[Mathematic evaluation of the deterioration of the renal function in chronic progressive kidney diseases].

The progression of chronic renal failure has been claimed to be predictable by means of mathematical models. The study shows that the logarithm of serum creatinine is less accurate than the reciprocal method for predicting the course of renal insufficiency. The fact that the reciprocal of serum creatinine decline linearly with time suggests that glomerular function is lost at a constant rate.

Standardization of 68(Ge+Ga).

The radionuclide 68Ga is mainly a positron emitter (89.2%), with a half-life of 67.7 min. It is used in nuclear medicine, being chemically extracted from the mixture 68(Ge+Ga); its precursor, 68Ge, disintegrates 100% by electron capture, with a half-life of 270.8d (Table of radionuclides, comments and Evaluation). A 4pibeta-gamma coincidence method was used for standardization, with a 4pi proportional beta-detector and a NaI(Tl) gamma detector. Registration of the capture radiations was avoided using foil absorption and a high beta threshold. Using supplementary foils for positron absorption, extrapolation graphs were obtained, with a mean slope of -4.4%. Care was taken to compensate for the loss of 68Ge during the preparation of solid sources for measurement. A combined uncertainty of 1.1% was estimated.

Low-level gamma spectrometry using beta coincidence and Compton suppression.

A low-level gamma-ray spectrometry system was developed using a Ge(Li) detector with 6% relative efficiency coupled to a 2pi beta plastic detector for coincidence selection and a massive NaI(Tl) detector for Compton suppression. The integral background count rate for (50-1500)keV was 0.5 s(-1)kg(-1) (Ge), using only beta coincidences. With Compton suppression, a value of 0.25 s(-1)kg(-1) (Ge) was obtained. Spectra with and without Compton suppression were studied for 60Co, 137Cs and 152Eu point sources. Considerations are made concerning the Compton suppression advantages in different situations.

Transjugular intrahepatic portosystemic shunt in adult liver recipient with delayed graft function.

BACKGROUND: Transjugular intrahepatic portosystemic shunt (TIPS) has become an effective treatment for the complications of portal hypertension. We assessed the feasibility and outcome of TIPS in liver transplant recipients who developed delayed graft function (DGF) with portal hypertension. METHODS: From June 2003 to June 2004, 80 cadaveric orthotopic liver transplantation (OLTx) have been performed at our institution. Five patients (6.25%) developed DGF with hyperbilirubinemia and ascites with severe portal hypertension and were treated with TIPS placement (in the 6-month time period from the transplantation). RESULTS: There were no complications related to the procedure. No episodes of encephalopathy were seen. Four patients had better control of the ascites. In one case, we observed complete recovery of the transplanted liver with normalization of the liver function test. Three patients underwent retransplantation (within 7 days from the TIPS), whereas 1 is still on the list 6 months after TIPS placement with recurrent episodes of ascites. CONCLUSIONS: In our preliminary series, TIPS reduced dramatically the portosystemic gradient and improved clinical conditions. The results were negatively affected by the fact that the transplanted liver did not recover its function.

Pediatric liver transplantation: preliminary results at Istituto Mediterraneo Trapianti e Terapie ad Alta Specializzazione.

Between July 2003 and November 2004 14 pediatric liver transplantations (LTx) have been performed in 12 children using cadaveric donors. The primary diseases were as follows biliary atresia in 9 cases, whereas the other 3 children were affected by cystic fibrosis, Langherans cells histiocytosis, and hepatoblastoma, respectively. Median patient waiting time was 103 days (range, 2-158); no patient died while on the waiting list. Patients who underwent transplantation included 7 boys and 5 girls, ranging in age from 6 months to 14 years (median age, 5 years). Recipient median weight was 16 kg (range, 6-38). Donor median age was 19 years (range, 3-47), whereas donor median weight was 74 kg (range, 15-90). All children who underwent primary LTx were United Network for Organ Sharing (UNOS) status 2B. Of the 12 transplanted patients, 9 received a left lateral segment (LLS) from an in situ split liver, whereas 3 received a whole graft. Two children developed an episode of acute cellular rejection on the seventh postoperative day, which was treated successfully with a course of intravenous steroids for 3 days. After a median follow-up of 245 days, 10 children are alive but 2 children died due to primary nonfunction (PNF) on the second postoperative day and septic shock on the fifth postoperative day after retransplantation for acute hepatic artery thrombosis, respectively. One child who underwent retransplantation for hepatic artery thrombosis on the 31st postoperative day after primary LTx is currently alive. Evaluation of our initial data suggests that the split liver technique has the potential to meet the needs of pediatric LTx allowing grafting early in the course of the original disease and reducing waiting time.

