[Adjusting a surgery department to new conditions in the German health service: day surgery, short-stay surgery, vascular centers and wound centers].

In April 1994, after the preconditions for ambulatory surgery came into effect, the day surgery ward was opened in the Hamburg-Harburg General Hospital. Besides ambulatory operations, this ward provides all surgical pre- and postoperative care. In addition to the surgical sections, different departments (urology, gynecology, dentistry, radiology, ophthalmology) take advantage of the ward. As preoperative diagnostics also could be completed ambulatorily, we were then able to establish a short-stay surgery department. This ward was opened in June 1996. One year later, the vascular center ("Gefässcentrum Harburg") was established by the departments for surgery, radiology, and angiology. This structure has been integrated into the day and short-stay surgery wards functionally and spatially. With the organizational structures mentioned and by setting up a wound consultation service, it is possible to optimize the capacity for admissions, operations, and beds. Government and health insurance demands for better processing can be filled and efficiency is increased.

[Knotted intravascular catheters--what should be done?].

INTRODUCTION: Fixed intravasal catheters are mainly caused by knots. Removal can be achieved by intervention or surgical exploration, but this is associated with additional morbidity and mortality. METHODS: 2 patients were operated for knotted catheters in our institution during the last 2 years, and their records are demonstrated. Treatment options, possible complications, catheter types and locations of knotting are analyzed by a medline search. RESULTS: the search revealed the data from 115 patients. 53 (46.1%) of all "lost" catheters were Swan-Ganz catheters. In 60.9% the catheters could be removed by radiological interventions. Open revision was necessary in 33% of all cases. The catheters were left in place when the clinical condition of the patient did not allow removal (n = 7). However, these patients suffered from a high mortality (5 of 7 patients). Over all mortality reached 8.7%. In the own two cases one removal by sternotomy and one by exploration of the right internal jugular vein were necessary, both operations succeeded without complications. CONCLUSION: Most of all "lost" intravasal catheters are removed by radiological intervention; only one third needs open surgical therapy. These procedures are harmful for the patient and bear considerable risks for complications.

[The giant, pendulant goiter. A case report].

A 48-year-old woman with a monstrous goiter was referred to our hospital. She noticed a growth of the goiter for about 20 years, until now she did not undergo surgery because she was afraid of the operation. In September 1999 she suffered from a cardiac decompensation because of a hyperthyreosis, so she decided to be operated. The examination showed a giant goiter, placed in front of the sternum. After a detailed check of risk factors, we resected the goiter in general anaesthesia. The thyroid gland weighed 5 100 grams, in the histologic examination no signs of malignancy were found. Postoperatively, a hypocalcaemia occurred, without clinical symptoms. The patient was discharged on the 6th postoperative day.

Iterative reconstruction: an improvement of technetium-99m MIBI SPET for the detection of parathyroid adenomas?

The purpose of this study was to assess the value of technetium-99m methoxyisobutylisonitrile (MIBI) single-photon emission tomography (SPET) and an iterative reconstruction algorithm for the preoperative localisation of parathyroid adenomas (PTAs). Seventy-two patients (26 male, 46 female, mean age 58+/-16 years) with known primary hyperparathyroidism were examined preoperatively. First, a thyroid examination was performed to detect possible MIBI-accumulating thyroid lesions. Planar scans were then acquired 15 and 120 min and tomographic images 120 min after intravenous injection of 740 MBq 99mTc-MIBI, using a triple-head gamma camera (Picker Prism 3000). Additionally, 99mTc-MIBI/ 99mTc-pertechnetate subtraction scintigraphy of the early planar images was performed. The SPET data were evaluated using an iterative reconstruction (multiplicative iterative SPET reconstruction: MISR) as well as a standard algorithm (FBP: filtered back-projection with application of a 3-D low-pass postfilter). The weight of the resected PTAs ranged from 110 mg to 5 g. Using planar MIBI scans, correct localisation of the side of the PTA was possible in 81% of cases (58% for PTAs weighing less than 500 mg). Sensitivity increased to 94% using SPET and FBP, while with MISR it rose further, to 97%. Patients with PTAs weighing less than 500 mg showed a sensitivity of 88% with MISR and 81% with FBP. Furthermore, there was a clear improvement in image quality using MISR. None of the normal parathyroid glands were visualised. This study indicates that, in comparison with planar scintigraphy, 99mTc-MIBI SPET is a more sensitive and specific tool for topographical localisation of PTAs, especially those that are small. There is a further improvement in sensitivity and image quality when iterative reconstruction is used instead of FBP.

