Pericardial multicystic proliferation.

We report the case of an intrapericardial lesion with the pathological features of a multilocular cyst, found on the adventitia of an aortic aneurysm in a 62-yr-old man. Microscopically, it consisted of numerous cavities lined by flattened or cuboidal cells with the immunocytochemical and electron microscopical features of mesothelium, sometimes forming a hyperplastic pattern. Numerous nerve bundles and collections of inflammatory cells were present in the stroma. These features were consistent with a reactive lesion.

Detection of anti-sense transcripts of the insulin-like growth factor-2 gene in Wilms' tumor.

The expression of the insulin-like growth factor-2 (IGF-2) gene was studied by means of both in situ hybridization and immunohistochemistry in eight cases of Wilms' tumor with different histological features. An anti-sense cRNA IGF-2 probe revealed the presence of abundant IGF-2 mRNAs in all eight tumors examined, localized in the most undifferentiated tumoral cells (blastemal cells); none of the tumors showed immunoreactivity for the anti-IGF-2 antibody. Using a sense cRNA IGF-2 probe, we also detected anti-sense transcripts of the IGF-2 gene in five of eight tumors. These transcripts were exclusively localized in the cells expressing the IGF-2 mRNAs. Although the function of these anti-sense transcripts is unknown, we think that their presence could explain the lack of IGF-2 peptides in Wilms' tumors despite increased expression of IGF-2 mRNAs.

Laminin receptors, collagenase IV and prognosis in node-negative breast cancers.

In 187 node-negative breast cancers, the expression of laminin receptors and collagenase IV was directly related in 52% of cases, independently of pathological (tumor size and histology) and biological (estrogen receptors and proliferative activity) features. Moreover, the presence of laminin receptors and collagenase IV did not appear to influence tumor proliferative activity, evaluated as 3H-thymidine labelling index. In this case series, relapse-free survival and overall survival at 6 years were significantly affected by tumor size, hormone receptor status and proliferative activity. Conversely, high levels of laminin receptors and collagenase IV failed to influence relapse-free or overall survival, whereas they were strong indicators of local-regional diffusion of the disease.

The morphogenesis of human hepatocellular carcinoma.

In both cirrhotic and non cirrhotic livers, hepatic carcinogenesis appears as a multistep process commonly starting from hyperplastic nodules and reaching HCC via a continuous spectrum of lesions. Minute HCC without a background of hyperplastic lesions have also been identified in cirrhotic livers. These observations suggest that the morphological progression of carcinogenesis in the human liver can develop through two different main pathways.

Evaluation of hepatocellular carcinoma aggressiveness by a panel of extracellular matrix antigens.

Invasion and metastasis requires a series of interactions between malignant cells and the extracellular matrix (ECM). Antigen markers that relate to these interactions were evaluated for prognostic correlation in human hepatocellular carcinoma. Basement membrane type IV collagen (cIV), type IV collagenase (cIVase), laminin, and laminin receptors (LRs)--all ECM antigens previously proposed to be modulated in association with tumor aggressiveness--were immunohistochemically investigated in 30 cases of hepatocellular carcinomas (HCCs). The pattern of antigen expression was correlated with 1) 36 months' clinical follow-up and 2) the pathologic grade. As a means of estimating the proliferation fraction, an additional antigen, Ki67, was also studied in this series. There were major differences in the distribution of cIV and laminin, and in the quantity of cIVase-, LR-, and Ki67-positive cells associated with grade and prognosis. A smaller quantity of cIV and laminin and a higher number of cIVase-, LR-, and Ki67-positive cells were detected in the poorly differentiated compared with the well-differentiated HCCs. The tumors with lower immunoreactivity for cIV and laminin components accompanied by a higher number of cIVase-, LR-, and Ki67-positive cells fall into a group with the poorest overall survival (P less than 0.006). The panel of antigens is proposed as a useful prognostic tool for evaluating HCC tumor aggressiveness.

The capsule surrounding primary liver tumors: wherefrom its prognostic significance?