Emission probabilities of the KX-rays following the decay of 237Np in equilibrium with 233Pa.

Following international EUROMET Project No. 416 and our recent measurement of the emission probabilities of the main gamma-rays of 237Np in equilibrium with 233Pa, a complementary work has been undertaken to determine the photon emission probabilities of the KX-rays associated with the decay of these nuclides. Two different analysis methods have been used: the KX-rays region was analyzed by fitting both Voigt and full response functions. Photon emission probabilities were determined with a relative uncertainty of about 2%.

Precise measurement of the activity of 186Re, 188Re radiopharmaceuticals.

The paper presents the results obtained in the assurance of traceability along the whole chain: production, distribution, use in hospitals, for the activity measurement of radiopharmaceuticals containing 186Re, 188Re and 186Re + 188Re mixtures. Standardization of the solutions, 4piPC-beta-efficiency relations and responses of a Centronic IG 12/20A ionization chamber and other radioisotope calibrators are presented. Gamma-ray and bremsstrahlung radiation contributions in the total response were evaluated. Some of the relevant results were compared with literature data.

Standardization and decay data of 237Np.

This paper reports contributions from participants in the EUROMET project (No. 416) which was entitled "237Np research into problems relating to purification, characterization and standardization". Primary standardizations were made by the defined low solid angle, coincidence, 4pi alpha, 2pi alpha and liquid scintillation counting methods. Secondary standardizations were made with calibrated gamma-ray spectrometers. Absolute X-ray, gamma-ray and alpha-particle emission probabilities were also determined. The results for the successful conclusion of both primary and secondary standardization are presented together with the values for alpha-particle and gamma-ray emission probabilities determined in this exercise. Several significant inconsistencies remain with the gamma-ray emission probabilities, and these are highlighted.

Standardization of 152Eu.

152Eu was standardized within the frame of a BIPM international comparison. The solution was prepared and bottled by PTB. The ampoules were measured at BIPM in the SIR ionization chambers and then dispatched to the participating laboratories. In the Radionuclide Metrology Laboratory of IFIN-HH, the solution was standardized using a coincidence method with "beta-efficiency extrapolation" by foil absorption. A simple 4pi beta-gamma-coincidence system was used, with NaI(Tl) and 4piPC detectors. No significant impurities were found apart for 0.5% 154Eu. To obtain linear extrapolation plots, a "gamma-window" of 180-1500 keV was chosen. Dead-times of 10 micros were used. A combined uncertainty of 0.24% was obtained, mainly from the extrapolation procedure component.

Standardization of 89Sr.

Two methods were used for the standardization of the pure beta-emitter 59Sr solution received in the framework of the 2000 BIPM comparison of activity measurements. The first method was by scintillation counting using the triple to double coincidence ratio (TDCR) technique. The existing three detector-system was improved by the inclusion of a MAC-3 module produced by LNHB-Saclay. This module contains the circuits for the coincidence, dead time and gate functions. This was the first use of this equipment in an international comparison. The DETECSZ program (LNHB) was used to obtain the activity concentration, at the reference time, as 26.09+/-0.21 kBq g(-1). The second method was the efficiency tracer technique, using a 60Co standard solution and a 4pi beta-gamma-coincidence system. The radioactivity concentration was calculated from a general extrapolation curve, by using data from several sources. The value was 26.58+/-0.28 kBq g(-1) on the reference date 01.10.2000, 00 h UTC. The two results are in agreement within the limits of their combined uncertainties.

Sulphur amino acid pattern in chronic liver disease.

The metabolism of sulphur-containing amino acids is impaired in patients with advanced liver disease, but very few data are available in less severe chronic liver disease. We measured fasting plasma levels of methionine, cystine and taurine in 10 healthy subjects and 50 patients with biopsy proven liver disease: chronic persistent/active hepatitis (30 cases), compensated cirrhosis (10 cases) and decompensated cirrhosis (10 cases). Hypermethioninemia (up to 10 times control values) was present only in decompensated cirrhosis. Cystine was markedly reduced in patients with compensated chronic liver disease, while in advanced cirrhosis its concentration was within the normal range. No differences in taurine plasma levels were observed between the various groups. This study suggests that a derangement in sulphur amino acid metabolism, possibly located at various steps along the trans-sulphuration pathway, is also present in mild forms of chronic liver disease.
