Hemoperfusion effect on oxygen-carrying function of preserved donor blood and erythrocytic mass.

The purpose of the study was to explore the preserved (7-21 days) donor blood and erythrocytic mass, following perfusion through hemosorbent SKN-D. After the sorption the oxygen-transport function of erythrocytes was found to improve considerably (oxyhemoglobin dissociation curve was shifted to the right on average by 4-4.2 mm Hg). The energy supply became better (ATP level was increased by 38-59%). In view of the above, long-preserved blood and erythrocytic mass following their perfusion through SKN-D hemosorbent can be considered a sufficiently valid transfusion medium.

Hemosorption of restored long-preserved blood.

Incubation of long-preserved donor blood erythrocytes (over 21 days) with rejuvenating solution Erythropifaden resulted in an increased ATP concentration to 2.03 +/- 0.34 mmol/l; increased 2,3-DPG to 3.89 +/- 0.51 mmol/l, and the number of erythrocyte discoid forms was raised to 30%. Further blood perfusion through SKN-D hemosorbent allowed to reduce the following adenine level to zero; riboxine by 90; citrate and lactate by 50%. The plasma K+ was decreased from 29.8 +/- 1.25 to 13.0 +/- 0.87 mmol/l, while blood pH rose from 6.675 +/- 0.018 to 7.310 +/- 0.028. At the end of the perfusion, the ATP level was seen to further increase to 2.53 +/- 0.28 mmol/l, and 2,3-DPG to 4.99 +/- 0.7 mmol/l. The indices of osmotic resistance and deformability of erythrocytes were normalized. The discocyte number reached 40%. The amount of red blood cell irreversible forms declined from 26 to 13% without any increment plasma free hemoglobin. The data obtained confirm the high efficacy of the combination of the rejuvenating and hemosorption procedures when applied to stored long-preserved blood.

Alterations in rheologic properties of stored blood as criterion of the functional state of erythrocytes in haemocarboperfusion.

Rheologic studies performed on rotational viscometers "Rheotest-2" and "VIR-78" within shear rates gamma interval of 0.1 s-1 less than gamma less than 1,312 s-1, revealed a substantial reduction in dynamic viscosity of the blood subjected to sorption treatment on activated charcoal. The importance of volume concentration of erythrocytes proper for dynamic viscosity is analysed. Special emphasis is placed on the necessity to take into account the dispersion of electrophoretic mobility of erythrocytes in analyzing surface properties of erythrocytes from stored blood before and after hemocarboperfusion. In addition, the normalization of electrosurface characteristics of erythrocytes and associated with them the aggregational property of blood after its purification by sorption is demonstrated. The restoration of the explored physical and chemical characteristics of blood, perfused through the sorbent, permits its more valuable transfusional medium to be considered in comparison to conventional stored blood with identical times of preservation.

Stored blood purification on activated charcoals.

The application of the hemosorption technique for storage, preservation and transfusion of the donor blood will result in a substantial increase in effectiveness of its application in numerous practically important situations, including even such as massive and exchange transfusion, or in patients with renal or liver insufficiency. Especially the metabolism of the erythrocytes, the intra- and extracellularic electrolyte supply of the blood and the pH level of the blood will be corrected according to the norm. Harmful products of the metabolism, microclots and impaired cells will also be reduced. The osmotic resistance of the erythrocytes rises significantly, the viscosity of the blood and the HBS-antigen level decrease.

[Correction of metabolic changes in preserved donor blood using hemosorbents].

The investigation of the effect of perfusion through the activated charcoal upon the biochemical and morphological parameters of the donor blood stored with the glucose-cytrate conservant during 14-21 days has shown normalization of the acid-base status, increased oxygen-transport function and energy balance of erythrocytes. So, blood continuously stored and purified with the help of sorbents is quite suitable for massive transfusions.

[Effect of erythropifadin on the morphofunctional characteristics of donor blood preserved for a long time].

Adenine- and riboxine-containing erythropifaden has been studied for possibility to be used for restoring a functional value of erythrocytes in conserved donor blood stored for 35-49 days. Energy supply and oxygen transport function of erythrocytes are shown to get normal and their morphological composition--to get considerably improved. Besides, osmotic resistance and deformability of erythrocytes insignificantly change while concentration of riboxine in blood and that of substances with average molecular mass remain considerably high. One of the ways to remove the above shortcomings is application of hemosorbents.

[A sorption microfilter for blood transfusions].

A special device is developed to purify the conserved blood from microaggregations and toxic agents. It also decreases the CO2 level and the citrate content. The device combines the properties of a microfilter and a sorption column. The specifications of the device are given.
