[Changes in the cardio-thoracic index during the treatment with ibopamine].

In order to confirm the efficacy of ibopamine for the management of congestive heart failure, as shown by the disappearance of clinical symptoms, the authors have assessed the cardiothoracic index before and after drug administration. In the 17 female patients examined the index was markedly improved, i.e. from an average of 0.561 +/- 0.058 to an average of 0.538 +/- 0.052, (p less than 0.01). This finding, together with the disappearance of dyspnea and edema, confirms that there was a change in disease pattern and that treatment with this drug was effective.

[Behavior of ionized calcium in hypertensive and normotensive patients].

Serum concentrations of ionized calcium have been measured in a group of 38 patients with essential hypertension (26 women and 12 men with mean age 61 +/- 12 years) and in a group of 19 normotensives (14 women and 5 men with mean age 70 +/- 12 years). Serum Ca++ levels were significantly lower in hypertensives than in normotensives (4.16 +/- 0.43 mg/dl and 4.46 +/- 0.38 mg/dl respectively, with p less than 0.05). The lower serum concentrations found in hypertensives may be due ions in arterial smooth muscle cells may cause an increase of vascular tone and peripheral resistances. Our results support, in agreement with other Authors, the possible role of ionized calcium in the pathogenesis of essential hypertension.

[The effect of beta blockers on the QT interval: the possible role of mechanisms other than beta block].

The authors evaluated QT interval changes after 7 and 14 days of treatment with 3 different betablockers--acebutolol, atenolol and penbutolol--in 3 groups of hypertensive patients. Acebutolol (400 mg u.i.d.) prolonged QTc interval in a statistically significant fashion, atenolol (100 mg u.i.d.) induced a significant and constant reduction, whereas QTc interval was shortened by penbutolol only after the first week, shifting toward pretreatment values during the second week. The physiological correlation between QT and RR was slightly modified by acebutolol and penbutolol, but markedly reduced after atenolol. Such differences might be of importance in clinical practice because are likely to be linked to different mechanisms intrinsic to each agent.

Electrophysiologic effects of carbochromene in man.

Sinoatrial and atrioventricular node functions were evaluated by the electrode catheter method in 13 patients with coronary artery disease before and 10 min after the i.v. infusion of 80 mg carbochromene. The drug had no effects on sinus node automatism, lengthened the atrial functional refractory period, and restored a 1 : 1 AV conduction at the same atrial rates at which a Wenckebach phenomenon occurred. These effects on atrial functional refractory period and AV conduction may be useful in the treatment of patients with coronary artery disease who present atrial or junctional arrhythmias.