Technetium 99m-MIBI-SPECT: A highly sensitive diagnostic tool for localization of parathyroid adenomas.

BACKGROUND: The aim of this study was to assess the value of technetium 99m-MIBI scintigraphy using the single photon emission computed tomography (SPECT) technique for preoperative localization of smaller (</= 1 g) parathyroid adenomas. METHODS: A total of 92 patients (34 men, 58 women; mean age, 60 +/- 13 years) with an established diagnosis of primary hyperparathyroidism and nondiagnostic ultrasonography (inclusion criteria) were scanned preoperatively. After a thyroid examination to check for other possible radionuclide-accumulating thyroid diseases, a planar technetium 99m-pertechnetate/technetium 99m-MIBI subtraction scintigraphy (15 minutes post injection) and tomographic images (120 minutes post injection) were acquired after intravenous injection of 740 MBq of technetium 99m-MIBI and using a 3-head gamma camera (Picker Prism 3000). Sensitivity was defined by the ability to predict the correct site of a parathyroid adenoma. RESULTS: All patients had parathyroid adenomas </= 1 g (53 patients, 0.5 -1.0 g; 39 patients, < 0.5 g). Correct localization of parathyroid adenomas to one side or the other was achieved in 87% of the patients using planar technetium 99m-pertechnetate/technetium 99m-MIBI subtraction scintigraphy. Sensitivity was increased to 95% by supplementary use of the SPECT technique and a 3-D display (volume-rendered reprojection for visualization). There was technetium 99m-MIBI accumulation in 11 benign thyroid nodes, but none of the healthy parathyroid glands were shown on the scan. CONCLUSIONS: This study indicates that technetium 99m-MIBI parathyroid scintigraphy is a sensitive and specific tool for topographic localization even of small parathyroid adenomas, especially with the use of SPECT. This method could help to improve the efficiency of parathyroidectomy (eg, by making unilateral exploration sufficient).

[99m-Tc-MIBI SPECT parathyroid gland scintigraphy for the preoperative localization of small parathyroid gland adenomas].

AIM: It was the of this study to assess the value of the 99mTc-MIBI scintigraphy in SPECT technique in the preoperative lateral location of small parathyroid adenoma (PTA). METHODS: 25 consecutive patients (8 male, 17 female, mean age 63 +/- 13 years) with the established diagnosis of primary hyperparathyroidism and non-diagnostic ultrasonography were scanned preoperatively. After a thyroid examination to exclude radionuclide accumulating thyroid adenoma, planar and tomographic images were acquired 20 min. and 120 min. after i.v. injection of 740 MBq 99mTc-MIBI using a 3 head gamma camera (Picker Prism 3000). The first 10 patients underwent an additional 201Tl/99mTc subtraction scintigraphy in a 2 days protocol. RESULTS: All patients had small, solitary PTA (< 1 g). 201Tl/99mTc subtraction scintigraphy (n = 10) showed only a sensitivity of 50%. Using planar MIBI-scintigraphy lateral location of PTA was possible in 18 cases (sensitivity: 72%). There was an increase in sensitivity up to 96% using the SPECT technique and the 3D display (volume-rendered reprojection). CONCLUSION: In contrast to 201Tl/99mTc subtraction scintigraphy, which showed only a low sensitivity, a reliable lateral location of small PTA was obtained using the tomographic 99mTc-MIBI scintigraphy. This method offers e.g. the possibility for the surgeon to perform an unilateral parathyroidectomy.

