and therapy of this type of entrapment neuropathy will be discussed.

Ultrasonographic evaluation of Legg-Calve-Perthes disease based on sonoanatomic criteria and the application of new measuring techniques.

This study attempts an ultrasonographic classification of Legg-Calve-Perthes disease according to sonoanatomic and sonometric criteria. Four stages, similar to standard radiological staging, could reproducibly be found in 29 patients with unilateral disease. The stages could be significantly differentiated according to descriptive sonoanatomic criteria and partially newly introduced sonographic measurements, including epiphysis-metaphysis-index and acetabulum-epiphysis-distance. Sonographic evaluation of Perthes hips is a simple and standardised procedure, which can serve to stage and monitor the course of the disease and hence reduce the amount of X-ray exposure and treatment costs.

Evaluation of liver function in an improved model of isolated liver perfusion.

A perfusion system was described for maintaining isolated guinea pig livers in a stable functional state for five hours. Oxygen supply, oxygen consumption, as well as perfusate flow were within physiologic limits. Liver function was investigated by means of indocyanine green elimination and galactose elimination. Both test substances showed no significant decrease in liver function throughout perfusion. Isolated liver perfusion is a useful tool to investigate problems in organ preservation and liver transplantation.

[Extracranial metastasis of meningeal tumors. Malignant meningioma with regional lymph node metastasis].

Metastases of a meningioma occurred in a 45-year-old man 4 years after removal of a falx meningioma and 1 year after removal of a malignant relapse. They were situated in the cervical, nuchal, and supra-clavicular lymph nodes. The review of 63 cases of metastasising meningial tumours from the literature, shows that lymph nodes are the most frequent site of metastases after lung and liver. The spreading of the tumour into the regional lymph nodes requires a penetration of the primary tumour into the galea, where the tumour cells may reach the lymphatic system. According to the literature, lymph node metastases are caused by benign as often as by malignant meningiomas, the majority being cases who underwent operation. Discussed, are the different theories as to why intracranial tumours only rarely show extracranial metastases.
