Priapism--conservative treatment versus surgical procedures.

Fourteen patients with priapism have been treated over the last 12 years. Two patients had a recurrence 2 and 5 years after the primary incident. Abuse of alcohol (often combined with psychotropic drugs) and the use of antihypertensive drugs were the most prominent aetiological factors. Three out of 9 patients treated conservatively and 5 out of 6 patients treated with a shunt procedure have preserved their erection ability. Our results indicate that conservative treatment is not justified and that a shunt procedure should be performed with the least possible delay.

Antegrade spermatic phlebography in the diagnosis of metastases from testicular tumors.

Antegrade spermatic phlebography was performed in 18 patients in connection with orchiectomy because of testicular expanding lesions and was compared with other diagnostic methods. Fifteen of the cases were teratomas or seminomas. In 6 patients metastases were demonstrated at phlebography and later confirmed. In one case no abnormality was found at phlebography, nor at CT, but at lymphography metastases were found in not enlarged lymph nodes. Due to technical error at phlebography one case was falsely considered abnormal. The technique is described and its use as a screening procedure for metastases is advocated.

Diagnostic and prognostic value of pedal lymphangiography in testicular teratomas.

Since 1960 pedal lymphangiography was followed by retroperitoneal lymphadenectomy in 60 patients with malignant teratoma of the testis. The accuracy of lymphangiography compared to the histological examination of the retroperitoneal lymph nodes was 88%. The prognostic value of lymphangiography seems to be of importance. Cavography and urography have given little additional information while modern techniques, such as computed tomography and perhaps ultrasound, probably will be of great help in the staging of patients with testicular tumours.

Impotence, smoking, and beta-blocking drugs.

Four patients complaining of erectile dysfunction and using tobacco or propranolol were examined with penile blood pressure measurements and Doppler recordings, calculating the penile acceleration ratio (PAR). After a change in regimen, erectile capacity was restored, PAR returned to normal, and blood pressure measurements revealed increased systolic penile blood pressure in three patients. The changes, especially in PAR, indicate a marked penile vascular reaction during smoking and beta-blocking treatment in these men.

Palliative embolization of arterial renal tumour supply. Results in 10 cases.

Palliative occlusion of the arterial renal tumour supply was performed in 10 patients and the follow up is reported. Nine of the patients had no subsequent nephrectomy. Spongostan (99% gelatin) was used as the embolic material in 4 patients with the addition of steel coils in 2. Bucrylate was used in 6 cases. Six patients are alive with survival rates presently ranging from 3 to 24 months after embolization. Improvement of the survival time cannot be estimated but local symptoms such as hematuria and pain may be treated in those patients with renal tumours who are not considered for surgery.

Carcinoembryonic antigen in urine in patients with urothelial carcinoma. An expression for the extent of inflammatory reaction of the urinary tract.

The concentration of carcinoembryonic antigen (CEA) in urine and serum was determined repeatedly during one year in 213 patients followed because of previously treated urothelial carcinoma of the bladder. The findings were correlated to grade and stage of previously treated tumour, given therapy, recurrence and the cytological evaluation of a midstream urine specimen. During the period of follow up 43 recurrences were clinically observed. With the exception for the content of inflammatory cells no correlation was found between the CEA levels in urine or blood and the parameters studied. Thus CEA in urine and/or serum cannot substitute for cystourethroscopy, urography and exfoliative cytology in the follow-up of patients previously treated for urothelial carcinoma.

Effects of terbutaline and isoproterenol on hyperkalemia in nephrectomized rabbits.

Terbutaline, an adrenergic beta2-receptor-stimulating substance, resembled isoproterenol in its hypokalemic effect on rabbits made hyperkalemic by nephrectomy three days prior to the experiments. Both agents lowered the serum potassium values. The duration of the terbutaline effect was longer and had less influence on the cardiovascular system than had isoproterenol.

Obstructive nephropathy with infection. A diagnostic pitfall.

Four patients with indistinct abdominal symptoms associated with fever are presented. In all cases the diagnosis proved to be obstructive nephropathy with infection. The proper treatment of the patients was delayed, essentially because of a non-investigative diagnostic attitude. The necessity of making a correct diagnosis in the early course of an acute abdominal disease with fever is emphasised and some investigations that may be of great value for reaching a precise diagnosis are indicated.

Experimental normocalcaemic hyperparathyroidism induced by renal venous congestion.

Ligation of the inferior vena cava above the renal veins in male Sprague-Dawley rats produces a predominantly transient renal injury with vascular congestion, tubular degeneration and necrosis with calcification. This is accompanied by impaired renal function with temporary azotaemia, hyperkalaemia, phosphate retention but with a normal calcium level. The parathyroid glands on the second postoperative day show cellular hypertrophy and on the third and fourth days signs of rapid cell proliferation with numerous mitoses. Thereafter the glands remain enlarged.

Advanced cancer of the prostate combined with hypercalcaemia.

Despite the high frequency of skeletal metastases from cancer of the prostate, hypercalcaemia is extremely uncommon in this condition. In two patients with advanced, poorly differentiated metastasizing cancer a fairly uniform clinical picture developed, with anaemia, leukocytosis, increased serum creatinine, thrombocytopenia, elevated alkaline and acid phosphatase levels and symptoms secondary to hypercalcaemia. The development of more effective agents against cancer of the prostate will probably afford longer palliation, but evidently at a risk of severe metabolic disturbances in the preterminal state.

Ambulatory inguinal hernia repair compared with short-stay surgery.

Two groups of patients operated on for inguinal hernia, one outpatient group and one inpatient group, are compared with respect to subjective distress and immediate postoperative complications. The groups were chosen at random and matched for sex and age. A large number of those who received treatment as outpatients suffered marked distress during the first postoperative days. Some form of intermediary or light nursing should be tried out for the outpatients so that if necessary they can stay the night after operation at the hospital. The number of postoperative complications was equal in the two groups. With suitable patient selection and with a small number of reserve places in a light-care ward, the majority of inguinal hernia operations can be performed on outpatients, resulting in a considerable economic saving and shorter waiting time.
