[Control over the function of the anastomosed segments of the digestive tract].

One of the most effective methods of prophylactics of incompetence of anastomoses sutures is the intraoperative registration of bioelectrical activity of the connected segments of the digestive tract when performing operations on the stomach and esophagus. The implanted electrodes allow analysis of the action potential fluctuation during the postoperative period followed by the corresponding conclusions about the state of blood circulation in hollow organs. The effectiveness and adequacy of management of the patients after the interventions is estimated by means of rheography of the intestine wall. The proposed program of controlling the state of proximal segments of the digestive tract gave good results in 42 patients after total and partial gastrectomies.

[The pathogenetic and clinical grounds for the advantages of nondrug procedures in the preoperative preparation of patients with diffuse toxic goiter].

An analysis of 615 patients operated upon for diffuse toxic goiter is presented. In 148 of them (24%) preoperative preparation was made with the help of nonmedicamental methods (extracorporal hemosorption, hyperbaric oxygenation (HBO), plasmapheresis). The state of euthyroidism was controlled according to their clinical symptoms and was verified by radioimmunological studies of triiodothyronine (T3), thyroxin (T4), thyrotropine (TTG). Indicators of corticosteroid hormones, thyrostimulating antibodies, antigenic HLA structure and antioxidative activity of different media of blood were also determined. The estimation of the data obtained as compared with results of morphological examinations allowed a conclusion that the above nonmedicamental methods in patients with diffuse toxic goiter are pathogenetically substantiated. Clinical expedience of their use is obvious. No lethal outcomes or complications resulted from using hemosorption and HBO. Single complications took place resulting immediately from catheterization of large venous vessels. Long-term results from 3 to 9 years have shown that recurrences of the disease in patients given nonmedicamental preoperative preparation were met much more rarely as compared with patients who obtained routine preparation (1.4% and 5.4 correspondingly).

[End-to-end anastomosis in right-sided hemicolectomy].

The authors made an analysis of the nearest and long-term results of treatment of 139 patients subjected to right-sided hemicolectomy with different variants of ileotransversoanastomoses. Best results were obtained after end-to-end enterocolostomy.

Action Potentials

[The main ways of improving results in treating combined injuries of the major vessels of the extremities].

Based on an analysis of the surgical treatment of 240 patients with traumas of the main extremity vessels (147 associated traumas included) the authors show that favorable outcomes of such injuries are due to such factors as timely diagnosis, successful struggle with the traumatic shock and blood loss, adequate and skillful technique of surgical measures, adequate treatment of concomitant injuries and character of therapy in the postoperative period.

[Diagnosis and treatment of adrenal cysts].

The authors have analyzed literature data of 303 cases of adrenal cysts and describe their two cases (both patients were operated on with good results). Problems of diagnosis and treatment of this rare disease are discussed.

[Acute ischemia of the arms].

The article analyzes results of the treatment of 162 patients with acute ischemia of upper extremities. Acute ischemia of upper extremities was shown to be an absolute indication for operation aimed at the reestablishment of adequate bloodflow. The character of surgery is predetermined mainly by genesis of acute ischemia. Postoperative lethality was 8.8%.

[Surgical treatment of combined occlusions of the aortoiliac and femoropopliteal segments].

Associated lesions of distal segments of arteries were established in 78,2% out of 206 patients on the basis of clinico-rentgenological examinations and intraoperative findings. The correction in the aorto-iliac segment associated with extended plasty of the femoral arteries and sympathectomy improved the blood circulation in 94,3% of observations and the multistory shunting proved to be unnecessary.

[Use of extracorporeal hemosorption for the preoperative preparation of toxic goiter patients].

The method of extracorporal hemosorption was used by the authors first in the clinical practice for the preoperative preparation of patients with thyrotoxicosis. Oxygenated SKN carbons were used as sorbents. The hemosorption was used in 16 patients with thyrotoxic goiter by the method developed in the clinic. The authors make a conclusion that hemosorption is an expedient method for the preoperative preparation of patients with a toxic goiter.

[Pathogenesis of acute peritonitis].

The article presents data on the dynamics of alterations in the ultrastructure of lymph capillaries in the small intestine wall in acute peritonitis (experimental-clinical investigation). They show an important role of the lymphatic bed of the small intestine in the development and outcome of this disease.

[Diagnosis of cholelithiasis in jaundice patients and the reasons for diagnostic errors].

The experience of diagnozing causes of jaundice in 2821 patients is described. In 428 (15,1%) patients the jaundice was of mechanical origin. The frequency and diagnostic value of certain symptoms in obstructive forms of jaundice are discussed. The diagnostic role of biochemical indices of the peripheral blood is also analyzed. Their relative significance is shown.

[Diagnostic and prognostic importance of the intensity of the blood plasma superweak intrinsic luminescence in acute surgical diseases of the abdominal cavity organs].

The authors present the results of the studies of the intensity of plasma super-weak chemical luminescence for the diagnosis of acute appendicitis (35 cases) and various forms of cholecystitis (131 cases). The increase of the intensity level of the blood plasma chemical luminescence has been found in all the forms of cholecystitis and in appendicitis. With the improvement of the patients condition the indices of the luminescence intensity start to normalize. On the grounds of their observations the authors consider that the use of this method in surgical practice will provide information on an early development of complications before their evident clinical manifestations occur.

[Is an appendectomy indicated when a diagnosis of acute appendicitis is not confirmed during the operation?].

In 236 out of 11,150 (1.8%) patients operated upon for acute appendicitis some other acute surgical and gynecological diseases were revealed. In this group of patients the vermiform process either seemed to be unchanged or its changes were considered to be secondary ones (reaction to an inflammation in adjacent organs). Histological study of vermiform processes proved the abscence of inflammatory alterations only in 35 cases. In the rest of patients either chronic or acute inflammatory process was found. The conclusion has been drawn that in case when diagnosis of acute appendicitis is not confirmed during operation, appendectomy does not produce any unfavourable effect upon the postoperative clinical course and outcome of the main disease and should be considered as a compulsory element of a surgical intervention.

[Surgical interventions in complicated forms of acute appendicitis].

Under study were the results of treatment in 10 056 patients with acute appendicitis, including 418 patients having its complicated forms. It is concluded that the extent of surgical intervention in complicated forms should be determined by the character of the complication, manifested in pathological changes of the peritoneal coat and patient's status. Appendectomy may be performed in most patients with appendicular infiltration. It is the authors' opinion that indications to peritoneal dialysis in peritonitis of appendicular origin should be largely restricted. The tactics employed by the authors enabled them to reduce the mortality in complicated forms of appendicitis up to 4.4%.
