Neuro-muscular dysfunction of the lower urinary tract in multiple sclerosis.

Urological problems in M.S. patients are very frequent and are usually very disabling. Complaints will often be misleading. Urodynamic investigations play a most important part in the diagnosis but are carried out with more difficulty than in other patients. The lower urinary tract dysfunction is classified as advised by the I.C.S. Committee. Detailed figures are given in the text. Evolution of urological disorders in M.S. is unpredictable. Treatment has to be conservative, flexible and reversible if possible. The goal of the treatment is to get a balanced bladder and complications due to urological manipulations must be avoided.

Long-term alpha-adrenergic-blocking therapy in detrusor-urethra dyssynergia.

Neurogenic dyssynergia was treated with alpha-adrenergic blockers. Only 21 of 43 highly selected patients with residual urines and low flow rates continued therapy with phenoxybenzamine for more than 6 months, despite initial benefits. The causes of therapy discontinuation are discussed. A combination of 5 mg dibenzylin, 3 times daily, with parasympathomimetic drugs is often necessary. The mechanisms of action of alpha-adrenergic-blocking agents are discussed.

Frequency analysis of the electromyographic activity in striated pelvic floor muscles: a preliminary report.

A power spectrum of the electromyographic signals in urethral and anal sphincters has been analyzed in 25 patients. Different spectra are observed in each muscle, with higher frequencies in the urethral sphincter. The high frequencies are manifest at short brisk contractions (e.g. cough) and disappear quicly during a sustained voluntary contraction. High frequencies are less pronounced in stress incontinence, but increase in spastic syndromes. The hypothesis is advanced that the proportion of active tonic and phasic motor units determines the efficiency of sphincter reactions.

[Micturitional disorders in multiple sclerosis (author's transl)].

Micturitional disorders in multiple sclerosis have often an insidious onset; they aggravate during acute attacks of the nervous disease and are strongly influenced by the general condition of the patient, the evolution of his muscular spasms and the therapeutical measures. This changing behaviour of the bladder makes it necessary to perform repeated cystometries, uroflowmetries and electromyographies of the striated sphincter in order to adapt vesical therapy to the evolutive stadia of the disease. Although good results are frequently obtained by drugs (sympathicomimetics, sympathicolytics, muscle relaxants, parasympathicomimetics, parasympathicolytics), by transurethral resections of internal and/or external sphincters and by phenolisation, we feel that urinary derivations remain indicated for some patients, especially in order to facilitate social integration.