[Blood group antigen expression in papillary carcinoma of the thyroid gland. An immunohistochemical and clinical study of expression of Lewis, ABO and related antigens].

Nine monoclonal antibodies, lectin from Ulex europaeus and neuraminidase enzyme were employed to demonstrate the occurrence of type 1 and type 2 blood group antigens in 104 cases of papillary carcinoma of the thyroid. The reagents applied, recognize the following blood group related antigens: CA-50 (sialylated type 1 precursor), CA-19-9 (sialylated Le(a)), Le(a), Le(b), A, B, H, Le(x), sialylated Le(x), and Le(y). Immunohistochemical studies revealed that papillary carcinoma of the thyroid, in contrast to histologically normal thyroid tissue, is characterised by a progressive expression of blood group antigens. Most tumours (84%) reacted with C-50 antibody, whereas only a minority of the tissues demonstrated the CA-19-9 antigen (38%). Type 2 structures Le(x) (47%) and Le(y) (13%) were found less often than their corresponding type 1 isomers Le(a) (71%) and Le(b) (62%). Desialylation with neuraminidase increased the Le(a) and Le(x) staining intensity in 27 and 44 case, respectively. Of the A, B, H antigens the A determinants encountered most frequently (24%). Comparative examinations of sequential sections of the same tumour revealed coexpression of type 1 antigens in the same areas. In carcinomas showing type 1 and type 2 antigen reactivity, a complementary distribution of the structures in different tumour areas was often demonstrated. Some tumours presented combined type 1 and type 2 antigen expression in the same cells, however, in distinct areas within the cell. A follow-up examination was carried out in 68 of the 104 cases. The observation time ranged from 12 to 217 months. Thirteen patients suffered from recurrence, of which 7 died. While lymphatic metastases occurred in 39 tumours, distant metastases were detected in 6 patients. Most of the recurrences were found in patients with tumour classification pT4 (n = 19), whereas none of the pT1 carcinomas (n = 20) showed recurrence. The clinical results were compared to the blood group antigen expression results. There was no correlation between antigen expression and differentiation degree of the tumour. The pT4 tumours showed a significant higher expression of the CA-50, CA-19-9, Le(a) and Sialyl Le(x) structures. Carcinomas expressing the Le(y) antigen were associated with a significant higher level of metastasizing capacity. The Le(y), H type 1 and H type 2 antigens occurred more frequently in recurrent tumours (n = 14). In contrast, none of the patients whose carcinomas expressed the A-antigens (n = 14) suffered from a recurrence or hematogenous metastasis. Multiple stepwise regression analysis was carried out to check the importance of each staining and clinical factor. In this analysis, "distant metastasis' was the most important parameter, whereas the staining results were of minor statistical importance.

[Frequency of malignancy and indications for surgery in nodular goiter in an endemic area].

Goitre operations were done in 2114 patients; 1648 patients had a simple solitary nodule or multinodular goitre with regressive changes. Malignancies were found in 54 cases with a clear-cut predominance of papillary carcinomas. Nearly half of the goitres diagnosed as solitary nodules preoperatively were shown to be multinodular. This discrepancy was taken into account in the determination of the rate of malignancy. Malignomas were found in 4.6% of "true" solitary nodules and in 2.8% in multinodular goitres. These findings confirm our liberal indications for operation in multinodular goitres with regressive changes also in the area of endemic goitre. The high incidence of microcarcinomas in our patients can thus be explained.

[Hypoferric anaemia due to a leiomyosarcoma of the proximal jejunum (author's transl)].

In an outpatient the diagnosis of a hypoferric anaemia and a transitory, masked, intestinal bleeding led to the discovery of a tumour in the proximal jejunum. The diagnosis was confirmed by operation. Radiographs and surgical specimen corresponded to each other. The exact diagnosis leiomyosarcoma could only be made histologically. Sarcomata of the proximal jejunum are extremely rare.
