Twelve vascularized iliac crest free-tissue transfers were used for oromandibular reconstruction, 2 as osteocutaneous and 10 as osseous flaps. Two patients had preoperative radiotherapy. Nine of the 12 reconstruction were carried out primarily, following radical surgery for oral cancer. One graft was lost by venous thrombosis. Evidence of bone union was noted in 11 patients, and no pseudoarthrosis was observed. The main advantage of this method is the availability of a large bloc of living composite tissue and both the functional and cosmetic results should be improved in patients undergoing resection of the anterior part of the mandible.

The authors have treated 37 pancreatic pseudocysts in 36 patients by ultrasonically guided percutaneous puncture and drainage. The indication for intervention was rapid enlargement or threatening rupture of the acute (younger than 6-8 weeks) pseudocysts with recurring fever in 5 cases and chronic pseudocysts causing abdominal symptoms in 32 cases. After puncture six of seven patients were recuperated, but later in three of them the pseudocysts recurred. In one case as a complication of the puncture the pancreatitis exacerbated and led to death of the patient. The outcome of percutaneous drainage was successful in 26 of 32 pseudocysts. During the follow-up (6-54 months) recurrence were observed without symptoms in three cases. In four patients the treatment was ineffective, therefore cystogastrostomy was performed. During the time of the percutaneous drainage two pseudocysts had been infected and abscesses developed, which were treated by oncotomy. On the basis of their favourable experiences the authors recommend the US-guided percutaneous durable drainage as primer intervention for treatment of the pancreatic pseudocysts causing symptoms.
The effects of different doses of calcitonin gene-related peptide (CGRP) on passive avoidance behaviour were studied in male rats following its intracerebroventricular (i.c.v.) administration. Treatment with doses of 200 ng, 300 ng, 500 ng or 1 microgram was performed prior to learning, immediately and 6 h after the learning session and 30 min before the 24 h retention test. CGRP enhanced the avoidance latency in a dose-dependent manner at each time studied, with the exception of 6 h after the learning session. It is concluded that CGRP might be able to lengthen the passive avoidance latency by facilitating learning and memory formation.
The rabbit was used as a model for nuclear transfer. A critical step in nuclear transfer is oocyte activation, which was evaluated in this research. Optimal field strength of an electric stimulus for activation was examined. A significantly higher activation rate in all criteria tested was achieved when oocytes were activated electrically with a field strength of 2.4 kV/cm versus 1.2 or 1.8 kV/cm. Also, electrical stimulation with combined alternating current (AC) and direct current (DC) was superior to DC stimulation alone for activation. In another study involving 586 oocytes, exposure of oocytes to cytochalasin B for 1 h followed by activation with electrical stimulation significantly improved development of the oocytes to blastocyst stage compared to oocytes without cytochalasin B pre-exposure (38% vs 26%, p less than 0.05). Cytochalasin B exposure alone (control), however, had no effect on activation. Exposing oocytes to activation medium without electrical stimulation also activated some oocytes. In the nuclear transfer experiment, blastomeres from 8-cell embryos cultured for 20-24 h to the 32-64-cell stage were used as nuclear donor cells. Of 491 oocytes used, 459 (93%) survived the enucleation and fusion procedure, 370 (81%) fused, and 284 (77%) developed into 2-4-cell embryos. A total of 243 of these 2-4-cell embryos were transferred to 15 pseudopregnant recipients and produced 8 young (3%). Although the efficiency is low, this study demonstrated that rabbit morulae cultured for 20-24 h to the 32-64-cell stage as nuclear donors for transfer remain totipotent.
Somatic chromosome analysis of a subfertile Brown Swiss bull demonstrated a three-breakpoint translocation involving chromosomes 1, 8, and 9 in G- and R-banded karyotypes. Based on standard bovine chromosome nomenclature, the translocation was defined as t(1;8;9)(q43;q13;q26). Synaptonemal complex analysis of the chromosome aberration by electron microscopy revealed a hexavalent configuration in 52 of 53 pachytene cells. Twenty-seven cells (51%) had a completely paired hexavalent configuration showing distinctly nonhomologous pairings between normal and/or translocated chromosomes involved in the exchanges. Thirteen cells showed a hexavalent configuration with centrally unpaired chromosome segments but with completely paired terminal arms. In 13 cells (including one at zygotene) the translocation chromosomes formed an open hexavalent, and in one cell there were two completely paired trivalents. Thirty-two cells at diakinesis-MI demonstrated 28 configurations, including one large hexavalent. Testicular histology, testis size, and seminal characteristics were normal.
A practical and reliable staining procedure was developed to distinguish the viability and acrosomal status of bull, boar and rabbit spermatozoa. The first stain with trypan blue or Congo red is rapid and avoids artifacts. This stain is precipitated by neutral red during the 2 min required for fixation. The precipitate gives a high contrast black color, resistant to the subsequent rinsing and persists during the time required for staining the acrosome with Giemsa. Ten classes of spermatozoa are distinguished (live or dead with intact acrosomes, loose acrosomes, damaged acrosomes, no acrosome, or with no acrosome and no postacrosomal ring). The intact acrosomes are purple, the loose acrosomes are dark lavender and the damaged acrosomes are pale lavender. The anterior part of the head of live spermatozoa with no acrosome is white or light pink and the same area of dead spermatozoa is white or pale gray. The postacrosomal ring is red. The postacrosomal area of the head of live spermatozoa is white or light pink and the same part of dead spermatozoa is black, dark violet or gray. The procedure did not give satisfactory results for stallion spermatozoa.
