Ostomy and pregnancy.

Pregnancy in a woman with ostomy poses significant concern in its management. A search for literature addressing this problem revealed minimal information. Questionnaires on this subject were sent to all of the members of the American Society of Colon and Rectal Surgeons, and the replies received from them were analyzed by the computer. Attempts were made to answer questions regarding stoma complications, recurrence of disease during pregnancy, type of childbirth (vaginal or cesarean), special needs during pregnancy, and any recurring problems with subsequent pregnancies.

Rectal procidentia in elderly and debilitated patients. Experience with the Altemeier procedure.

The term "rectal procidentia" implies circumferential full-thickness descent of the rectum through the anal sphincter. This condition occurs because of several structural and anatomic changes and offers a challenge in its management. There are over 50 different surgical procedures described to correct this condition. Unfortunately, when it occurs in the elderly and infirm, there is very little that can be done. Such poor-risk patients should be managed with a perineal repair as described by Altemeier. The purpose of this article is to review our results and experiences with Altemeier's procedure in managing rectal procidentia in elderly and debilitated patients.

Giant and recurrent condyloma acuminatum: appraisal of immunotherapy.

Eighty-four cases of condyloma acuminatum were presented, with conventional treatment, e.g., surgical excision and fulguration with or without Podophyllin application. Recurrence rate was 28 per cent to 65 per cent, depending on the number and duration of treatments. Immunotherapy was used on 17 patients with recurrent and giant condyloma acuminatum. Condylomata were successfully eradicated in 94.1 per cent of patients, which indicates that immunotherapy is a very effective alternative in the treatment of recurrent and giant condyloma acuminatum.

Retroperitoneal and mediastinal emphysema as a complication of colonoscopy.

Retroperitoneal and mediastinal emphysema as a complication of colonoscopy, though infrequent, may occur. It is important to realize that this can be a benign clinical condition that will resolve with conservative management. A case of retroperitoneal emphysema after routine colonoscopy is reported with a review of literature and a brief discussion.

Comparison of two agents in prevention of urinary retention after benign anorectal surgery.

In an attempt to substantiate the efficacy of Urecholine and Dibenzyline in prevention of urinary retention, 263 patients who had rectal surgery for benign anorectal abnormalities were divided into three comparable groups; one group was given Urecholine, another group was given Dibenzyline in 10-mg doses orally, four and eight hours after surgery and the third group was given nothing as a control. The control group had a higher incidence of urinary retention 28.2 per cent; but Dibenzyline did not prevent urinary retention in 20 per cent of the patients and was no more effective than Urecholine at 22.4 per cent.

Condylomata acuminata in an infant and mother: Report of a case.

A case of a 16-month-old child who had had perianal condylomata acuminata since the age of 3 months, born to a 20-year-old mother who had had extensive anorectal and vulvovaginal condylomas during her pregnancy and delivery, is presented. The probable mode of transmission of the condyloma virus and its application to this case are discussed.
