The prediction of ovulation: a comparison of the basal body temperature graph, cervical mucus score, and real-time pelvic ultrasonography.

Ninety-five menstrual cycles were studied in 20 women undergoing donor artificial insemination (AID). In 49 cycles basal body temperature (BBT) changes were charted daily and both daily cervical mucus scoring (modified Insler score) and daily realtime ultrasonography (USS) were performed from day 11 to ovulation. AID was performed only on the day of follicular rupture. A control group, not subjected to USS, were inseminated two to three times per cycle over 46 cycles in the periovulatory period. The Insler score was found to be a reliable indicator of follicular development and rupture. The BBT was found to be less reliable than the Insler score or USS. While USS may be used to confirm follicular development, the Insler score is reliable and less costly.

The effects of ovarian adhesive disease upon follicular development in cycles of controlled stimulation for in vitro fertilization.

This study was designed to determine the effect of periovarian adhesive disease upon follicular development. Forty-one clomiphene citrate/human menopausal gonadotropin/human chorionic gonadotropin-stimulated cycles for in vitro fertilization and embryo transfer were studied. Each patient was assessed ultrasonographically before laparoscopic oocyte recovery. The number of follicles in each ovary greater than 1.2 cm was counted. By laparoscopy it was possible to determine the degree of periovarian adhesive disease. Sixteen patients had bilateral adhesion-free (AF) ovaries, 12 had bilateral adherent (A) ovaries, and 13 had one AF and one A ovary. In the 13 patients with one ovary AF and the other A, the mean number of follicles +/- 1 standard deviation (SD) was 3.4 +/- 1.4 and 1.2 +/- 1.1 (P less than 0.001), respectively. A total of 116 follicles was noted in 45 AF ovaries (mean +/- 1 SD, 2.6 +/- 1.3) and 59 follicles in 37 A ovaries (mean +/- 1 SD, 1.6 +/- 1) (P less than 0.001). From these data it was concluded that the presence of periovarian adhesive disease inhibits folliculogenesis by a yet undetermined mechanism.

Outcome and effect of medical intervention in women experiencing infertility following removal of an ectopic pregnancy.

The outcome of pregnancy and the effect of medical intervention were reviewed in patients who had had a previous ectopic pregnancy and a subsequent history of infertility. The case records of 3,650 patients were reviewed. Seventy met the study criteria. Twenty patients either voluntarily withdrew or were discouraged on medical grounds; one conceived. Five of the remainder of the patients conceived prior to laparoscopy. In the other 45 patients, the remaining tube was normal in 13, irreparable in 16, and suitable for surgical repair in 16. To date, among the original patients, there have been 13 live births (18.5%) and two ectopic pregnancies (2.8%). One patient was delivered of a live-born infant subsequent to surgical intervention. The pregnancy outcome in this group of patients is poor. Medical intervention was useful primarily in ruling out other causes of infertility and to provide advice to the couple on future fertility. Medical intervention altered outcome in a very few cases.

Correlations between laparoscopic and hysteroscopic findings in 497 women with otherwise unexplained infertility.

Laparoscopy and hysteroscopy were undertaken simultaneously in 497 women presenting with otherwise unexplained infertility. Of them, 285 complained of primary infertility and 212 of secondary infertility. Laparoscopic evidence of pelvic inflammatory disease in the absence of endometriosis was noted in 103 patients with primary and in 108 women with secondary infertility. Lesions were noted on hysteroscopy in both groups as well as in 34% of women with normal laparoscopic examinations. No specific dependency could be demonstrated between any laparoscopically detected lesion and hysteroscopically detected lesion. A significant dependency was demonstrated only between a history of secondary infertility and uterine adhesions (chi 2 = 12.03, 1 df, p less than 0.01). While the combined endoscopic findings did not demonstrate any dependency, the findings of this study demonstrate the importance of hysteroscopy in assessing all patients with infertility, especially those with secondary infertility.

A comparison of definable traits in women requesting reversal of sterilization and women satisfied with sterilization.

More women of reproductive age are being sterilized. Some of these women regret the decision and subsequently request a reversal of sterilization, whereas others do not. This study was undertaken to develop a profile of easily definable traits of 159 women who requested a reversal of sterilization and compare it with that of 160 women who apparently were satisfied with sterilization. Statistically significant differences were found. Remarriage was the most common cause for regret among women in the group which requested reversal of sterilization. Women in this group married younger, completed their family earlier, and were sterilized at a younger age. These women had significantly fewer live children and had undergone more therapeutic abortions (p less than 0.005).

Histologic reaction to a new microsurgical suture in rabbit reproductive tissue.

A newly introduced coated polyglactin suture was compared with a polyglycolic acid suture following microsurgical repair of standard rabbit uterine incisions. The degree of inflammatory response and tissue fibrosis for each suture was scored, and the results were compared. At 30 days, an inflammatory response was noted in both groups, with a greater response noted in the polyglycolic acid group. At 90 days, the inflammatory response and tissue fibrosis at the site of repair was evident in both groups, with no significant difference between the groups. The previously reported difference between these two absorbable suture materials in their earlier formulations could not be demonstrated in rabbit reproductive tissue. If an absorbable suture is used for tubal surgery, the choice can be made on considerations other than tissue reactivity.

Maternal nutrition in pregnancy. Part I: a review.

Maternal undernutrition may result in a greater deprivation of the fetus than has previously been believed. The infant not only may be "light for dates" but also has an increased risk of perinatal disability or death secondary to gross neurologic and developmental abnormalities. This article reviews current knowledge of the energy, protein, iron, vitamin, sodium and calcium requirements in pregnancy, with special reference to the management of the underweight and overweight pregnant women.

Maternal nutrition in pregnancy. Part II: the implications of previous gastrointestinal operations and bowel disorders.

Pregnant patients who have undergone a gastrointestinal operation for morbid obesity or who have active inflammatory bowel disease or hyperemesis gravidarum run a risk of undernutrition or even severe malnutrition with an increased risk of spontaneous abortion or fetal damage. This article reviews the medical and nutritional management of these gastrointestinal conditions.

Outpatient postconceptional fertility control with vaginally administered 15(S) 15-methyl-PGF2alpha-methyl ester.

15(S) 15-methyl-PGF2alpha-methyl ester was administered as vaginal suppositories to terminate very early gestation in 16 outpatients who were thought to be pregnant. All patients received between 2.5 and 5.0 mg. of the prostaglandin compound. Pregnancy was confirmed later in 10 of the women. In these patients vaginal bleeding started 3 to 6 hours after the start of therapy and continued for an average of 8 days. By subjective assessment, the amount of bleeding during this time was equal to or slightly heavier than that of a normal menstruation for each woman. Patients were followed for 2 to 4 weeks with serial measurements of serum progesterone and HCG. There were no failures at early pregnancy interruption. Gastrointestinal side effects rarely were encountered; no serious complications occurred and no curettage had to be performed because of heavy bleeding. The vaginal use of the methyl ester of 15(S) 15-methyl-PGF2alpha for the termination of early pregnancy seems promising as a reliable and parctical outpatient procedure.