Primary liver neoplasms in cirrhotic and non-cirrhotic patients were studied by electron microscopy and immunohistochemical methods for extracellular matrix (ECM) antigens. A capsule of variable thickness was present in many expanding hepatocellular carcinomas, while it was absent in those of small size, and either fragmented or absent in the infiltrating ones. In the capsules of early onset, fibronectin was the most frequent stromal glycoprotein. In the completely formed capsular structures, fibronectin, type-V collagen and laminin were the most common macromolecules seen. No differences were evident in the pattern of ECM macromolecules in the capsules surrounding hepatocellular carcinomas compared with those found in benign lesions, such as hepatocellular adenomas or focal nodular hyperplasia. The possibility is discussed that the capsule could be a result of the following sequence: tissue with expansive (not infiltrative) growth-mechanical compression-ischemic necrosis of surrounding tissues-repair process with ECM deposition.

Incidence of intratubular germ cell neoplasia in androgen insensitivity syndrome.

Gonadal histology was investigated by means of conventional microscopy in 6 patients with complete androgen insensitivity syndrome, in 11 with incomplete androgen insensitivity syndrome, and in 3 with 5 alpha-reductase syndrome. Twelve subjects were prepubertal and 8 pubertal. In all patients gonadal tissue was removed as a prophylactic measure and no patients gave rise to any clinical suspicion of a tumour. Eight patients with incomplete androgen insensitivity syndrome, 5 of whom (62.5%) were prepubertal, showed intratubular germ cell neoplasia and in 6 of them it was bilateral. Histochemical and immunohistochemical analysis showed considerable agreement between atypical morphological aspects and positive response to Schiff's periodic acid and to staining with the anti-placenta alkaline phosphatase antibody. Our patients were characterized by one of the highest reported incidences of intratubular germ cell neoplasia, particularly at prepubertal age. These findings would seem to indicate that a rethink is needed concerning the general opinion that patients with androgen intensivity syndrome have practically no risk of developing malignancy, and that orchidectomy is not advisable before puberty is completed.

Primary liver neoplasms: evaluation of proliferative index using MoAb Ki67.

Thirty-two cases of primary liver neoplasms comprising 12 benign, 15 malignant, and 5 cases with equivocal histopathological features between benign and malignant have been investigated using monoclonal antibody (MoAb) Ki67, which reacts with a nuclear protein expressed in the G1, G2, S, and M phases of the cell cycle. The Ki67 score (positive cells/total neoplastic cells) seems to correlate to the classes of lesions tested. In the hepatocellular carcinoma (HCC) group, the percentage of labelled nuclei, ranging from 15 to 50 per cent, showed a good correlation with Edmondson-Steiner's histological tumour grade. A percentage of positive cells similar to that of the proved low-grade HCCs was detected in the five neoplastic lesions in which the routine histopathological criteria of malignancy were not fulfilled. The benign neoplasms showed a very low growth fraction, similar to that of normal or cirrhotic tissues. The use of the Ki67 score seems to offer useful information about the biological behaviour of some liver masses and may help in the differential diagnosis of hepatocellular adenoma versus carcinoma.

Changes in the alveolar connective tissue of the ageing lung. An immunohistochemical study.

The modifications of the extracellular matrix (ExM) components in the alveolar parenchyma of elderly subjects were investigated using a panel of polyclonal antibodies. The elastic fibers showed a notable decrease along the alveolar walls while type III collagen increased when compared with that of non-elderly controls. No variations of these components were detectable in the alveolar ducts or in the respiratory bronchioli. An increase in the thickness of the alveolar basement membranes was detected in some of the subjects when antibodies against type IV collagen and laminin were used, while antibodies to fibronectin and type V collagen did not reveal any modifications compared with the controls. The modifications revealed in the lungs of the elderly can be related to the alterations of the elastic recoil and pulmonary compliance observed in these subjects.

Small liver masses in cirrhotic patients. A pathological clue for the morphogenesis of human hepatocellular carcinoma.

