Pregnancies following the interval double-transfer technique in an in vitro fertilization-embryo transfer program.

In 16 women undergoing IVF-ET, with more than four embryos available, two transfers were performed (24 hr apart), instead of one. The pregnancy rate was 25%, as compared with a pregnancy rate of 6.2% in 16 women having fewer than four embryos available and in whom one conventional transfer was performed. Since in our IVF-ET program, we do not freeze the extra embryos available, the purpose of the double transfer was to use all the embryos available and to improve the chances of finding the "implantation window" open. It is our feeling that multiple transfers may be beneficial to improve the pregnancy rate in an IVF-ET program without freezing facilities. However, a larger controlled clinical trial is necessary, in order to establish the advantages of multiple transfers in terms of pregnancy rate in comparison with the potential disadvantages.

Patent ductus arteriosus after prolonged treatment with indomethacin during pregnancy: case report.

Indomethacin was administered to suppress premature uterine contractions in a twin pregnancy from the 27th until the 32nd week of pregnancy. Patent ductus arteriosus was diagnosed after delivery in one twin and persisted patent for a period of 4 weeks. A hypothesis of the constrictive influence of prostaglandin synthetase inhibitors on the fetal ductus arteriosus during intrauterine and neonatal periods is suggested.

Continuous extraovular prostaglandin F2 alpha instillation for late pregnancy termination in patients with previous cesarean section delivery.

Late pregnancy termination by continuous extraovular instillation of prostaglandin F2 alpha (PGF2 alpha) was successfully performed in 13 patients with previous cesarean section (CS) delivery. The indications for pregnancy termination were intrauterine fetal demise in nine patients, social indications in two patients. Down's Syndrome in one patient and quadruplets pregnancy in one patient. One patient had a previous classical vertical cesarean section, two had two previous and one three previous low segment transverse (LST) cesarean sections. The other nine patients had one previous low segment transverse cesarean section. The instillation of prostaglandin solution was obtained by using a new double balloon catheter designed for extraovular pregnancy terminations. All patients aborted following the procedure with no complications. The instillation abortion time ranged from 5 1/2 to 20 h (mean 13.3 h).

Continuous extraovular prostaglandin F2 alpha instillation for second-trimester pregnancy termination.

One hundred midtrimester pregnancy terminations were successfully induced with continuous extraovular infusion of prostaglandin F2 alpha (PGF2 alpha). The solution was infused into the extraovular space with a newly developed double-balloon catheter. The mean instillation abortion time was 12.9 h. Abortion was completed in 48% of the patients within 12 h, in 80% within 18 h, and in 96% within 24 h. The mean total dose of PGF2 alpha instilled was 12.4 mg/patient. Only four patients aborted after 24 h from the insertion of the catheter and instillation of the PGF2 alpha. Side effects were few. Thirteen patients (13%) had nausea and vomiting, and 14 (14%) had transient pyrexia (above 38 C). No major complications occurred. It appears that this method has a high success rate, good patient tolerance, and an acceptable safety factor with few minor side effects.

A new method for rapid determination of sperm concentration in bull and ram semen.

A simple method for rapid evaluation of bull and ram sperm concentration is described. In this technique, a sample from an undiluted specimen was placed in a special 10-mum counting chamber and examined either by an ordinary or a phase-contrast microscope. The pattern distribution of the observed spermatozoa was matched as closely as possible with one of five pictures of a standard scale. This scale was prepared from serial photomicrographs that ranged from 100 to 1500 million per ml. The number that appeared at the corresponding photograph immediately indicated the sperm concentration of the tested specimen. In this way values up to 1500 million per ml could be determined rapidly with no further procedures. More concentrated specimens needed slight dilution to make them suitable for direct evaluation. Accuracy and reliability were statistically evaluated; the mean deviation from the conventional method was less than 15.2% with confidence level of 95%.

Maternal pulmonary edema occurring after therapy with ritodrine for premature uterine contractions.

Intravenous administration of beta-sympathomimetic drugs in combination with betamethasone or dexamethasone is a treatment often used for the suppression of premature uterine contractions and for the stimulations of fetal lung maturity. It has been suggested that the combination of these drugs may be responsible for maternal pulmonary edema and five such cases have been reported in the literature (1,2). We report a patient who developed pulmonary edema during ritodrine therapy. Corticosteroids were not given in this case.

Use of a sealed mini-chamber to investigate human sperm motility in real time under aerobic and anaerobic conditions.

To study the correlation between metabolism and motility, ejaculated human spermatozoa were washed in media containing glucose, pyruvate, and deoxyglucose in various combinations. Spermatozoa suspended in these media were incubated in sealed mini-chambers and subjected to aerobic or anaerobic conditions at 37 degrees C. The effect on the patterns of sperm motility was investigated in real time by direct observation and objective determination with the multiple exposure photography (MEP) method. The motility of spermatozoa incubated in media containing excess of glucose showed similar changes of motility quality with time, whether exposed to aerobic or anaerobic conditions, and in both cases motility lasted about 13 h. Motility of sperm incubated with pyruvate only was of a much lower quality, especially under anaerobic conditions, and in both circumstances lasted about 7 h. When glycolysis of fructose remnants was totally inhibited by deoxyglucose and sperm were incubated with pyruvate only, motility lasted for 2 h under aerobic conditions and only for about 1 h under anaerobic conditions. It is concluded that the main metabolic process that supplies energy for sperm motility is glycolysis, under both aerobic and anaerobic conditions. Oxidative respiration was less efficient as a source of energy for sperm motility. When glycolysis was inhibited and oxidative respiration was eliminated under anaerobic conditions, sperm motility lasted only for about 1 h, probably by using intracellular energy reserves.
