[Sneddon syndrome].

There is presented a case a 42 year old woman, who was admitted to the Department of Internal Diseases, the Institute of Dentistry, Medical Academy in Warsaw, with suspected bacterial endocarditis. Two episodes which indicate a lesion in the central nervous system (as right side hemiparesis and mixed aphasia) in the patients with valvular heart disease, were the basis of this suspicion. Bacterial endocarditis was not confirmed during hospitalisation. Diagnosis of Sneddon's syndrome was established based on skin lesion of livedo reticulatis type with typical picture in skin biopsy and on the lesions in the central nervous system confirmed by MRI. Moreover the patients had arterial hypertension and Raynaud's syndrome. We presented the diagnostic difficulties of Sneddon's syndrome, a course of the disease, the factors, which affect its prognosis and attempts to treat this syndrome based on this case.

[Seeking a correlation between functional and imaging findings in patients after acute alcoholic pancreatitis].

The aim of the present study was to correlate the imaging and functional findings in patients who recovered from acute alcoholic pancreatitis (a.a.p.). 35 patients, 4-7 years following acute alcoholic pancreatitis underwent: secretin-cerulein test (SCT), ultrasound (US) and computed tomography (CT) of pancreas. Control group comprised 15 patients in which endoscopic, imaging and functional findings excluded organic G.I. disease. Exocrine function impairment was stated in 65.7%. US revealed pancreatic changes in 37.1% patients and CT--in 45.7%. Significant correlation between the degree of structure abnormalities in imaging techniques nad the severity of exocrine pancreatic function impairment was found (p < or = 0.05). However, in about one third of the patients studied, changes revealed in imaging techniques were not accompanied by function impairment and vice versa. Patients after acute alcoholic pancreatitis should be followed-up to early recognize and treat the revealed changes. Imaging and functional methods should be considered as strictly complementary.

Goiter incidence in region of Katowice in relation to iodine deficiency and iodine prophylaxis (Katowice coordinating center).

Geographical region of Katowice is situated in an upland built of carbonate rocks, often with predominance of loss. A randomized trial was carried out in the urban population of this area. 1080 children aged 7-13 were examined. In 505 urine iodine was measured. 17.5% of the examined children had enlarged thyroid glands (Ib, II, III) in palpation, 82.3% did not have goiter (O, Ia). Interviews with patients revealed that 48.8% of the examined children used iodized salt, while 51.2% did not. Mean urine iodine concentration was 70.6 micrograms/l. No correlation between use of iodized salt, urine iodine concentration, and goiter was found. However, prevalence of goiter and low urine concentration suggested deficiency of this microelement in the examined industrial region. Therefore there is need for further investigations and for a profound change of the existing ineffective iodine prophylaxis.

[Dermatoses characteristic in particular occupations. I. Various branches of industry].

Dermatoses characteristic for several branches of industry: metal, electrotechnical, photochemical and pharmaceutical and for laminators and galvanizers are discussed. The periods are given after which skin changes appear, their primary localisation and the allergens specific for the particular occupations. In photo-chemical industry workers, beside dermatitis. lichen planus was also noted.

[Allergy to industrial oil components].

The paper presents the data on the allergy to chromium in the workers dealing with oils. Single cases of hypersensitivity to additives refining industrial oils, such as triethandamine and hydrazine are described. Gross reactions between triethanolamine and the component of shampoos treithanolamine oxyethylane - sulphate have been demonstrated.

[Cement as a metal allergy-inducing agent].

Three groups of patients suffering from eczema, exposed to cement, have been examined. Altogether there were 293 subjects. No particularly strong sensitizing properties of the Polish cement have been found. This, however, calls for confirmation. Differences, hitherto unknown, between masons and concretors have been found at the middle stage of eczema incubation and at middle age when occupational skin lesions occur, indicating a clearly higher allergy risk in concretors. On the other hand the sensitivity to particular metals has been similar in all building workers and did not differ from that in other countries.

[Dressings made of multilayer unwoven fabric].

The multilayer unwoven fabric dressings have been examined both in laboratory and clinic. The properties of metalized and ordinary dressings have been compared. In clinical examinations the usefulness of metalized materials was proved to burns and granulating wounds. Also the advantages of unwoven fabric materials over the classic dressings have been ascertained.

New contact allergens and allergen sources.

In the report new contact allergens and allergen sources detected in Warsaw in the period 1975-1977 are described. They are divided into 3 groups: industrial allergens, remaining occupational allergens and cosmetics. There are given some data concerning the substances present in industrial oils, hardeners and epoxy resin solvents, drugs sensitizing nurses, several new sources of chromium allergens, essential oils and synthetic flavours. Patch tests with potassium dichromate removed after 24 and 48 hours.

The rate of release in vitro of four potassium dichromate concentrations (0.5, 0.1, 0.05 and 0.01%) was determined. In the course of 24 h, 90.1-97.5% was released of the total K2Cr2O7 amount in 48 h. Out of 30 patients positive to 0.05% dichromate when the patch test was done routinely, 29 were also positive when the patch was removed after 24 h. The effect of the earlier removal of the patch test was also noted in seven other patients, when results of tests with lower dichromate concentrations were taken into account.

Allergens

[Diagnosis of diabetes mellitus and disorders of glucose tolerance in patients after acute pancreatitis].

The study aimed at evaluating an incidence of diabetes mellitus and carbohydrate tolerance disorders as well as insulinemia in patients with the history of the acute pancreatitis. Baseline glycemia was determined in 50 patients with a history of the acute pancreatitis and in 15 healthy individuals (aged between 18 and 65 years). Blood sugar was then determined 30, 60, 90 and 180 minutes following loading with 75 g of glucose. Fasting insulinemia and that following loading with 75 g glucose were determined at the same time period. Diabetes mellitus was diagnosed in 6 patients (12%) whereas carbohydrate tolerance in 4 patients (8%). A decrease in insulin response to carbohydrates was noted in 36 patients (72%) with a history of the acute pancreatitis in comparison with the control group. The obtained results suggest that the acute pancreatitis significantly decreases endocrine functioning of the pancreas. Therefore, metabolism of carbohydrates should be checked particularly in the individuals with a history of the acute pancreatitis without the symptoms of both diabetes mellitus and sugar tolerance disorders but with the signs of decreased insulin response to carbohydrates.
