Rhabdomyoma of prostate.

An asymptomatic nineteen-year-old white man presented with a markedly enlarged and indurated prostate. Urologic evaluation including cystoscopy, prostatic ultrasound, pelvic computerized tomography (CT) scan, and pelvic nuclear magnetic resonance imaging revealed a prostatic mass impinging on the bladder. Transrectal biopsies returned the pathologic diagnosis of rhabdomyomatous hamartoma. We report the first case in the English literature of extracardiac rhabdomyoma found in the male genitourinary tract.

Anatomic explanation for bulbar urethral pseudostricture.

Interpretation of retrograde urethrography can be complicated by the appearance of an apparent well-defined proximal bulbous urethral stricture which later is not identified on urethroscopy. Two illustrative cases of this confusing finding are presented, and the anatomic explanation is reviewed.

Choice of antibiotic for lower urinary tract infections in men.

When lower urinary tract infection is present in a male patient, prostatic involvement must be considered. A 30-day course of treatment with one of the antibiotics shown to be superior in treating prostatitis is recommended. It is hoped that this program will eliminate needless progression or relapse of infection and reduce the subsequent risk for development of chronic prostatitis.

Small cell carcinoma of prostate.

Small cell anaplastic carcinoma (SCC) involving the prostate is extremely unusual, either as a primary or as a secondary metastatic focus. A case of SCC presenting as prostatic obstruction and diagnosed with prostate biopsy is presented and the literature reviewed.

Simple aid for upright voiding after partial penectomy.

Inadequate penile length after partial penectomy may result in the socially unacceptable inability to project the urinary stream over the scrotum. A simple adaptation of a 60 cc syringe is described as an aid to contain and direct the urinary stream.

Traumatic testicular torsion: case report.

Trauma has been reported as an infrequent cause of testicular torsion. A case of trauma-induced testicular torsion is reported and literature reviewed. The inclusion of torsion in the differential diagnosis of post-traumatic acute scrotal pain is important to ensure early appropriate management.

Is antibiotic administration indicated after outpatient cystoscopy.

The incidence of urinary tract infection was examined in 138 patients who underwent diagnostic cystoscopy in an outpatient clinical setting with preoperatively sterile urine. A prospective randomized trial was conducted to compare the incidence of infection in patients who received no antibiotic (controls, group 1) and those who received a postoperative course of oral antibiotics (group 2). The incidence of bacteriuria after cystoscopy was 2.8 per cent in group 1 (control) and 1.5 per cent in group 2, with an over-all incidence of 2.2 per cent. Only 1 patient presented with a symptomatic infection (0.7 per cent). This study has confirmed the safety of outpatient cystoscopy and has demonstrated that routine administration of postoperative antibiotics is not indicated.

Pure primary testicular carcinoid: a case report and discussion.

We report a case of pure primary testicular carcinoid tumor and review the literature. Distinctions are emphasized among the 3 subgroups of testicular carcinoids and their clinical significance. Criteria to establish the diagnosis of pure primary testicular carcinoid are offered.

Trial of ibuprofen to prevent post-vasectomy complications.

Sperm granuloma and epididymitis remain 2 of the most common and incapacitating complications of vasectomy. A study was designed to evaluate the possibility of reducing these inflammatory complications with a prophylactic course of a nonsteroidal anti-inflammatory drug. Patients undergoing outpatient vasectomy were randomized into 2 groups. Group 1 received a prophylactic course of ibuprofen and group 2 received no medication. The ibuprofen was tolerated well and no adverse reactions were noted. There was no increase in postoperative bleeding or hematoma formation. There was no clinical benefit or decrease in complication rate in the ibuprofen-treated group.

"Turban" scrotal dressing.

Edema and hematoma are common complications of scrotal surgery. We describe a new technique of scrotal wrap dressing that diminishes markedly the incidence of these postoperative complications.

Preoperative laparoscopic localization of the nonpalpable testis.

Laparoscopic examination was performed successfully on 14 patients with 17 nonpalpable testes. The procedure was successful in determining the location in 16 of 17 nonpalpable testes, and this information was beneficial for selecting the subsequent operative approach. Laparoscopy was safe in the younger child (10 patients were 3 years old or less and 7 patients were less than 2 years old), and it added an insignificant increase in operative time.
