The authors report the case of a 15-year old boy with Tangier disease characterized clinically by grossly enlarged tonsils, liver and spleen. Blood counts showed signs of hypersplenism, and blood lipid assays disclosed hypercholesterolaemia with moderate hypertriglyceridaemia and very low levels of high density lipoproteins. The diagnosis was confirmed by biopsies of the liver, spleen, rectal mucosa, tonsils and bone marrow, all tissues which contained foamy histiocytic cells staining like adipose cells. Following splenectomy, the child developed an unexplained, persistent (6 months) fever, then died suddenly of digestive haemorrhage. A genetic study showed a high degree of inbreeding, with 2 deaths probably due to Tangier disease. The authors contrast the surprising severity of this case with the apparent benignity of the disease as described in other publications.

Fibrin stabilising factor (FSF) was studied in the circulating blood of 196 children. These 196 children comprised three groups: 131 controls (group A), 20 children with diseases of potential repercussion on FSF determination (group B), and 45 children with Schönlein-Henoch syndrome (group C). Determinations from groups A and B produced normal values, but results from group C were significantly lower at the onset of the vasculitis. Seventeen children with Schönlein-Henoch syndrome have had complications, in 7 of whom these were severe. The decrease in FSF levels was correlated with the severity of such complications, and an increase in FSF was associated with recovery. Determination of FSF activity appears to help in the diagnosis of Schönlein-Henoch vasculitis, as well as helping to monitor the course of the disease and assessing the risks of complications.

Bronchial fiberscopic examinations were conducted in 51 small babies and infants under general (9 cases) or local (42 cases) anesthesia. Most of those performed under general anesthesia employed the Venturi effect for ventilation through the fiberscope. By using this technical artefact it was usually possible to avoid the use of an intubation catheter and thus to employ most technical procedures (aspiration, biopsy, bronchial brushing). Possible complications of the method are described and the relative indications for fiberscopy as against rigid bronchoscopy discussed. Endoscopy employing flexible and rigid tubes should be complementary examinations during endobronchial exploration in infants.

Rapid radio-immune assaying of LH was carried out four times daily on 52 women in the preovulatory phase of the cycle. This method enables the precise detection of the LH surge and recovery of the mature oocyte by laparoscopic aspiration of the follicular content, just before ovulation, which is between 28 and 36 hours after the start of the LH surge. The oocytes was recovered in 27 out of 41 patients with a normal cycle (66%). The rate of recovery per follicle aspirated is comparable in a natural cycle to that after treatment with Clomid (80 to 84%).
