[Late assessment of the hip joint treated with the Freyka pillow without complications for congenital dysplasia and dislocation].

Clinical and radiological assessment of 504 hips in 389 patients of 10 to 21 years of age (13 years 10 month on average) has been carried out. The patients were treated by Freyka pillow in the childhood. Those with no reduction achieved primarily and operated on because of residual dysplasia as well as patients with severe avascular necrosis were excluded from this study. In 99% of the hips no clinical abnormality was found except for the change of rotation range--decreased external rotation. Radiographic assessment revealed 60% excellent results and 35% good ones with dominant residual feature of increased anteversion of the femoral neck. Remaining 5% were rated as fair--these were the cases that should have been operated on at a suitable time.

[Surgical treatment of residual congenital dysplasia with dislocation of the hip joint after treatment with the Frejka pillow].

The characteristics of patients and the radiological condition of hip joints that required surgery because of residual dysplasia in the course of growth after treatment by the Frejka pillow have been presented. That treatment was employed in 4 per cent of the treated hips. The operations consisted in inspection of the joint, transiliac osteotomy after Dega with subtrochanteric osteotomy with detorsion of the femur, of the same type of operation without opening of the joint, performance of only transiliac osteotomy or only subtrochanteric osteotomy. Because late evaluation at over 10 years of life revealed 5 per cent of hips with poor rebuilding that should be treated surgically, it has been determined that the total number of cases requiring surgery after treatment by the Frejka pillow is 9 per cent.

Abduction treatment in late diagnosed congenital dislocation of the hip. Follow-up of 1,010 hips treated with the Frejka pillow 1967-76.

There are many countries such as Poland where treatment of congenital hip dislocation is started late. The purpose of this work was to report our results in this group of children. 1010 hips in 780 children with congenital dislocation of the hip were treated with the Frejka pillow. The early results were evaluated in 830 hips at 15-36 months of age and the late results in 527 hips at a mean age of 14 (10-21) years. 90 percent of the children were treated by the same physician. The age at the onset of the treatment varied from 2 weeks to 24 months, with 12 percent younger than 3 months and 28 percent older than 6 months. The initial degree of dislocation was determined with our own index. Radiographic results were evaluated with a scoring based on four or six parameters. There were 6 percent failures, including lack of reduction or redislocation at the time when the child started to walk. Ischemic necrosis was observed in 14 percent of the hips, with significant permanent sequelae in 5 percent. Indications for surgical treatment of residual dysplasia were found in 4 percent of the hips evaluated early; and in the group evaluated late, still 5 percent of the hips required operation. There was good ability for spontaneous remodeling between the age of 3 and 7 years, whereas around the age of 10, the radiographic appearance of the hip became stabilized. At the end of treatment and at the time when the children started to walk, 59 percent of the early evaluated hips were still insufficiently remodeled; but in cases evaluated late, 95 percent of them had a normal or almost normal radiographic appearance. At that time, the clinical state of the children was satisfactory. The results of treatment depended on the initial degree of displacement. Only when treatment was begun after 5 months of age did the patient's age affect the treatment results. The Frejka pillow successfully reduced and stabilized these hips.

[Radiological study of the development of the hip joint in children with congenital hip dislocation treated by the Frejka cushion].

The changes in the radiological state of the hip joints between the 2-nd and the 21-st year of age in persons treated for congenital dysplasia of the hip with dislocation have been determined on the basis of the analysis of 3.005 X-razy pictures of 4 or 6 radiological parameters of 916 hip joints in 708 children and the total point evaluation. On comparing the rate of excellent, good and poor results in individual years of life, it has been concluded that marked and quick rebuilding occurs between the 2-nd and the 7-th year of age, and on the other hand, after 10 years of age the radiological state of the hip joint stabilizes.

[Early results of the treatment of congenital hip dysplasia with dislocation at the Preluxation Clinic in Poznań].

In 1955-1987 over 5,000 children were treated for congenital dysplasia of the hip with dislocation. A uniform method of treatment by the Frejka pillow was used. The condition of 823 hips in 636 children at 3-6 months after the start of walking and at the age of 15-36 months was evaluated. Stable reposition was found in 94 per cent of the cases but those hips were not satisfactorily rebuilt. The degree of rebuilding was determined using four radiological parameters and a point scale. Excellent rebuilding was found in 8 per cent, good in 33 per cent and fair in 59 per cent of the cases. Osteochondronecrosis of the head occurred in 13.6 per cent of the hips (severe in 2 per cent). The used treatment method was determined to be successful. Children should be followed up after the end of treatment for rebuilding of the hip joints.

[Radiologic dislocation value of the dysplastic hip joint in the child].

The discussed index is determined by the comparison of the proportion of localisation of the proximal and of the femur and transverse dimension of the pelvis by x-ray examination. It has been found by radiological measurements of 400 hip joints children in the age of 3-24 months, in whom dysplasia of the hip joint with dislocation was diagnosed. The displacements were divided into 5 grades on the basis of extent of lateral and superior dislocation. The index makes displacement objective, replacing the visual assessment; it allows to compare the displacements on x-ray pictures performed at different ages and in different conditions of x-ray exposition. It has proved to be very useful in diagnosis and analysis of treatment result of large material of congenital dysplasia of the hip with dislocation.
