A graded work site intervention program to improve sun protection and skin cancer awareness in outdoor workers in Israel.

OBJECTIVES: A graded worksite intervention program to improve sun protection and skin cancer awareness of outdoor workers was implemented and evaluated longitudinally over a period of 20 months. METHODS: Outdoor male workers (144/213 recruits) from geographically separated units of the Israel National Water Company were allocated to complete (n = 37), partial (n = 72) or minimal (n = 35) intervention groups. Subsequent to the assignment and training of local safety officers, an educational and medical screening package was provided to the corresponding groups either once, or repeatedly a year later. Personal sun protective gear was provided upon repeated intervention. Outcome measures were evaluated through self-response questionnaires administered prior to the first intervention pulse, and 8 months after the first and second interventions. RESULTS: A 15-61% improvement in sun-protection habits was noted in the entire study population 8 months after initialization, compared to no sunscreen use, 20% sun-exposed skin area and highest mean occupational exposure dose of 1.68 MED/day at pre-test. An even greater use of sunscreen was evident 1 year later in the complete and partial intervention groups, + 80% and + 52%, respectively. The baseline rate of self-examination of the skin in the same two groups (49%) increased significantly at post-test (+ 71% and + 53%, respectively). CONCLUSIONS: This integrated intervention program led to significantly improved sun protection and skin cancer awareness. Repeated intervention combined with the supply of sun-protective gear contributed to an even greater impact.

A new computer program for mass screening of visual defects in preschool children.

AIMS: To test the effectiveness of a PC computer program for detecting vision disorders which could be used by non-trained personnel, and to determine the prevalence of visual impairment in a sample population of preschool children in the city of Beer-Sheba, Israel. METHODS: 292 preschool children, aged 4-6 years, were examined in the kindergarten setting, using the computer system and "gold standard" tests. Visual acuity and stereopsis were tested and compared using Snellen type symbol charts and random dot stereograms respectively. The sensitivity, specificity, positive predictive value, negative predictive value, and kappa test were evaluated. A computer pseudo Worth four dot test was also performed but could not be compared with the standard Worth four dot test owing to the inability of many children to count. RESULTS: Agreement between computer and gold standard tests was 83% and 97.3% for visual acuity and stereopsis respectively. The sensitivity of the computer stereogram was only 50%, but it had a specificity of 98.9%, whereas the sensitivity and specificity of the visual acuity test were 81.5% and 83% respectively. The positive predictive value of both tests was about 63%. 27.7% of children tested had a visual acuity of 6/12 or less and stereopsis was absent in 28% using standard tests. Impairment of fusion was found in 5% of children using the computer pseudo Worth four dot test. CONCLUSIONS: The computer program was found to be stimulating, rapid, and easy to perform. The wide availability of computers in schools and at home allow it to be used as an additional screening tool by non-trained personnel, such as teachers and parents, but it is not a replacement for standard testing.

The recovery from ultraviolet radiation-induced erythema and melanoma risk factors: a case-control study.

The proportion of subjects recovering from skin erythema induced by a single ultraviolet radiation challenge of 6 times the minimal erythema dose during a 3-week period was lower in 47 patients with stage I cutaneous melanoma than in 48 healthy control subjects with similar risk factors of increased sensitivity to ultraviolet radiation (p = 0.045). This difference indicates that the patients with melanoma were more susceptible to prolonged ultraviolet radiation-induced skin damage than the control subjects. Prolonged erythema response was significantly associated in the melanoma group with decreased minimal erythema doses (odds ratio [OR] = 11.3) and with the presence of freckles (OR = 5.5), and was associated in the control group with light eye color (OR = 5.8). Prolonged ultraviolet radiation-induced erythema is neither a unique feature of melanoma patients nor a useful marker for identifying risk groups for cutaneous melanoma.

Adaptive assay of radioactive pulmonary aerosols with an external detector.

The accuracy of the assay of radioactive pulmonary aerosols is influenced by the spatial distribution of the aerosol in the lungs. In general, there is considerable uncertainty in the spatial distribution of the aerosol beginning a few months after inhalation. This paper develops an adaptive approach for optimizing the assay of non-uniform spatial distributions of pulmonary aerosols. An adaptive assay is one in which the design of the assay system is modified during operation in response to measurements obtained on-line. Experimental results are presented which compare the performance of an adaptive assay with that of the standard measurement procedure. We study the assay of 241Am distributed in the lungs of a Lawrence Livermore realistic phantom using a NaI-CsI Phoswich detector.