The authors make known the indications and method of the free osteocutaneous forearm flap. They have established from the literature data, and by their own experience of 4 cases, that the use of free osteocutaneous forearm flap seems to be the most ideal method when the bone defect is not bigger than 6-8 cm and only the replacement of mucosa is required.
Free radial forearm flap was used to replace the tissue defect caused by pull-through operation for cancer of the mouth in 13 cases. There was no total or partial flap failure. The greatest advantage of the method is that the restriction of the tongue movement is less when compared to the results of other methods since the thin forearm flaps are well adaptable and are of a pliable nature. The newly developed lower alveolar crest makes the prosthetic rehabilitation possible.
The first paper on the beneficial effect of selegiline in the treatment of Parkinson's disease was published 17 years ago. In the first ten years several clinical studies were conducted world-wide to prove its efficacy as an adjuvant to the basic 1-dopa therapy. Although the design, duration and number of treated patients were different, the overall results were mainly uniform. The patients treated with 1-dopa selegiline combination showed marked improvement in disability, increasing duration of 1-dopa effect and marked lessening of dose-related fluctuations were seen, compared to those treated with 1-dopa alone. It became also generally evident that selegiline allows a 10-30% decrease in 1-dopa dose. The investigations in which selegiline has been used as monotherapy have been triggered by some purely clinical experience and by the discovery of pathobiochemical mechanisms of MPTP toxicity which causes fairly similar clinical picture to Parkinson's disease and can be successfully prevented by selegiline. The newly diagnosed parkinsonian patients treated with selegiline alone showed marked improvement in their clinical state. Beside the actual clinical effect on disability, selegiline slowed down the progression of Parkinson's disease. The latter fact has been proven in the largest clinical trial ever done with selegiline (DATATOP). Selegiline monotherapy can delay the need for 1-dopa substitution by around one year or longer. Based on the continuously growing clinical experiences it became widely accepted that selegiline is the drug of choice as initial treatment for newly diagnosed parkinsonian patients.(ABSTRACT TRUNCATED AT 250 WORDS)
The authors state their experiences using trapezius osteomusculocutaneous flaps in 22 clinical patients for primary reconstruction of mandibular defects. The additional investigative study examines the anatomy of 100 osteomusculocutaneous flaps in 50 fresh cadavers. 78 percent of the major vascular pedicle arose form the thyreocervical trunk. The advantages of this method are as follows: 1. This flap provides a well-vascularized bone graft; 2. The operation comparing to other bone grafting methods is simple; 3. After radical neck dissection the trapeziu muscle protects the carotid artery; 4. The vascularized bone graft can be used after irradiation; 5 Good functional and esthetic results can be obtained.
Individually inactive N- and C-terminal fragments of the m5C-methyltransferase M.BspRI can complement each other resulting in specific, in vivo methylation of the DNA. This was shown by cloning the coding regions for N- and C-terminal parts of the enzyme in compatible plasmids and co-transforming them into E.coli cells. The enzyme could be detached at several different sites, producing either non-overlapping or partially overlapping fragments capable of complementation. Reconstitution of the active methyltransferase from inactive fragments was demonstrated in vitro, as well. Another GGCC-specific methyltransferase, M.BsuRI, showed a similar complementation phenomenon. Moreover, interspecies complementation was observed between appropriate fragments of the two closely related enzymes M.BspRI and M.BsuRI. Fragments of structurally and functionally more different methyltransferases were unable to complement each other.
Drug therapy for inflammatory bowel disease was limited for many years to sulfasalazine and some corticosteroids. Since the discovery that 5-aminosalicylic acid (5-ASA) is the active moiety of sulphasalazine in the treatment of chronic inflammatory bowel disease, several new drugs have been developed. These consist either of slow- or delayed-release formulations of plaint 5-ASA (mesalazin) or sulpha-free azo-compounds of 5-ASA. The varying bioavailabilities of the different formulations and compounds make it possible to use them alternatively in different clinical situations. A review of the literature is given and suggestions as to how the new drugs can be used in different diseases.
A case with a tumor developed in an undescended testis diagnosed by arteriography is shown. Radiological diagnostic procedures in the diagnosis of cryptorchism are discussed. It is established that selective testicular arteriography is of great value in the preoperative diagnosis of an intra-abdominally located tumor developed in an undescended testis even in the area of modern high resolution non invasive imaging modalities.
On the basis of the treatment of 1400 patients who suffered from kidney and ureter stones we may conclude that 80-82% of the extractable stones can be treated with ESWL monotherapy. ESWL proved to be successful in almost every case where there were 2 or 3 cm large--or larger--kidney stones or stones at almost any part of the ureter. The extraction of the larger kidney stones can be helped with stent put up previously, nephrostome or repeated ESWL. We apply other invasive methods more and more rarely. The aiming and treatment of ureter stones which can be found at different parts of the ureter is better with adjuvant laying and diagnostic methods. In the case of staghorn calculi ESWL can be combined with percutaneous operations. In the case of large staghorn calculi traditional operations seem to be the most successful methods after which percutaneous treatment or ESWL can be performed if there are fragments left behind.
ESWL treatment for renal calculi was used in 19 occasion in 16 children aged 6-15 years. 13 children were stone free 3 months after the treatment. All of the stones were located in the renal calices or in the renal pelvis. The possibilities of the procedure, as well as its effects were examined with the use of urography, ultrasound, isotop studies and magnetic resonance imaging.
