Interposition mesocaval shunt for chronic primary occlusion of the hepatic veins.

Five patients with primitive chronic Budd-Chiari syndrome were treated by Dacron interposed mesocaval shunts for medically uncontrollable ascites. In two instances, hepatomegaly and ascites disappeared for four and four and one-half years. In one patient with severe stenosis of the inferior vena cava, moderate ascites required tapping once a month one year later, despite proved prosthesis patency. In two patients, death occurred ten and 30 days after shunting due to thrombosis of the graft. Inferior vena cava stenosis appears to be the major factor for decision of opportunity and type of portacaval shunt. From our material, we can describe three types of stenosis: type I, due to caudate lobe hypertrophy, and type II, due to right lobe hypertrophy, are suitable for side-by-side portacaval or mesocaval shunts. Type III, regular and extended narrowing of inferior vena cava, observed in long term evolutive forms, is presumably due to fibrosis and is not a good indication for conventional infrahepatic shunting procedures. Since this study was completed, another patient had a side-to-side portacaval anastomosis for chronic Budd-Chiari syndrome without caval stenosis. The patient has been observed for seven months, and ascites did not reappear. This underlines the importance of a complete radiologic and hemodynamic preoperative study of inferior vena cava outflow impairment.

The authors formulate some safety rules in view of two case histories of bowel complications following voluntary termination of pregnancy using suction techniques. Where it is not certain that a perforation has occurred laparoscopy is necessary. When perforation has been confirmed a laparotomy is undertaken in order to check the state of the bowel throughout its length, because when lesions of the digestive tract are treated rapidly the results are good whereas when they have to be treated for peritonitis the outlook is poor.

Presence of antigen-antibody complexes in antiallogeneic and antixenogeneic sera.

Antilymphocyte sera (ALS), either allogeneic (for example, Lewis anti-August rat serum) or xenogeneic (the classic ALS), may retain the antigen used for the immunization of the animal producing the serum in the form of antigen-antibody complexes, at least when the animal is bled within the first 2 weeks following the last immunizing injection. Evidence for this has been obtained in vivo and in vitro. In vivo experiments showed that injection of Lewis anti-August serum to a normal Lewis rat can induce a delayed active immunization against the August strain. In vitro studies proved the presence of August antigens in the anti-August sera, by precipitation of the antigen-antibody complexes with 6 M urea, removal of the antibody by filtration, and demonstration that the filtrate specifically inhibits complement-dependent cytotoxicity of anti-August antibodies. The interpretation of a number of previous works on so-called passive enhancement of grafts or on various effects of ALS may have to be reconsidered on the basis of these findings.

Animals
