Approximal caries and frequency of bitewing examinations in Swedish children and adolescents.

The aim of this study was to assess the number of, and time intervals between, bitewing examinations performed on a group of Swedish patients between 3 up to and including 18 yr of age and to relate them to the accumulated posterior approximal caries experience of the patients as found in the last bitewing examination before age 19. The patients showed a marked variability in approximal caries experience. 25% accounted for about 1% of the total number of carious lesions and restorations accumulated up to age 19 while 25% accounted for 60%. From age 9 up to age 18 more than 75% of the patients were subjected to at least one bitewing examination annually. Between 10 and 15 yr of age more than 90% received annual radiographic examinations. Mean number of bitewing examinations was 10.4 and the average time interval between bitewing examinations was 11.5 months. 11% of the variation in the number of bitewing examinations and 8% of the variation in average time intervals between bitewing examinations could be explained by the number of lesions and restorations in the approximal surfaces of the patients accumulated up to age 19.

Predicting development of approximal dentin lesions by means of past caries experience.

The aim of this study was to assess the accuracy in predicting future development of approximal dentin lesions using radiographically observed numbers of restored and carious surfaces at age 13 as diagnostic criteria. ROC curve technique was applied employing the development of at least one new dentin lesion from age 13 to age 15 and 18 respectively, as validating criterion. Two groups of patients were considered, residing in communities with water fluoride content of 0.2 ppm and 1.2 ppm, respectively. Results showed that the development of dentin lesions could be predicted with an accuracy similar to that of many other dental radiographic procedures. This implies that it ought to be possible to use previous caries experience as a means of individualizing the scheduling of future radiographic procedures.

Intervals between bitewing examinations in young patients when applying a radiologic algorithm.

Every prescription of a radiograph should be based on an evaluation of the individual patient benefit. An algorithm developed to make it possible to individualize the frequency of bitewing radiographs was applied on a group of young patients in order to evaluate if its application could increase the time intervals between bitewing examinations. The time intervals between bitewing examinations actually performed on 221 patients from exfoliation of all primary teeth up to and including 18 yr of age were determined. The number of surfaces with carious lesions as well as restorations in the posterior approximal surfaces were diagnosed from the radiographs. The intervals between bitewing examinations according to the algorithm were estimated and compared to the actual ones. For most patients the time intervals between examinations increased when the algorithm was applied, without a concomitant risk of missing lesions developing into the deeper parts of the dentin. The results render support to an individualized scheduling of bitewing radiographs based on number of and extent of approximal lesions in baseline radiographs.

Orofacial functions in patients with congenital and acquired maxillary defects: a fluoroscopic study.

By means of videofluoroscopic recordings, chewing, swallowing and speech were monitored in nine patients with congenital and acquired maxillary defects. All of them were rehabilitated with a maxillary obturator prosthesis. The defective region was seldom used for chewing. No leakage between the obturator and surrounding tissue was observed, either for solids or for liquids. All of the prosthetic reconstructions were surprisingly stable during function. In the phonetic analysis, speech production was judged to be restored almost to normal. Videofluoroscopy may be valuable as a complement to other functional diagnostic procedures, and for the evaluation and improvement of rehabilitation with an obturator prosthesis.
