The relationship between loneliness and social interaction in women prisoners.

The increase in women prisoners is a societal issue and affects not only the woman, but also her family of origin and her children. This study did not support the hypothesis that more frequent visits from friends or family decrease loneliness, but it did support the hypothesis that the presence or absence of friends and the number of friends within the facility make a significant difference in the loneliness experienced by women prisoners. It is within the scope of nursing practice to lessen the burden of loneliness and to improve the physical and emotional health of the person by addressing the issue of loneliness.

Further surgical management of gastric ulcer with unsuspected malignant change.

This paper considers the further surgical management of patients who have had a gastric ulcer removed which was apparently benign but which on subsequent histological examination has proved to be a carcinoma. A retrospective review of 59 such patients has been undertaken. A non-radical partial gastrectomy was performed in the first instance on all these patients; 24 of them had a second operation once the correct diagnosis had been reached and 35 had no further surgery. These two groups are compared. The overall 5-year survival in the non-revisional group was only 23% compared with 56% in those treated by a second operation. It is thought that revisional surgery should be undertaken within 6-8 weeks, provided the age and general condition of the patient are satisfactory. Since 1970 the routine use of fibreoptic endoscopy and biopsy has virtually eliminated the problem.

Partial gastrectomy for haemorrhage.

An analysis was made of the results of 566 partial gastrectomies for haemorrhage from gastroduodenal ulceration between 1953 and 1962 with regard to mortality, morbidity, and long-term follow up. With rigid criteria for selection of patients for surgery, the overall mortality rate for ulcerative gastroduodenal haemorrhage was 8.6%. The actual operative mortality rate more than doubles if an emergency operation is performed later than four days after the patient's admission with haemorrhage. Postoperative and later bleeding complications occurred in 5% of patients. Regardless of the length of ulcerative history, over 92% of patients have clinically satisfactory long-term results. Six per cent required further operation, after which, they too had clinically satisfactory results.

Adolescent pregnancy in the United States: not a minority issue.

Adolescent pregnancy is on the rise in the United States. This fact is often stressed in the media with an emphasis on minority cultures. It is true that the United States ranks seventh in the industrialized world in the incidence of teen pregnancy and the highest incidence is found among black adolescents of low socioeconomic status. However, care must be taken in linking early childbearing with race. The persistence of poverty, joblessness, and lack of hope for the future are recurring themes in the quest for understanding the phenomenon of adolescent pregnancy, and these factors are increasingly being experienced by white as well as black adolescents.