Eleven small primary liver lesions detected in four cirrhotic patients are reported in detail. Some lesions showed the histopathological features of macroregenerative nodules (MRN) containing foci of cellular and structural atypia or of hepatocellular carcinoma (HCC). Other lesions showed the histological abnormalities of the early developmental stage of HCC without a background of MRN. Finally one lesion is described with an equivocal background between benign and malignant, containing a focus of carcinoma. All lesions presented suggest the presence of different pathways in the morphogenesis of human HCC.

Different metastatic aggressiveness by murine TS/A clones: ultrastructure, extracellular glycoproteins and type IV collagenolytic activity.

Cell ultrastructure, extracellular matrix glycoproteins, and type IV collagenolytic activity have been examined in four murine TS/A clones characterized by different metastatic aggressiveness. In vitro, highly metastatic clones (E) exhibited slightly less differentiated ultrastructure, and high type IV collagenolytic activity, while low metastatic clones (F) showed a more differentiated cytotype, with either high and low collagenolytic activity. Type IV collagen, laminin, and fibronectin were expressed without correlation with the metastatic efficiency; keratin was slightly more evident in E than in F cells. The morphological differences between E and F clones were less evident in the tumors produced by subcutaneous injection, and markedly reduced in the relative metastases composed only of the less differentiated cytotype; differences were also reduced in cells cultured on extracellular-matrix-coated flasks. The present study shows that no general correlation is observed between the studied parameters and metastatic aggressiveness.

Extracardiac rhabdomyoma: An immunocytochemical study and review of the literature.

Two cases of 'adult' rhabdomyoma and 2 of 'fetal' rhabdomyoma have been immunologically studied using several antisera specific of skeletal muscle fibers proteins. This paper also deals with a review of the literature on these rare benign skeletal muscle proliferations. It is concluded that 'adult' rhabdomyoma contains fetal myosin and therefore the term 'adult' does not seem immunologically appropriate any longer. In addition the tumoral cells show the same level of differentiation as seen in neonatal skeletal muscle. Therefore it seems that the definition of neonatal rhabdomyoma is more appropriate for these benign muscular proliferations. The 2 cases of fetal rhabdomyoma appear phenotypically similar to normally developing myoblasts and contain fetal myosin. Therefore it seems that the definition 'fetal' is here appropriate for what appears to be a disorganized proliferation of otherwise normal fetal skeletal muscle fibers. The differential diagnosis between these latter tumors and rhabdomyosarcomas has been discussed.

Hepatocellular carcinoma: expression of basement membrane glycoproteins. An immunohistochemical approach.

The identification and characterization of collagenous and non-collagenous glycoproteins have made it possible to use specific antibodies as tools for the topographical localization of the various connective tissue components, and thus to follow the progression of parenchymal-stromal interactions. This investigation is an approach to the study of in vivo relationships between basement membrane components (type IV collagen, laminin) and neoplastic cells of hepatocellular carcinoma. Ten cases of hepatic carcinomas were analysed and paraffin-embedded sections were immunostained with anti-laminin and anti-type IV collagen antibodies. The avidin-biotin-peroxidase complex technique was used. In well differentiated neoplasms with hepatic tumour cells organized in a trabecular pattern lined by sinusoid structures, type IV collagen was always detected at the sinusoidal level. Laminin was evident only in two cases with a prominent intraparenchymal vascular bed. In less differentiated neoplasms, sinusoids were almost absent and only large tumour vessels were positive for both laminin and type IV collagen. At the interface between tumour tissue and the surrounding stroma, some carcinomatous elements were surrounded by laminin and type IV collagen. Our data further support the hypothesis that basement membrane phenotypic expression may be influenced both by the degree of tumour differentiation and by the characteristics of the micro-environment.

Primary liver cell carcinoma. New insight for a more correct approach to its classification.

