Stromal sarcoma arising on endometriosis. A clinicopathological and immunohistochemical study of 4 cases.

The development of stromal sarcomas on the foci of endometriosis is extremely rare and the differential diagnosis from other tumors of myogenic, vascular, hemopoietic or epithelial origin may present great diagnostic difficulties. We investigated the clinicopathological and immunohistochemical characteristics of 4 cases of endometrial stromal sarcoma that developed on endometriotic foci of the uterus, vagina and omentum. Thye were classed as high grade (1/4) or low grade (3/4) malignant potential tumors, according to their mitotic activity. Immunohistochemically these tumors gave a positive reaction to vimentin, but were negative to desmin, smooth muscle actin, factor VIII, EMA and LCA. These characteristics permit their identification and a proper therapeutic approach.

Human papilloma virus in verrucus carcinoma of the vulva: an immunopathological study of three cases.

The pathological features of three cases of verrucus carcinoma of the vulva, examined in our laboratory during the last decade are presented. Verrucus carcinoma of the vulva is of special interest because of its rarity (6.5% of our cases of malignant lesions of the vulva), its special morphology and the problems in differential diagnosis that are encountered. There is also evidence that there is an association with HPV infection. We present the pathological features of three cases of verrucus carcinoma that we studied in the last decade in our Laboratory among 48 malignant lesions of the vulva. An immunohistochemical detection of HPV was performed by the streptavidine-viotine method and classification by in situ hybridization showed the presence of HPV 6/11 in all cases.

Transvaginal color Doppler and CA125 as tools in the differential diagnosis of postmenopausal ovarian masses.

OBJECTIVE: To evaluate whether transvaginal color Doppler and CA125 are valid in differentiating malignant from benign postmenopausal ovarian masses. METHODS: Sixty-two women with ovarian masses were studied with transvaginal color Doppler ultrasound before surgery. Serum CA125 levels were also measured. Resistance index (RI) and pulsatility index (PI) were calculated from the waveforms generated from blood flow within the ovary. RESULTS: Benign tumors had a significantly higher pulsatility index (mean 1.23 +/- 1.02; range 0.65-2.99) and resistive index (mean 0.78 +/- 0.22; range 0.5-1.1) than did malignant tumors. However some overlap in individual values for benign and malignant lesions was found. RI and PI sensitivity were significantly higher than those with CA125 (P < 0.05). Blood flow was detectable by color Doppler imaging in 95% of cases with malignant masses. CONCLUSION: Doppler sonographic evaluation of resistance and pulsatility indexes in the vessels of ovarian masses together with CA125 increased the sensitivity of prediction of malignancy for pelvic masses, but further work is needed before the validity of these factors is proved.

Aggressive angiomyxoma of the pelvis: a clinicopathologic study of a case.

A case of aggressive angiomyxoma in a 38-year-old pregnant woman is described. These clinicopathologically distinctive soft tissue tumors comprise a quite rare entity. They are usually located in the pelvic or perineal soft parts and occur predominantly in women. On gross inspection, these lesions are typically soft bulky masses, frequently with a multilobulated contour and of gelatinous appearance. They are characterized by slow growth, infiltration of adjacent structures, absence of distant metastases and a tendency to recur locally, probably secondary to incomplete excision. Therefore, treatment should consist of wide surgical excision, as complete as technically possible. This is the second case of aggressive angiomyxoma of the pelvis occurring in a pregnant woman.

The diagnostic value of laparoscopy in 2365 patients with acute and chronic pelvic pain.

OBJECTIVE: To discriminate the etiology in 2365 patients with acute and chronic pelvic pain (APP, CPP). METHODS: Diagnostic laparoscopy was carried out in 736 patients (31.1%) with APP and in 1629 (68.9%) with CPP. In 315 patients (13.3%) the diagnostic procedure was extended to operative laparoscopy. RESULTS: The most frequent laparoscopic findings in patients with APP were acute salpingitis and pelvic adhesions (22.8%) and ectopic pregnancy (19%), while in patients with CPP the most frequent findings were pelvic adhesions (35.4%) and endometriosis (24.6%). In 7.5% of patients with APP and 24% with CPP, laparoscopy did not reveal any pathological finding in the pelvis. Among the 315 patients in whom operative laparoscopy was carried out, 40% suffered from APP and 60% from CPP. In the 446 patients (18.9%) without laparoscopic findings no treatment was given, while of the remaining 905 patients 40% were subjected to laparotomy and 60% received conservative treatment. The total incidence of side effects reached 4.7% and serious side effects resulting from emergency laparotomy occurred in 0.15% of patients with pelvic pain. CONCLUSION: Our results in a large group of patients with pelvic pain show that there are discrepancies in the incidence of laparoscopic findings between patients with APP and CPP. Discrepancies between the two groups of patients were also found during operative laparoscopy, the treatment administered after laparoscopic diagnosis and the complications encountered.