Effect of radiation on the production of interleukins and T-lymphocyte activities.

The effect of 0-400 rad 60Co gamma-ray doses on distinct steps in the process of murine T-cell activation by concanavalin A (Con A) was investigated. When C57BL/6 spleen cells were stimulated immediately after irradiation, production of interleukin-1 (IL-1) and interleukin-2 (IL-2) was not impaired. Concomitantly, the display of IL-2 receptors in Con A-induced reactivity to IL-2 was not affected. The proliferative response was markedly diminished by increasing doses of radiation. The effect of radiation was found to depend not only on the delivered dose but also on the time interval between irradiation and stimulation of the lymphoid cultures. When the mitogenic stimulus was delayed for 24 hours following irradiation, IL-1 production was not diminished, whereas IL-2 production was impaired by doses greater than 200 rad. The proliferative response was diminished to a markedly higher degree as compared to the degree it was diminished in cell cultures stimulated by Con A immediately after irradiation. IL-2 production and the proliferative response to Con A of irradiated cell suspensions, cultured without mitogen for 24 hours post irradiation, were also assessed after adjustment for cell death. In this case, an impairment in IL-2 production that was dose dependent was apparent, but still the levels of IL-2 secreted by 400-rad irradiated cells reached high levels. In contrast, the proliferative response to Con A could not be restored. When T-cell growth factor was added concomitantly with Con A to irradiated cell cultures, a radioprotective effect could be observed.

Solar retinopathy: visual prognosis in 20 cases.

A report of 20 cases of solar retinopathy associated with watching an eclipse of the sun is presented. Visual prognosis was generally good and most patients had good vision at the end of six months. Paramacular scotomata, image distortion and macular changes were, however, still present in the majority of patients. Transmission of energy through various protective devices was measured with a fluxmeter. The finding of retinal damage after exposure of even short duration through protective devices that reduce the incident irradiance of the sun up to 5,000 times indicates that the damage observed may be other than thermal in nature. It is concluded that no safe method for directly viewing a solar eclipse is generally available, and that the public, especially schoolchildren and their children, should be better acquainted with this hazard.

New Tyndall photometer.

An instrument is described that can accurately measure delicate changes in the Tyndall phenomenon in the anterior chamber of the eye. An important feature is its use of an external light source and its simple adaptation to the slit lamp.

Interrelation between plasma magnesium and 25 hydroxy-vitamin D3 levels in chronic renal failure.

In a series of 18 patients suffering from varying degrees of chronic renal failure treated by diet alone, there was a statistically significant positive correlation between plasma magnesium levels and plasms 25 hydroxycholecalciferol (25 HCC) levels. This is thought to be due either to the direct action of 25 HCC on the bone liberating magnesium from bone crystal, or to an increased intestinal absorption of magnesium caused by 25 HCC.

Uptake of aluminium ion by the liver.

The specific uptake by the rat liver of 28Al was shown. There was specific uptake of 28Al by liver cell nuclei and DNA. This uptake was blocked by prior treatment with stable aluminium. It is concluded that aluminium enters the liver cell by some specific mechanism and that the reaction of aluminium with DNA inside the hepatocyte nucleus could be a mechanism responsible for the development of aluminium-induced experimental porphyria in the rat.

The aetiology of senile osteoporosis: secondary hyperparathyroidism due to renal failure.

The aetiology of senile osteoporosis was investigated in a series of elderly normal persons (mean age 76.8 years) who were compared to 18-19 year old normal controls. Osteoporosis was estimated by standard radiological morphometric and densitometric techniques, in the metacarpals and thoraco-lumbar vertebrae. Serum parathyroid hormone levels were significantly higher in the elderly group, and correlated well with morphometric and densometric measurements of osteoporosis. Creatinine clearance was reduced in seven out of nine of the elderly group, and correlated well with the degree of osteoporosis. Serum thyrocalcitonin levels were reduced in the elderly. Tubular reaborption of phosphate and TmP/GFR were in the hyperparathyroid range in the elderly group and correlated well with the degree of osteoporosis. The hypothesis is advanced that the osteoporosis of old age is a result of parathyroid overactivity, caused by asymptomatic chronic renal failure. The suggestion is made that a diet low in phosphorus might reduce the incidence of osteoporosis in old age by reducing the parathyroid overactivity.
