Carbon dioxide insufflation for more comfortable colonoscopy.

Forty patients examined with the use of carbon dioxide insufflation during fiberoptic colonoscopy showed no significant residual gas on plain radiographs taken 30 minutes after the procedure. By contrast, 19 of 20 patients examined with air insufflation showed excessive distension of large and/or small intestine. The rapid absorption of carbon dioxide may be a safety factor in a few patients and adds to the comfort of the procedure in many others by virtually eliminating overdistension afterwards. It also makes it technically possible to perform a barium enema immediately after colonoscopy.

Immunoglobulin concentrations in pig follicular fluid.

IgG is the predominant immunoglobulin in pig follicular fluid. IgA is present in amounts second to IgG, while IgM is detectable in traces only. The concentration of IgG in the fluid increases as the follicles enlarge to the Graafian preovulatory size, while IgA levels seem not to be affected. Follicular fluid of prepubertal gilts contains slightly higher IgA concentration compared to sow follicular fluid although its IgG content is much less. There is no correlation between the IgG and IgA concentrations in the porcine follicular fluid; this may suggest different rates of filtering of these immunoglobulins into the follicles.

Multiple endoscopic polypectomies for schistosomal polyposis of the colon.

Endoscopic polypectomy alone dramatically reduced bowel frequency and colonic blood-loss in seven patients who had multiple schistosomal polyps which had persisted despite previous medical treatment. The disappearance of hypoproteinaemic leg oedema in five patients was associated with a corresponding rise in serum albumin. Up to 100 polypectomies could be performed during a single endoscopic examination. Multiple endoscopic polypectomy quickly improved the well-being of debilitated patients with schistosomal polyposis.

Immunohistochemical studies of the local immune system in the reproductive tract of the sow.

Tissues from the reproductive tract of sexually mature female pigs were examined histologically using antisera to porcine IgG, IgA and IgM coupled to horseradish peroxidase. Plasma cells synthesising all three immunoglobulins were found in all regions of the tract, with IgA-producing cells as the predominant type. Larger numbers of plasma cells were found in the lower region of the tract and the number of plasma cells was raised during oestrous. Intercellular staining of immunoglobulins was also observed and was also under hormonal influence. The results indicate the presence of a local mucosal immune system in the female reproductive tract of the pig.

Quantitation and origin of immunoglobulins A, G and M in the secretions and fluids of the reproductive tract of the sow.

Immunoglobulin quantitation of fluids obtained from several regions of the female reproductive tract indicated the presence of IgG, IgA and IgM. IgG was almost invariably present in greatest amounts and IgM always made the smallest contribution. Although much of this immunoglobulin was derived from serum, evidence of the molecular size of IgA, the IgA and IgG ratios and that obtained from experiments involving the injection into sows of 125I-labelled immunoglobulin indicated that local synthesis within the tract also occurred.

Quantification and origin of the immunoglobulins in porcine respiratory tract secretions.

The immunoglobulin content of porcine nasal, tracheal and bronchio-alveolar secretions was determined. IgA was the predominant immunoglobulin in nasal and tracheal secretions, there being no significant difference (P greater than 0.01) between their IgA:IgG ratios of 2.7:1 and 2.4:1, respectively. In contrast, IgG was the predominant immunoglobulin in bronchioalveolar secretions, the IgA:IgG ratio being 0.7:1. The ability of tracheal and lung tissue to synthesize IgA and IgG was determined in vitro. Tracheal cultures synthesized more IgA than IgG whereas in the lungs the reverse occurred, the ratio of IgA:IgG synthesis being 1.4:1 and 0.4:1, respectively. The proportion of serum-derived IgA and IgG in respiratory tract secretions was determined by measuring the transfer of 125I- and 131I-labelled immunoglobulins from serum. It appeared that more than 97% of the IgA was produced locally. More significantly a proportion of the IgG was also synthesized locally. This proportion was smallest in nasal secretions (21.8%) and greatest in bronchio-alveolar secretions (62.6%). The results suggest that IgG, a large proportion of which is locally synthesized, is important in the homeostatic mechanisms of the lower respiratory tract.

Studies on the formation of alpha-amylase by Thermomonospora vulgaris.

Conditions affecting the formation of alpha-amylase by static cultures of the thermophilic actinomycete Thermomonospora vulgaris were studied. The organism failed to grow under submerged culture conditions or when the culture medium was devoid of CaCO3-alpha-Amylase was produced during the logarithmic phase of growth and maximum yield was obtained after 3 to 9 days of incubation. Growth and amylase formation took place only in a range from 45 degrees to 55 degrees C; optimum temperature was 55 degrees C. Of the tested carbon sources only starch induced enzyme formation. Maximum enzyme yield was obtained when starch concentration of the medium was 2% and when ammonium citrate served as a nitrogen source. Crushed clay pots could substitute for CaCO3 of the medium, but growth and amylase yield were less.

Characterization of a red pigment-antibiotic produced by Streptomyces sp. strain NRC-C7.

The streptomycete strain NRC-C 7 isolated from some Egyptian soil samples, produces a red-coloured pH indicator antibiotic active against gram positive bacteria. The producing organism belongs to the grey series of the genus Streptomyces and has grey coloured aerial mycelium with violet pigmented substrate mycelium, the violet pigment does not diffuse into the agar medium. Glucose sodium nitrate medium proved the most suitable medium for the production of this antibiotic. The pigment was extracted from the broth and purified. The solubility, Rf values, minimal inhibiting concentrations, optical rotation, UV, and IR spectra were studied. Melting point 180, elemental analysis: carbon 51.71%, hydrogen 6.25%, and oxygen 42.04%.

Amylase of the thermophilic actinomycete Thermomonospora vulgaris.

alpha-Amylase of the thermophilic actinomycete Thermomonospora vulgaris was partially purified. Maximal enzyme activity was obtained at 60degreeC and pH 6.0. KM value was l.4%. The effect of some metal salts on enzyme activity was studied. Enzyme activity was inhibited by by KCN, EDTA, and iodoacetate. Inhibition by EDTA was completely nullified by CaCl2, but the inhibition by iodoacetate was not overcome by 2-mercaptoethanol. Exposure of the enzyme to pH 7.0 and 9.0 for 2 hr. did not affect the enzyme, but exposure to pH 3.0 for few minutes completely inactivated the enzyme. Exposure of the enzyme to 60degreeC resulted in an appreciable inactivation and exposure to 80degreeC completely inactivated the enzyme. Addition of CaCl2, 2-mercaptoethanol, or enzyme substrate the 60degreeC exposed enzyme. However, bovine serym albumin had a protective effect when the enzyme was exposed to 60degreeC but not to 80degreeC. The enzyme was stable in the presence of 8 M urea.

Supra-sternal notch tuberculous abscess: a report of three cases.

Extra-pulmonary tuberculosis remains a diagnostic and therapeutic challenge; its clinical presentation can mimic a wide range of pathological conditions. Here we report on 3 female patients who presented with supra-sternal masses that were suspected clinically to be of thyroid origin. By use of fine-needle aspiration cytology (FNAC), they were proved to be tuberculous lesions involving the pre-tracheal lymph nodes. Serological examination for HIV-I/II was not reactive in the 3 patients. The patients responded well to a regimen of multi-drug therapy. It is concluded that extra-pulmonary tuberculosis should be considered in the differential diagnosis of thyroid or para-thyroid swellings and that FNAC is a simple, quick and reliable procedure in the diagnosis of extra-pulmonary tuberculous lesions involving the neck.