Genuine stress incontinence: prospective randomized comparison of two operative methods.

Eighty-one women with clinical and urodynamic findings of genuine stress incontinence and genital prolapse were randomly selected to be surgically treated with either anterior colporrhaphy or Burch colposuspension. Each patient had a complete clinical and urodynamic evaluation before surgery and at 2 months and 3 years after surgery. Differences in cure rates between the two procedures at the 2-month post-operative evaluation were insignificant; however, at the 3-year post-surgical evaluation, the cure rate of women who had undergone Burch colposuspension was significantly higher than that of women who had undergone anterior colporrhaphy (cure rates were 88% and 57%, respectively; P < 0.001). The Burch colposuspension was more effective than the anterior colporrhaphy in the stabilization of the bladder base, neck and proximal urethra as confirmed by transvaginal sonography. Post-operative spontaneous voiding was uneventful in both procedures. Results of this study demonstrate that the Burch colposuspension in our hands was more effective in treating genuine stress incontinence and pelvic relaxation than was anterior colporrhaphy.

Ovarian surface serous papillary carcinomas: a clinicopathologic study.

Between 1982 and 1992, 129 patients with malignant epithelial ovarian tumors were reviewed with identification of 10 patients having surface papillary serous carcinoma of the ovary and one having peritoneal papillary carcinoma. The gross operative specimens, histopathologic condition, and treatment records were reviewed. All patients had disease involving the omentum, and the abdominal and pelvic peritoneum, and they all corresponded to stage III, according to FIGO. The median age at presentation was 58-years (age range, 43 to 73 years). All patients had a total abdominal hysterectomy and bilateral salpingo-oophorectomy, and all received chemotherapy. The median survival time for all patients was 15 months. Three patients are alive 3 to 4 years after the initial diagnosis. In conclusion, surface serous ovarian cancers and peritoneal papillary serous cancers have common histologic appearance and the same responsiveness to surgical therapy and to chemotherapy and should be treated similarly.

Diagnostic histologic criteria of atypical adenomatous hyperplasia and well differentiated adenocarcinoma of the endometrium.

The purpose of the study was to evaluate certain histological features of adenomatous hyperplasia diagnosed in endometrial curretings, that will enable us to distinguish atypical glandular hyperplasia from early well-differentiated adenocarcinoma. The material for this study was composed of 519 endometrial hyperplasias diagnosed in curettings. Of these, 244 were adenomatous hyperplasias, 109 were cystic hyperplasias, 93 mixed types and 73 were atypical hyperplasias. The cases of atypical hyperplasia were subsequently operated on, and among them, 26 proved to be adenocarcinomas. All histological sections of the currettings showing the atypical hyperplasia were re-evaluated to find criteria diagnostic of adenocarcinoma. The authors have considered as diagnostic criteria the stroma infiltration, the mitotic activity, the nuclear atypia, the stratification of nuclei and the presence of necrosis. The stroma infiltration seems to be the most important indication of an adenocarcinoma of the endometrium.

The role of steroid hormones in cervical ripening.

