Long-term bladder dysfunction in boys with posterior urethral valves.

We report a series of 60 children operated on for posterior urethral valve (PUV) before the age of 5 years and followed up for 10 years. This work aims to study the vesico-sphincteric sequelae of this malformation. 38 patients showed early urinary incontinence post-operatively. Of them, 20 were re-evaluated 10 years later by pelvic ultrasonography, pressure flow studies combined with EMG and assessment of renal function. 8 patients refused to undergo these investigations and 10 were lost to follow-up. Of the 28 evaluable patients, 22 are currently continent and a complete urodynamic study was performed for the 6 (10%) incontinent patients. Of them, 3 showed uninhibited detrusor contractions. The vesical compliance was frequently within the normal limits. No case of detrusor-sphincteric dyssynergy or urethral hypotony was found. 2 were successfully treated by oxybutinin and biofeedback reeducation, 2 were partially improved but their prostates are still in growth and the possibility of implanting an artificial sphincter was discussed in the remaining 2. The urodynamic evaluation of those patients in the long-term is encouraging.

Anaerobic treatment of condensates: trial at a kraft pulp and paper mill.

The Domtar Papers pulp and paper mill in Windsor, Quebec, Canada, investigated the potential for anaerobic treatment of contaminated kraft mill condensates. The objectives of this project were to assess the technical feasibility of replacing the steam stripper with anaerobic treatment, to provide basic information for the design of an anaerobic treatment process for condensates, and to provide information on treated condensate quality for eventual reuse. The project involved extensive chemical characterization of condensates, followed by treatability tests. The tests included laboratory bench-scale tests and on-site pilot testing using direct feed from the process. Characterization showed that the organic content of the condensates was essentially methanol, as expected, but that foul evaporator condensates had high sulfide contents. It was found that undiluted foul condensates at the Windsor mill are toxic to the anaerobic biomass because of these high concentrations of sulfides. Treatment of combined condensates is possible at an approximate volumetric loading of 10 to 12 g/L.d chemical oxygen demand (COD) with good production of biogas (0.35 L/g of COD removed) and excellent methanol removal (better than 95%). The biogas produced is of excellent fuel quality with close to 90% methane, but with a high sulfide content (close to 4%).

[Endoscopic treatment of prenatally diagnosed ureteroceles].

INTRODUCTION: The authors report a series of 16 infants presenting with an antenatally diagnosed ureterocele, treated immediately, during the neonatal period, by endoscopic incision. The objective of this treatment is to ensure effective decompression of the ureterocele. MATERIAL AND METHODS: The immediate postnatal assessment revealed 17 ureteroceles, including 7 intravesical ureteroceles (with one bilateral form) and 10 ectopic ureteroceles. Endoscopic opening allowed collapse of all of the ureteroceles and usually ensured improvement of renal function. Vesicoureteric reflux was observed secondarily in 7 infants with an ectopic form of the disease, and was present primarily in two other patients. RESULTS: In patients with an intravesical form, endoscopic treatment was found to be effective on its own and no complementary procedure was required. In contrast, in ectopic forms, a second operation was required in 9 cases: presence of vesicoureteric reflux (8 cases) required resection of the ureterocele with vesical reconstruction and ureteric reimplantation according to Cohen's technique. In one case, total ureteronephrectomy was performed due to complete absence of function of the kidney concerned. CONCLUSION: Endoscopic opening, performed soon after birth, can be considered to be the first-line treatment of antenatally diagnosed ureteroceles. Its main complication remains the secondary development of vesicoureteric reflux Radiological surveillance therefore appears to be essential.
