The prevalence and etiology of impotence in 101 male hypertensive outpatients.

Erectile dysfunction and impotence has a high prevalence among male hypertensive patients. Whether this relates mainly to specific drug side effects or to primary pathogenic disorders is unknown. In the present study 101 male patients from our outpatient hypertension clinic answered detailed questionnaires about hypertension and sexual function. Patients with perceived impotence were offered a thorough penile evaluation and examination performed by specialists in the urology department. Twenty-seven (27%) men had impotence. The main cause of impotence was an arterial dysfunction (89%). The prevalence of impotence was related to the degree of secondary organ manifestation, reflected by World Health Organization (WHO) classification I-III (P = .01). Intermittent claudication (P = .001) and ischemic heart disease (P = .005) were the best determinants in this respect. Twelve impotent patients (44%) ascribed onset of impotence to drug initiation. A variety of drugs were incriminated in the occurrence of drug-induced impotence. In summary our results indicate that impotence in hypertensive men is caused mainly by penile arterial vascular changes, probably atherosclerosis. Drug-induced impotence could well be the result of blood pressure reduction itself and not specific drug side effects.

Transcutaneous nitroglycerine in the treatment of erectile dysfunction: a placebo controlled clinical trial.

Transcutaneous nitroglycerine has previously been reported to be effective in the treatment of erectile dysfunction. We present our results with the drug in a randomised, placebo controlled, double-blind crossover study. The effect of the drug has been evaluated both under laboratory conditions and in a home study. Eighteen men were included in the study. No effect of nitroglycerine could be demonstrated under laboratory conditions by application of the Rigiscan Monitor. In the home study there was no statistical significant effect of transcutaneous nitroglycerine compared to placebo (P= 0.2813).

Bladder diverticulectomy: operative technique.

We present a simple technique of bladder diverticulectomy. Etiology, pathogenesis, diagnosis, and treatment of bladder diverticula are discussed. We recommend this technique for common use, since it is safe and time-saving.

Residual urine in 75-year-old men and women. A normative population study.

The prevalence of residual urine in elderly people in the community is poorly documented. The aim of the present study was to assess the prevalence and range of residual urine in elderly people in the community. As part of a major epidemiologic study (The Glostrup Population Study), 140 75-year-old men and women were included in this study (92 men, 48 women). All were selected at random from the Central Person Register by age and residence. All had a single sonographic measurement of residual urine with a prevoid volume of 150 ml or more. More than 10 ml of residual urine was found in 91 of the 92 men (median 90 ml; range 10-1502 ml), and in 44 of the 48 women (median 45 ml; range 0-180 ml). Our figures correspond to those previously reported in selected patient populations. It must be emphasized, however, that our findings are merely descriptive, as the clinical significance of a single residual urine determination is questionable.

[Androgen insensitivity syndrome--testicular feminization--Morris syndrome. A surgical procedure in cases when a testis is found during herniotomy in a girl].

The androgen insensitivity syndrome is uncommon, but still the most frequent male pseudohermaphroditism. It is an X-linked recessive disorder characterized by androgen receptor defectiveness of various intensity. In the state of complete insensitivity, the male karyotype 46-XY develops a female gender with bilateral testes. Inguinal hernia is a common clinical presentation for a young child with complete androgen insensitivity. Thirty-five to forty percent of those with Morris syndrome come out with incarcerated hernias containing a testis, some even pass unnoticed at elective herniorraphy on apparently normal infant girls. It is recommended to perform orchiectomy when a testis is found during surgery. Subsequently the patient should be referred to major centres for further and thorough surveys.

Analgesic effect and tolerance of Voltaren and Ketogan in acute renal or ureteric colic.

Fifty-six patients with renal or ureteric colic were entered into a randomised, prospective, double-blind investigation of the analgesic efficacy and tolerance of Voltaren versus Ketogan, both administered intramuscularly. There were no significant differences regarding pain-relief but side effects were fewer in patients treated with Voltaren.

Heart rate responses to moderate linear body accelerations: clinical implications in aeromedical evacuation.

Linear accelerations during take-off and landing were measured in both civilian and military aircraft usually involved in aeromedical evacuations in Denmark. Accelerations of similar durations and magnitudes were induced in six healthy subjects transported in an ambulance in different supine positions. Heart rate responses recorded depended significantly on the position of the subject. It was concluded that seriously ill patients must be positioned transversely to the axis of acceleration during aeromedical evacuation.

Ketanserin versus placebo in carcinoid syndrome. A clinical controlled trial.

Seven patients, four women and three men, with a median age of 64 years (range, 53-74 years) were randomized to treatment with either ketanserin, 40-160 mg daily, or placebo. The study included 13 treatment periods with ketanserin and 13 with placebo. All patients had typical symptoms, with flushing as their main complaint. The diagnosis had been verified histologically. All had elevated excretion of 5-hydroxy-3-indoleacetic acid in urine. During the trial ketanserin reduced the number of flushing attacks in five patients (p less than 0.05). Diarrhoea was reduced in two patients. No side effects were observed. Ketanserin seems valuable for the symptomatic relief in the carcinoid syndrome.

The effect of a synthetic met-enkephalin analogue (FK 33-824) on gastric acid secretion and serum gastrin in man.

A synthetic met-enkephalin analogue was infused into seven healthy human subjects on a background infusion of pentagastrin to study the effect on gastric acid secretion and serum gastrin concentration. Gastric acid secretion rose from 4.2 meq H+/15 min to 7.8 meq H+/15 min during infusion of met-enkephalin, 0.1 micrograms/h. Met-enkephalin in a dose of 1 microgram/kg/h had no effect on pentagastrin-stimulated acid secretion. Serum gastrin concentration remained unchanged throughout the study. It is suggested that met-enkephalin could be a partial agonist on the parietal cell. The possible role of met-enkephalin in the cephalic phase of gastric acid secretion is discussed.