Dehydroepiandrosterone sulphate (DHEA-S), estradiol (E2), estriol (E3), progesterone (P4) and cortisol (F) maternal plasma concentrations were determined using enzyme or fluoroimmunoassays in 28 healthy pregnant women prior to the induction of labour, performed for various reasons, in order to evaluate their impact on cervical ripening. Mean maternal and gestational ages were 24.7 +/- 3.6 years and 39.1 +/- 1.9 weeks, respectively, and the gravidity ranged from 0-4. The subjects were divided according to cervical maturity into Group A (n = 13, Bishop scores > 4) and Group B (n = 15, Bishop scores < 4). There were no statistically significant differences observed between the two groups as to concentrations of oestradiol, oestriol, progesterone and cortisol. However, significantly higher concentrations of DHEA-S were found in group A compared to group B (1335 +/- 885 ng/ml and 558.2 +/- 191.4 ng/ml, respectively), (p < 0.001). In conclusion, the high DHEA-S concentrations in maternal plasma may play an important role in pregnancy by producing favourable cervical conditions for delivery or by triggering the labour itself.

Blighted ovum: ultrasonic, histopathologic and hormonal considerations.

Thirty women experiencing early pregnancy failure (blighted ovum) were studied at frequent intervals with serial measurements of serum human chorionic gonadotropin (beta-hCG), progesterone (P4) and estradiol (E2). The diagnosis of blighted ovum had previously been made by careful ultrasonic examination (s). Histopathologic findings in the placenta were studied in all cases. Hormonal levels were variable and in most of the cases indistinguishable from normal pregnancies. In only 50% of the cases the histopathologic examination revealed findings compatible for the blighted ovum criteria.

The diagnosis of luteal phase defect using different diagnostic criteria.

We evaluated the real incidence of the luteal phase defect (LPD) syndrome in 149 women who previously had been diagnosed by four independent researchers as having it, by the use of a combination of 3 criteria: basal body temperature (BBt), the histological appearance of the endometrium (EB) and the serum level of progesterone (P). The women had been divided according to etiological factors of the LPD syndrome into 4 groups (A, B, C and D). When the 3 criteria were used simultaneously (category II), the incidence of LPD among the patients was 41%; when P and EB (category III) and P and BBt (category V) criteria were used, the incidence was the same in each case (45%). On the contrary, with the use of BBt and EB (category IV) criteria the incidence was 65%. Furthermore, when the above-mentioned categories were used in each of the groups separately, the smallest deviation between the initial and final diagnosis of LPD was found in groups A and B, whereas the largest deviation was found in groups C and D. We conclude that the highest incidence of patients who remained with the diagnosis of LPD was observed after utilization of category IV (EB and BBt) criteria and we recommend that, in cases where LPD is diagnosed based on P levels, only higher serum levels be accepted as indicative of the syndrome.

Culture of seminal fluid in infertile men and relationship to semen evaluation.

Bacterial flora of the seminal fluid and its influence on semen quality, was examined in 225 asymptomatic unselected men. Each semen sample was cultured aerobically, anaerobically, for genital mycoplasmas, and for Chlamydia trachomatis. Semen analysis was made according to standard methods recommended by the W.H.O. All 225 semen samples had microbial isolates. All isolates had colony counts of 10(2) colony forming units (cfu/ml). Thirty-three cases had greater than 10(2) cfu/ml, 85 cases had greater than 10(3) cfu/ml and 78 cases greater than 10(5) cfu/ml. The most common organisms isolated were Ureaplasma urealyticum in 86 samples and C. trachomatis in 26 samples. The most frequent abnormal parameters were viability (117 of 212, 52%), motility (85 of 212, 40%) and number of sperm cells (74 of 225, 32.8%). No significant correlation was found between abnormal semen parameters and presence of U. urealyticum, and C. trachomatis. We concluded that asymptomatic bacteriospermia (infection) in the semen did not significantly affect the count, motility or morphologic features of the specimen.

Sexually transmitted diseases in women with urethral syndrome.

Frequency of micturition and dysuria were prominent symptoms in 135 (57%) of 237 women with urethral syndrome. Ureaplasma urealyticum, Mycoplasma hominis and Chlamydia trachomatis were the principal organisms associated with the urethral syndrome (38.41%, 28.14% and 11.11%, respectively). Escherichia coli was cultured from four patients and Herepes genitalis and Neisseria gonorrhoea were isolated from five patients. Infections with more than one organism were frequent. Thirty-one of 135 patients were infected by two organisms, 27 by three and 4 patients by four microorganisms. Vaginitis due to Garnerella vaginalis, Candida spp. and Trichomonas vaginalis was discovered in 52 (39%) of 135 patients.
