Study of ileostomy in dogs, functional aspects, optic and scanning electronic microscopy.

Containing ileostomy has been a permanent aim of researchers. Different technics for containing in the ileal derivations has been proposed, very little filled the requisites, but no one reached the desired aim, and the problem continues unsolved. The authors, based in experimental study in two groups of dogs submitted to ileostomy, with and without preservation of ileal pylorus, observed the following parameters: weight dog clinical evaluation of animals (diarrhea, infection of surgical wound and malnutrition) volume of ileal excretions electrolytes (Na+, Cl- and K+) pH of ileal excretions and deaths. After sacrificing the animals we analyzed on scanning electronic microscopy, the presence of mucus, clearness of mucosa surface, general aspect of terminal ileum mucosa, and on optic microscopy, neutrophilic infiltration, increasing of number of goblet cells, hypotrophy of villosities and determination of ileal villosities height. After statistic analysis of results we concluded that ileostomy with preservation of ileal pylorus, markedly attenuated the variable of control in the experiment. Conservation of ileal pylorus, consequently has an effective action on the control of ileal leakage, protecting its mucosa from serious histopathologic changes.

Surgery of pancreatic dermoid cyst with ligature of superior mesenteric vein.

The authors present a case report of a 21 years old patient with a pancreatic dermoid cyst, clinically visible as an abdominal mass. The complete excision of the cyst obliged an elective ligature of the superior mesenteric vein, without complications. Comments are made concerning this pathology and the venous connection bearing in mind recent data from the references consulted.

[Comparative study of the suture of pancreatic stump with absorbable and unabsorbable threads in rats].

The safety of suture and healing of pancreas after surgery with absorbable and unabsorbable suture was studied. One hundred and twenty female rats was underwent a segmentar pancreatic resection. The pancreatic suture was performed with catgut in Group I (n = 48), polygalactin in Group II (n = 48) and silk in Group III (n = 24). The animals were sacrificed at 5th, 10th, 20th and 30th days after surgery and analized at macroscopy: inflammatory signs, fistula, abscess and suture bearing in tissue. Microscopic findings were: suture bearing in tissue, inflammatory aspects against suture, necrosis, edema, neovascular modifications, fibrosis and the collagen's type. After comparing the results with Wilcoxon's two sample test, we concluded that pancreatic healing was not modified with differents sutures, and then pancreatic absorbable or unabsorbable suture was safe.
