Serum creatine kinase kinetics after endomyocardial biopsy in cardiac transplant patients.

The objective of this study was to define the changes in serum creatine kinase (CK) and creatine kinase-MB isoenzyme (CK-MB) activities following endomyocardial biopsy. Ten cardiac transplant recipients underwent routine surveillance endomyocardial biopsy via the right internal jugular vein with single-use disposable bioptomes. Serum CK and CK-MB levels were measured at baseline and post-biopsy at 15, 30, and 45 minutes, 1, 1.5, 2, 4, 6, 8, 12, 16, 20, and 24 hours. There was no significant increase in serum total CK after biopsy. No measurable change in CK-MB was observed in 40% of patients. CK-MB increased significantly in the remaining patients (N = 6) at 30 minutes (p < 0.05), and continued to rise to a peak at 4 hours post-biopsy, after which CK-MB gradually decreased to baseline. However, the increase in CK-MB did not meet criteria for the diagnosis of myocardial necrosis, hence should not be confused with the change in CK-MB occurring with myocardial infarction. These findings suggest that serum CK-MB activity is useful in the differential diagnosis of chest pain in cardiac transplant patients in the setting of recent biopsy.

Emergency medicine applications of echocardiography.

Echocardiography represents an important, accessible tool in the evaluation of the critically ill emergency department patient. Echocardiography provides anatomic and physiologic information about the cardiovascular system safely and at the bedside. This modality may permit early and more accurate diagnosis and management of a wide range of disease processes while avoiding the risks and costs associated with other diagnostic strategies. It is imperative that emergency department physicians have a working understanding of the applications and limitations of echocardiography to make best use of its considerable patient care potential.

Management of orthotopic heart transplant recipients at Brooke Army Medical Center.

Advances in heart transplantation and post-operative rejection therapy have given new hope to patients with end-stage congestive heart failure and cardiomyopathies. The growing numbers of pre- and post-transplant patients and the complexity of their care underscore the need for a multispecialty approach to patient management. This study examines the success of a multi-service program at Brooke Army Medical Center which offers pre- and post-transplant care to active duty, dependent, and retired military personnel.

Prosthetic heart valve.

There are tens of thousands of patients with prosthetic heart valves implanted today, and although complications are infrequent, this may not hold true as patients live longer with prosthetic valves and as more and newer valves are implanted. A familiarity with the management of such patients will aid the emergency physician in the evaluation of this increasing population of patients and will enable the early detection of potentially deadly complications.

Transesophageal echocardiographic features and management of retained intracardiac air in two patients after surgery.

Intracardiac residual air after open-heart surgery may carry a significant risk of embolization with associated morbidity and death. We hereby report two cases of retained intracardiac air detected in an unusual location with transesophageal echocardiography in the superior aspect of the left atrium after completion of standard air-evacuation procedures. Characteristic echocardiographic features and the impact on immediate management are described along with a review of the literature.
