Optic disc surface smoothness and visual field indices.

BACKGROUND: In order to ascertain whether the optic disc surface smoothness (ODSS) could be used to predict the amount of visual field damage in glaucomatous eyes, the correlations between ODSS and visual field indices were evaluated. METHODS: One eye was randomly chosen from each of 40 normals and 50 patients with glaucoma. The morphology of the optic disc surface was analyzed using the Topcon IMAGEnet X Rev-3.51b. In order to define the level of "smoothness" of the optic disc surface, the differences of the relative position of each surface point was studied by measuring the standard deviation (SD) from the average height of the points (number of points ranged from 623 to 1883 depending on the size of the disc area) that identify the optic disc surface. All the subjects were also examined with the Humphrey perimeter (Program 30-2) and the results were analyzed by Mann-Whitney U-test and Spearman rank correlation coefficient. RESULTS: The difference in ODSS between the glaucomatous and the normal eyes was statistically significant. No significant correlation was found between age and ODSS. A significant correlation was found between ODSS and mean deviation (MD) and between ODSS and corrected pattern standard deviation (CPSD). In the glaucoma group there were significant correlations between ODSS and the perimetric indices MD and CPSD; no correlations were found in the normal group. CONCLUSION: The ability of ODSS to reflect the level of visual field change in glaucomatous eyes appears to be rather limited, even if statistically significant. This fact does not limit the clinical usefulness of ODSS as a diagnostic parameter in consequence of its strong correlations with the other morphometric parameters and its high sensitivity and specificity in separating normal from glaucomatous eyes.

Effect of betaxolol on primary open-angle glaucoma and normal-tension glaucoma patients.

This study is aimed at establishing the efficacy of the therapeutic agent, betaxolol, in diurnal control of IOP (intraocular pressure). Therapy was performed on 32 eyes affected by POAG (primary open-angle glaucoma) and 16 eyes affected by NTG (normal-tension glaucoma). Two preparations of betaxolol were utilized: betaxolol hydrochloride 0.50% (Betoptic) was administered to 15 POAG and 7 NTG eyes; betaxolol hydrochloride 0.25% ophthalmic suspension (Betoptic S) was administered to 17 POAG and 9 NTG eyes. IOP measurements were taken every two hours from 8 a.m. to 8 p.m. IOP was measured before therapy and at 12 hours, 30 days, and 3 months of therapy. Betaxolol hydrochloride 0.50% was more effective at lowering IOP during the day. Diurnal pressure peaks, which are a risk factor concerning the maintenance of visual field in glaucoma patients, were also reduced using 0.50% betaxolol hydrochloride.

Bacterial load and protein deposits on 15-day versus 1-day disposable hydrophilic contact lenses.

PURPOSE: This study quantified the bacterial load and protein deposits on 1- and 15-day disposable contact lenses after use in normal wearers. METHODS: Sixteen patients were randomly assigned to a 1-day contact lens (1-Day Acuvue) in one eye and to a 15-day contact lens (Acuvue) in the contralateral eye. Only one specified solution was allowed for the care of 15-day lenses. All patients were evaluated every month for 6 months (at times T1, T2, T3, T4, T5, T6). At times T1, T3, and T5, the lenses were removed in a sterile fashion and sent for laboratory quantification of Staphylococcus aureus and Pseudomonas aeruginosa. At T2, T4, and T6, quantification of protein deposits was determined, and at T0 and T6, impression cytology of the conjunctiva was performed. RESULTS: P. aeruginosa was not identified on any lens. At T1, T3, and T5, S. aureus was significantly greater on the 1-day versus 15-day lenses (p < 0.001). In contrast, protein deposits were significantly greater on the 15-day lenses at all time points (T2, T6: p < 0.01; T4: p < 0.05). Impression cytology of the 15-day lens eyes revealed a worsening trend compared to the 1-day lens; however, no statistically significant differences were found between the two groups (p = 0.29). CONCLUSION: Results of this study suggest that the use of cleaning and preservative solutions can alter the ocular surface bacterial environment of the contact lens wearer and that these changes are not a direct consequence of contact lens wear. The bactericidal activity of these solutions could, with time, also affect ocular surface cells, leading to contact lens intolerance and ocular surface disease.

