A practical approach to laparoscopic cholecystectomy.

A prospective study of 500 consecutive cholecystectomies was initiated with the introduction of laparoscopic cholecystectomy. Laparoscopic cholecystectomy was attempted in 96% of patients presenting with primary gallbladder disease and was completed in 95%. There were no deaths or bile duct injuries. Two patients undergoing laparoscopic cholecystectomy were transfused for postoperative bleeding, and only one patient required reoperation for any reason. A prospective study showed reduced operating time (20 minutes) and patient charges ($546) using electrosurgical dissection compared with laser. Reusable trocars were used without any associated injury or morbidity. An effective strategy for selective cholangiography was developed based on patient history, liver enzymes, and common duct diameter. In conclusion, laparoscopic cholecystectomy appears to be a safe operation. The cost-effectiveness of laparoscopic cholecystectomy can be enhanced ($1,271) with no loss of patient benefit using the combination of electrosurgery, reusable trocars, and selective cholangiograms in low-risk patients.

Large cell carcinoma of the lung with isolated jejunal metastasis.

Isolated small bowel metastasis of bronchogenic carcinoma is so distinctly unusual that only three previous case reports have been located. The authors report the case of a 59-year-old white male, who succumbed due to complications of a jejunal metastasis two months after left upper lobectomy for large cell carcinoma. The diagnosis was suspected preoperatively on the basis of a CT scan.

Recent trends in pulmonary resection.

The charts of 131 consecutive pulmonary resections were reviewed at the Jackson VA Medical Center and analyzed for trends in etiology, pathology and types of resection. Analysis of this group, as well as follow-up, are presented. We feel that this review has delineated some current trends in pulmonary resectional surgery and identified some areas for progress in the future. Only by careful assessment of one's work can one evaluate performance and plan for the future.

Effectiveness of insulin therapy on altered renal calcium transport in diabetic rats.

The uptake of 45Ca was measured in slices of kidney cortex from normal rats, streptozotocin-diabetic rats, and streptozotocin-diabetic rats treated early and late with insulin. Insulin therapy was performed such that blood glucose levels were controlled in half the treated diabetic animals but not in the others. Considerably earlier than evidence of nephropathy (i.e., proteinuria and increased BUN levels) in streptozotocin-diabetic rats, there was a significant decrease in active uptake of calcium by the kidney. Insulin therapy, begun immediately upon diagnosis of diabetes, maintained normal calcium transport even when blood glucose levels were not controlled. On the other hand, insulin therapy, begun 1 mo after diabetes was confirmed but before evidence of nephropathy, did not restore calcium transport to normal whether or not blood glucose was controlled. We conclude that this biochemical mechanism, which possibly may be implicated in the pathophysiology of diabetic nephropathy, is clearly influenced by duration of insulin deficiency and not by the degree in hyperglycemia.
