Risk factors dynamics in multifactorial prevention of ischaemic heart disease.

The effectiveness of preventive measures in populations subjected to a different degree of preventive activity was according to the dynamics of risk factors (RF) tested in the course of four years. Among 6619 examined men aged 40-59 years, a high incidence of RF (79.2%), their connection with ischaemic heart disease (IHD), and frequent simultaneous incidence of several RF even in subjects without IHD were found. The four-year preventive intervention programme was efficient as regards a decline in the incidence of arterial hypertension (AH) from 24.8% to 19.8%, and smoking from 40.1% to 30.6%, simultaneously with a decrease in the mean level of these RF; these favourable changes did not occur in the control group. Data are analysed concerning the percentage of normalization and the incidence of an elevated BP level, cholesterol and body weight. The study proved the possibility of preventively influencing the incidence of AH in the population, and showed the importance of applying preventive measures in subjects with "borderline" values of RF, such as AH and overweight.

[Approaches to the secondary prevention of arterial hypertension among the population (data from a cooperative study on the multifactorial prevention of ischemic heart disease)].

A cooperative study has reviewed the prevalence of arterial hypertension (AH) among 40- to 59-year-old male residents of three cities (Moscow, Minsk and Kaunas) and the possibility of organizing a long-term outpatient treatment of newly-detected AH cases as well as the efficiency of the latter. Of 16,703 individuals screened, AH was detected in 27.9%. Evidence is obtained that preventive and therapeutic procedures for hypertensive patients are in need of an improvement. In a middle-aged population, moderate AH is prevalent (nearly 67.7%). Coronary risk factors, such as hyperlipoproteinemia and excessive body weight, were more frequent in AH patients, as compared to the general population, an argument in favor of the need for an improved combined approach to AM control in the population. Active combined treatment of the recognized cases over 5 years proved effective as it brought arterial blood pressure back to normal. Patient participation in the treatment and its contribution to treatment efficiency are reviewed.

[Chest pains detected on the basis of the WHO standard questionnaire and their prognostic significance].

A WHO standard questionnaire for the detection of angina pectoris was used in examination of 6914 males aged 40 to 59. The character of the pain syndrome was correlated with risk factor (RF) levels, the prevalence of ECG-signs of ischemic changes, the incidence of cases of myocardial infarction (MI) during a 5-year follow-up period. Examinees with angina pectoris had the highest levels of systolic and diastolic AP, cholesterol, body mass, the highest frequency of lethal and nonlethal MI. Examinees with chest pains resulting from physical exercise but atypical of angina had higher levels of the main RF and ECG-signs of ischemic changes and higher rates (2-fold) of new cases of MI and its mortality as compared to persons without chest pains or with pains unrelated with physical exercise during a 5-year prospective study. This evidence suggests a necessity of a more detailed investigation of the latter group to reach the entire group of CHD patients.

[Preventive examination of the population of the medical district of an urban polyclinic].

Prophylactic examination of the population in a district of a city out-patients clinic was performed. Altogether 689 persons (298 men and 391 women) aged 16 to 64 were examined. A great prevalence of chronic non-communicable diseases (CND) and common risk factors were revealed: CHD in 16.7% of the examinees, cerebral changes in 0.6%, diabetes mellitus in 3.9%, chronic bronchitis in 9.3%, arterial hypertension (AH) in 29.6%, smoking in 26.1%, disturbed carbohydrate tolerance in 15.7%, hypercholesterolemia in 13.5% and excess body mass in 35.5%. In 40% of the patients with AH it was combined with other chronic diseases necessitating a multidisciplinary approach to CND control in the patients with AH. The study showed that the population was not ready enough for prophylactic examination.

[Differential diagnosis of arterial hypertension at a city polyclinic with mass population screening].

The paper discusses the problems of differential diagnosis between varying types of arterial hypertension detected during mass screening of the population. Randomized examination was made in 45% of male population aged 40-59 years. Arterial hypertension was observed in 26.3% of the examinee; 24.5% showed a rise in the arterial pressure for the first time. The methods of investigation available at the city polyclinic allowed the authors to diagnose hypertension in 85.2% of the patients. The results of isotope renography included into the complex of the study enabled them to determine the indications for the method to be used in mass screening of the population for revealing arterial hypertension.

[Results of the active detection and 2-year treatment of arterial hypertension at a polyclinic].

Patients with arterial hypertension (AH) in one of out-patient clinics (group I) were actively treated and supervised during two years according to a special programme, the elements of which had been adjusted to the conditions and possibilities of an out-patient clinic. Patients with arterial hypertension in another clinic (group II) were treated in a conventional way by district physicians. This work proves the possibility of taking a complex of active measures against AH in a municipal out-patient clinic, which allows one to improve significantly the aid to patients with AH and to increase the number of patients treated and receiving an adequate treatment.

The incidence of arterial hypertension in the male population aged 40-59 years in Moscow and Kaunas.

The authors examined 1480 Moscow and 1568 Kaunas male residents aged between 40 and 59 years. The incidence of arterial hypertension (AH) in Moscow and Kaunas amounted to 27.9 and 21.8% respectively. Of the total number of subjects examined in the two cities 24.5% (Moscow) and 59.4% (Kaunas) were unaware of their condition. Only 12.8% of the Moscow residents and 3.2% of the Kaunas residents were treated. The results are analyzed with respect to earlier therapy, its efficacy and standardization of measurement of arterial pressure in the course of cooperative studies. The high incidence of AH, ignorance of the patients of their condition, the low percentage of patients with an established diagnosis of AH emphasize the need for devising a system of identifying hypertensive patients and for treating them appropriately.

[Experience in conducting a multifactor ischemic heart disease prevention program (data from a cooperative study in Moscow and Kaunas].

A program of multi-factor prevention of ischemic heart disease (IHD) conducted in Moscow and Kaunas is briefly described. Preliminary data on the prevalence of IHD and its risk factors in the two towns are shown and data concerning arterial hypertension are analysed in detail. The preliminary results of measures of primary IHD prevention obtained in the first year of observation are reported. Regular treatment during the first year assisted in increasing the number of patients who were under medical care to 78% in Moscow and to 43% in Kaunas; the number of effectively treated patients increased to 48% in Moscow and to 56% in Kaunas.

[Detection and treatment of arterial hypertension at a district polyclinic].

Arterial hypertension was revealed in 65 of 210 (30.9%) males 40 to 59 years of age living on the territory of one doctors' locality of a district out-patient clinic. Among patients with increased arterial pressure 32.3% received hypotensive treatment which proved effective in 10.7%. The results of six-month treatment of 30 patients are analyzed. The most significant decrease in diastolic arterial pressure was encountered in the group of persons who were given regular treatment.