Hepatocellular carcinomas, cholangiocellular carcinomas, and "indeterminate" forms have been studied by immunohistochemical detection of several antigens: alpha-fetoprotein and CEA, as well as type IV collagen, laminin, and keratin. While the first two antigens were variably detectable in different samples, in the poorly differentiated areas of hepatocellular carcinoma type IV collagen and laminin were both absent. These basement membrane glycoproteins constantly surrounded the cholangiocellular carcinoma tubular structures and were always present in those areas of the "indeterminate" group where the cells were organized as pseudoglandular or ductular aggregates. Keratin was absent in all cases of hepatocellular carcinoma, was variably detectable in the "indeterminate" neoplasms and was always present in cholangiocellular tumors. The neoplasias with mixed histopathological features usually showed morphological and biochemical biliary differentiation at the periphery, in contact with the collagenous stroma. We postulate that in some cases hepatocarcinoma cells undergo metaplastic transformation giving rise to cholangiocellular forms. Type IV collagen, laminin, and keratin seem to be accurate markers of phenotypical differentiation and show that a continuous phenotypical spectrum exists between hepatocellular and cholangiocellular entities.

Monoclonal antibody to HSV2 protein as an immunodiagnostic marker in cervical cancer.

The present study was designed to evaluate the possible use of monoclonal antibodies (mAbs) as diagnostic adjuncts to exfoliative cytology and tissue sections in intraepithelial (CIN) and invasive cervical cancer. Specimens were collected from 42 patients with various degrees of CIN, 15 patients with invasive cancer and two patients with condylomatous changes only. mAb H17, that recognizes a herpes simplex virus protein (ICP) representing a component of the viral ribonucleotide reductase, stained atypical exfoliated cells from 55% of patients with mild dysplasia and 100% of those with more severe lesions. The mean percentage of positive atypical cells increased as a function of the grading of CIN (32.6 +/- 6.3%, 63.5 +/- 2.7%, 67.9 +/- 8.1%, 81.4 +/- 10.1%, and 85.6 +/- 2.0% for mild, moderate, and marked dysplasia, CIS, and invasive cancer, respectively). Only a very small proportion of atypical cells from only two patients stained with a mAb to another herpes simplex virus protein (gA/B). Normal squamous, metaplastic, inflammatory, or koilocytotic cells did not stain with the mAbs. Of the 15 cases examined by cryostatic fresh sections with immunohistochemical techniques, only one case of invasive cancer did not stain with mAb anti-ICP, and all controls were negative. The high specificity and sensitivity of MAbH17 suggests that it may be a useful diagnostic/prognostic marker in CIN.

Immunohistochemical study of basement membrane antigens in bronchioloalveolar carcinoma.

Bronchioloalveolar carcinoma (BAC), not yet completely defined as a biologic entity, has recently been classified into two different types. Immunohistochemical investigations, aimed at characterizing basement membrane (BM) behavior in the two types of BAC, revealed different distribution patterns. The first (Type I BAC) showed a linear staining for laminin and Type IV collagen similar to normal lung. Fibronectin was widely present in the septal interstitium and patchily distributed along the BM. The second (Type II BAC) showed a variable reaction for Type IV collagen and fibronectin, whereas laminin was absent or appeared as short, interrupted tracts around the epithelial neoplastic population, similar to conventional adenocarcinoma of the lung. These results suggest that only Type I BAC shows structural characteristics different from those of conventional adenocarcinoma of the lung.

Effect of bromocriptine on plasma catecholamines in normal subjects and prolactin-secreting tumor patients.

The effect of short term bromocriptine (5 mg/day, 5 days) on plasma epinephrine (E), norepinephrine (NE), dopamine (DA) and prolactin (PRL) was studied in 4 normal women and 6 bearing PRL-secreting tumors. When studied on placebo no significant differences were found between controls and patients in E, NE and DA plasma levels. Bromocriptine induced: a 70% decrease in PRL levels in both groups, a significant (p less than 0.05) decrease in plasma NE levels in the control group, no significant change in plasma NE levels in the hyperprolactinemic patients when considered as a group. These results do not indicate that bromocriptine is a useful tool in the diagnosis of defective central dopaminergic regulation since individual responses of the PRL-secreting tumor patients were variable. Nevertheless, the impaired response of the group as a whole may be suggesting an underlying alteration of DA2 receptor activity in these tumor patients.