RA/DA cumulative curve analysis of local and diffuse neuroretinal rim area damage in glaucoma patients.

AIM: To evaluate the validity of cumulative rim/disc area (RA/DA) curve analysis as a clinical tool for the identification of glaucoma induced optic disc pathology. METHODS: 71 normal and 83 glaucomatous eyes were evaluated from a series of 154 subjects recruited for this study. For each eye, the cumulative distribution of RA/DA was calculated from 36 equally spaced rim sectors of each optic disc obtained by the automatic evaluation of simultaneous videographics (Image-net X Rev.3/51b). To increase the sensitivity of this analysis in early glaucoma and in normal eyes, these cumulative curves were subsequently divided into two equal segments and the slopes of their respective regression lines compared. RESULTS: The median RA/DA value obtained from the 36 sectors was significantly different in glaucomatous eyes compared with normals (p < 0.001). Nevertheless, the curves (5th-95th percentile of the cumulative curves distribution) of early glaucomatous eyes fell within the normal range. When the cumulative curve of these marginal cases was then divided into two equal segments, the comparison of the slopes of the regression lines showed a significant difference (p < 0.05) in 100% of early glaucomatous eyes. Furthermore, normal eyes were shown to be true negatives in 93% of the cases in which no significant difference between the two slopes was observed. CONCLUSION: Analysis of the RA/DA cumulative curve from 36 sectors of the optic disc was a valid method for the identification of glaucomatous disc pathology; however, a further calculation of the slopes of the two RA/DA regression lines was needed to identify early glaucomatous damage.

Hepatitis C virus in patients with porphyria cutanea tarda: relationship to HCV-genotypes.

Porphyria cutanea tarda (PCT) is a rare metabolic disorder characterized by an abnormal porphyrin metabolism and typical cutaneous lesions. Recently a strong association between PCT and hepatitis C virus (HCV) has been proposed. Studies in south Europe have shown high prevalence (53 to 91%) of HCV markers in patients with PCT. We studied HCV genotypes in 72 subjects: 40 with PCT and 32 patients with chronic liver disease. A high rate of HCV-RNA positive PCT patients (84%) was observed, reflecting an active HCV replication, the genotypes study showed a prevalence of genotype 1b in PCT patients (61.2%). These findings implicate HCV in the aetiology of PCT-associated liver disease suggesting that hepatitis C serological and virological testing could be indicated in all patients with PCT.

[Magnetic resonance pyelography: clinical use of ultrafast breath-hold sequences in obstructive uropathy. Personal experience].

INTRODUCTION: We investigated the diagnostic yield of MR pyelography (MRP) performed with two ultrafast breath-hold sequences in obstructive uropathy patients. MATERIAL AND METHODS: Thirty-four patients with US demonstration of urinary tract dilation were examined with MRP at 1.5 T and with a standard body coil. HASTE sequences (TE 66, ETL 128, FA 180 degrees, 1 NEX, MA 240 x 256, slice thickness 4 mm, TA 13 s) were always performed and 26/34 patients were also examined with single-shot TSE sequences (TE 1100, ETL 240, FA 150 degrees, 1 NEX, slice thickness 7 cm, MA 240 x 256, FS, TA 7 s); 20/34 patients were given 500 mL superparamagnetic contrast agent (Lumirem). MRP findings were interpreted independently by two radiologists and analyzed for image quality, the presence/absence of dilation, the obstruction grade (mild, moderate, severe), site (intrarenal; ureteropelvic junction; proximal, mid-, distal ureter; indeterminate) and cause (intrinsic obstruction from a stone or other conditions; stenosis; indeterminate cause). The sensitivity, specificity, positive and negative predictive values for the detection of urinary tract dilation were calculated for each reviewer and interobserver agreement was calculated with the k analysis relative to the presence, grade, site and cause of urinary tract dilation. RESULTS: MRP examinations were considered technically adequate in all cases by both observers. As for the presence of urinary tract dilation, the values were 100% for the first observer and 97%, 100%, 100%, and 95%, respectively, for the second observer. Interobserver agreement was .98 for dilation presence (excellent), .80 for dilation degree (excellent), .62 for dilation site (good) and finally .69 for dilation cause (good). DISCUSSION AND CONCLUSIONS: MRP performed with the ultrafast breath-hold technique provides very good results in diagnosing urinary tract dilation, as well as the obstruction grade, site and cause, with results equal or even superior to those of non-breath-hold sequences. Ultrafast MRP lasts only 10 minutes, meaning it occupies the magnet shortly and costs less: it can be thus considered a routine alternative to conventional diagnostic imaging, especially ivp, in the evaluation of obstructive uropathy.

