[Vascular anastomoses of small caliber vessels. Comparison between continuous or interrupted sutures].

Continuous sutures are generally used to perform anastomoses on vessels less than 2 mm in diameter, but some surgeons advocate interrupted sutures which they claim cause less stenosis and provide better vascular potency in the long term. Arteriovenous fistula for chronic haemodialysis is a good model of anastomosis on 1 to 2 mm wide vessels. We used this model to compare the 2 techniques in 2 randomized groups of 20 patients each. The type of suture (continuous or interrupted) was drawn by lot in non diabetic patients who were having their first fistula for chronic haemodialysis. All anastomoses were performed by end-to-side implantation of a superficial vein onto the radial artery. Prolene 8/0 was used as suture material. The maximum follow-up period was 2 years. No significant difference in vascular potency could be demonstrated between the 3 groups. Thus, continuous sutures performed under suitable conditions can be used to perform with greater ease anastomoses which are as patent as those performed with interrupted sutures.

[Obstacle to left ventricle ejection after mitral valvular replacement with a Carpentier-Edwards bioprosthesis. Apropos of 2 cases].

Impairment of the left ventricular ejection induced by a mitral prosthesis remains an infrequent complication, perhaps because it often is unrecognized. This complication would mainly be seen with ball prostheses or standard bioprosthesis used in mitral valve replacements with a small ventricle. Two cases have been recently reported during mitral valve replacement. The two patients were successfully re-operated upon, one after five weeks and the second immediately after the first procedure. A review of the literature enable to specify the etiology and diagnostic criteria with the assistance of Doppler-sonocardiography. Early re-operation may prevent death, often due to a post-operative myocardial dysfunction.

Isolation of Corynebacterium suis from the prepuce of boars.

A survey for prevalence of Corynebacterium suis infection in boars was conducted on a Minnesota farm and at a slaughter plant. Preputial swab specimens were streaked onto Columbia colistin-nalidixic blood agar to which metronidazole (50 mg/L) had been added. Of 97 boars surveyed at a slaughterhouse, 32 (32.9%) were culture-positive, whereas 23 of 38 (60.5%) boars from 1 farm were culture-positive. The lower isolation rate at a slaughterhouse was attributed to the fact that these boars ejaculated before sampling.

[Intratubular bodies in testicles of children].

Intratubular bodies are not rarely to be found in testicles of children. They are 5 times more freqlently noticed scrotal ones. These storages frequently cause a swelling of the tubuli seminiferi, which are thatswhy called as Ringtubuli. With the help of the fluorescence - and polarisation microscopy and the laser microspectrochemical analysis examinations of the delicate tissue tock place. Intratubular bodies predominant consist of protein and they have an amorphous fine structure.

[Results of a regional survey on the treatment of rhizomelic pseudopolyarthritis and temporal arteritis. Apropos of 242 cases treated by various modalities with synthetic antimalarials, corticoids and non-steroidal anti-inflammatory agents].

The authors report the results of a retrospective therapeutic survey concerning 176 cases of rhizomelic pseudopolyarthritis (RPP) and 66 cases of temporal arteritis (TA). Of 128 cases of RPP treated initially by synthetic anti-malarials (SAM) and non-steroidal anti-inflammatory agents (NSAI), 66 were followed up until cure which was obtained after a mean of 23 months and 3 subsequently received brief steroid therapy. 45 cases of RPP were treated initially with corticosteroids. They were generally associated with SAM which enabled early weaning of the steroids, towards the 8 th month, or at least reducing the dose. Cure was obtained within 24 months. Three patients were treated by NSAI and gold therapy. After cure, 5 cases of recurrence and 1 case of TA were observed. 40 cases of TA were initially treated with SAM and NSAI. Twenty cures were obtained within a mean of 28 months. 4 patients later received brief corticosteroid therapy because of an extension of the signs, including two cases of ocular manifestations with a resolving course. Of 25 cases of TA initially treated with steroids, 20 received SAM in combination, or in relay which enabled either steroids, weaning towards the 14th month or a reduction in the dose of steroids. Cure was obtained in an average of 35 months. One case of impaired visual acuity occurred during corticosteroid treatment. Immunosuppressants were used in one patient. No cases of recurrence were observed. Iatrogenic complications with SAM were rare, generally benign and reversible, in contrast to those associated with corticosteroid therapy.(ABSTRACT TRUNCATED AT 250 WORDS)
