[Reactions of cerebral vessels to hyperbaric oxygenation in the normal state and in patients with cerebral ischemia].

During initial sessions of hyperbaric oxygenation (HBO), the reduction of the pulse blood content in health (with no cerebral injury) is not pronounced and does not reach the significant level whereas in the course of final sessions it does not almost manifest itself. In contrast to that, in the main group of patients (with sequels of ischemic insult), the above reaction manifests itself more clearly, being statistically significant both in the course of initial and final HBO sessions, which demonstrates greater vasoconstrictor preparedness. That reaction is especially remarkable in the hemisphere of predominant injury (HPI) whereas in the relatively preserved hemisphere, it is slightly weaker and occupies as if an intermediate place between the HPI and normal.

[Changes in respiration and hemodynamics in patients with cranio-cerebral injury after hyperbaric oxygenation].

Respiration, central hemodynamics, gas and acid-alkaline balance of the blood were examined in the course of hyperbaric oxygenation (HBO) sessions in 30 patients in the acute period of severe craniocerebral injury, including 15 patients with a favourable outcome and 15 patients with a subsequent lethal outcome. In the first group, HBO gave rise to a decrease in the minute respiratory volume (MRV) and, in particular, in the minute blood volume (MBV). Acid-base and gas balance of the blood remained unchanged or improved. This attests to intactness of oxygen reactivity of the brain and body on the whole and allows establishment of a favourable prognosis of the further course and outcome of disease. The second group manifested a different dynamics: the initial MRV was greater, further increasing in the course of HBO sessions, acid-alkaline and gas balance of the blood worsened towards the development of non-compensated respiratory alkalosis which reflects ineffective hyperventilation and disturbed perversed oxygen reactivity.

[Therapeutic use of hyperbaric oxygenation in lesions of the spinal cord].

Hyperbaric oxygenation (HBO) in a pressure chamber with an oxygen pressure of up to 2.8 absolute atmospheres administered to patients with various lesions of the spinal cord (trauma sequelae, discogenic ischemic myelopathy, states after tumor removal, etc.,) was frequently followed by a more or less pronounced regression of the existent neurological symptomatology. Pelvic and motor functions as well as sensitivity and reflexes returned to normal and the pain syndromes were alleviated. The therapeutic response to HBO is directly dependent on the role played by reversible functional lesions of the spinal cord in the general picture of the disease.

[Therapeutic use of hyperbaric oxygenation in ischemic strokes].

Hyperbaric oxygenation is a valuable adjunct to the complex of measures devised to treat patients with ischemic cerebral strokes, including those who have undergone surgical operations on the extracranial portion of the cerebral major vessels. Although the best therapeutic effect of hyperbaric oxygenation is observed in the acute period of a stroke, its employment may be useful in many patients in the residual period of the stroke as well. The therapeutic action of hyperbaric oxygenation may be due to not only a certain recovery of the function of the damaged neural centres but also to the mobilization of various mechanisms of the CNS compensatory recovery.

[Experience with the use of hyperbaric oxygenation in neurosurgical clinical practice].

Hyperbaric oxygenation proved an effective means of hypoxia control in the central nervous system of neurosurgical patients, which manifested itself mostly in a regression of the general cerebral and focal neurological symptoms. The most striking effect of hyperbaric ocygenation was received in cases of complications following cerebrovascular procedures. At the same time signs of normalization of the bioelectrical activity of the brain and of an elevated tonicity of the intracranial vessels were observed. The electrophysiological control of the functional state of the body helps to choose an optimum regimen and to avoid the possible side effects of oxygen toxicity under elevated pressure.

[So-called regional cerebral hypertension].

The theory of regional cerebral hypertension is based on methodological imperfect and indirect studies of blood pressure in superficial temporal arteries. The use of a modified direct oscillographic study of these arteries, as well as of a direct (bloody) study of the figures of pressure in them during catheterization refutes the concept of a significant lower level of this pressure compared to the pressure in magistral arteries in normal conditions. These data served as a basis for the premises of the theory of regional cerebral hypertension.

[Prognostic value of the reactions of resuscitated neurological patients during hyperbaric oxygenation].

A complex of various physiological reactions encountered in critically ill patients with brain vascular disturbances and brain traumas in the course of hyperbaric oxygenation (NBO) have been studied. Different nature of reactions has been established in patients who survived and in those with lethal outcome. Two main types of reactions have been identified: prognostically favourable and prognostically unfavourable. The difference is associated with the oxygen reactivity of the affected brain and body. The reactivity was either retained or damaged, which made NBO effective in some cases and ineffective in others.

Brain Injuries↗

[Hyperbaric oxygenation in neurotraumatology].

The use of hyperbaric oxygenation (HBO) as a therapeutic measure has a favorable effect, in the postoperative period also, on traumatic affections of different levels of the nervous system (the brain, spinal cord, peripheral nerve trunks and plexuses. It allows the severity of the neurologic deficit and the degree of the patient's disability to be reduced and in some cases it prevents the growth of vital disorders. At the same time, the effect may exert a certain load on the adaptation mechanisms which are very limited in many neurosurgical patients. In view of this, the tolerance of the patient's organism to this load must be taken into account in choosing the HBO schedules. The tolerance is decisively determined by the intactness of the integral function of the central nervous system and diminishes markedly as the impairment of the consciousness of patients with primary affection of the brain gradually becomes deeper and longer in duration.

Brain Injuries↗

[Hyperbaric oxygenation in the complex treatment of patients with aneurysms of the cerebral vessels].

Experience in the use of hyperbaric oxygenation (HBO) in 56 neurosurgical patients with cerebral vascular aneurysms, mainly in the postoperative period, testifies to the expediency of including this method in the complex of therapeutic measures for these patients. The deep oxygenation of the body tissues, the brain included, produced by HBO makes it possible to prevent in many cases the development of severe and stable neurological defects in the postoperative period and contributes to the normalizaion of vitally important functions. Th therapeutic efficacy of the method is more marked when it is applied sufficiently early (before poorly reparable secondary disorders occur in the organism).

[Main types of electroencephalographic reactions in neurologic patients under intensive care and hyperbaric oxygenation therapy].

Bioelectrical activity of the brain was monitored by EEG during exposure to hyperbaric oxygenation (5 check-ups per session), which was a component of intensive care of neurological patients. The data obtained in the course of 395 sessions in 182 patients helped distinguish four types of reactions of such patients to hyperbaric oxygenation. Analysis of these data and the results of clinical examinations discloses the functional significance of these types and helps more objectively define the indications for further application of this treatment modality.