Iron and porphyria cutanea tarda.

In order to evaluate the pathogenetic role of iron in Porphyria cutanea tarda (PCT), the metabolism of iron was studied in 440 patient with PCT and associated chronic liver disease (CLD) and in 91 nonporphyric CLD patients (used as a control group). The parameters considered were the following: serum iron, ferritin, Total Iron Binding Capacity (TIBC) and percent saturation of transferrin. The statistical analysis showed that the differences between the means, in the two groups, were not significant in any of the parameters examined. To investigate the possible relationships between iron metabolism and other chemico-clinical parameters concerning the porphyric disease, the associated hepatic disease and hemometry, we studied the correlations between iron parameters and total urinary and serum porphyrins, serum copper, serum albumin, hemoglobin, red blood cells, ALT, AST, CHE and GLDH. This investigation was only possible in the last 99 cases. In addition to the obvious correlations between the parameters concerning iron metabolism, the highly significant (p < 0.001) correlation between ferritin and enzyme activities which indicate cytolysis (ALT, AST, GLDH) is extremely interesting. The results seem to point to the tentative conclusion that the alterations of iron metabolism are more related to the hepatocellular necrosis than to the metabolism of porphyrins.

[Therapeutic considerations on a case of mixed teratoma of the testis with seminomatous component].

The Authors present a case of mixed teratoma of the testis with seminomatous cells, occurred in a teenager. Performed the preoperative oncological staging, the patient was submitted to trans-inguinal left orchifunicolectomy. The presence of cells originated by the three embryonal layers, of stroma distributed in a periepithelial fashion and big cells with clear cytoplasm, permitted to formulate the hystological diagnosis of mixed teratoma, mature and immature, associated with seminomatous cells. The observation of this case offer the opportunity for prognostic and therapeutic considerations.

The value of in vitro ultrasonography in the intraoperative staging of gastric cancer. Blind study of 93 cases.

The correct staging is of crucial importance in the choice of surgical treatment in patients affected by stomach carcinoma. In an attempt to achieve a greater accuracy in the diagnosis of depth of tumor invasion, we propose to perform ultrasonography of the gastric wall during surgery (in vitro ultrasonography). Immediately after removal by the surgeon, the portion of the stomach containing the tumor is submitted to ultrasonography with a linear 7.5-MHz probe. To obtain good ultrasonographic images it was necessary to interpose an echo-free standoff pad between the probe and the gastric wall and between the latter and support surface. On completion of ultrasonography, the surgical specimen was sent to the pathologist, who was unaware of the diagnosis formulated on the basis of in vitro ultrasonography (VUS). The VUS diagnosis of T corresponded to the pathological diagnosis in 87/93 cases (93.5% accuracy). In the early cancers the diagnosis was correct in 24/28 cases (85.7%); in the advanced cancers the diagnosis was correct in 63/65 cases (96.9%). In comparison with the results of the other preoperative and intraoperative techniques the accuracy of in vitro ultrasonography in diagnosing the depth of tumor invasion was clearly superior. We feel that VUS may become a basic instrument in T staging and an aid to the surgeon in deciding the extent of the resection and of lymphadenectomy in patients with gastric cancer.

Incidence of hereditary porphyria cutanea tarda (PCT) in a sample of the Italian population.

The determination of the enzymatic activity of URO-D in erythrocytes is the screening method used for differentiation between hereditary and non-hereditary forms of porphyria cutanea tarda (PCT). The aim of the present work was to establish the relative frequencies of the symptomatic and hereditary forms by the determination of the URO-D enzyme in the PCT patients who were regularly treated at the Centre for Porphyrins in our Institute. In the course of this work we also examined the statistical properties of the distributions of both normal and porphyric subjects, so as to be able to suggest values for discriminating between normal subjects and the various types of porphyric subjects.
