Distribution of axillary node metastases by level of invasion. An analysis of 539 cases.

Five hundred and thirty-nine patients with carcinoma of the breast treated with total axillary dissection and with positive axillary nodes were evaluated. The total number of lymph nodes removed was 11,082, with an average of 20.5 nodes per patient. The average number of lymph nodes at the first level was 13.8, at the second level 4.5, at the third level 2.2. The average number of nodes was 20.7 in cases treated with Halsted mastectomy, 20.9 with total mastectomy and axillary dissection, 20.3 with quadrantectomy and axillary dissection. Of 3259 metastatic nodes, 64 were site of micrometastases; 797 were partially involved, 441 were totally involved and 1957 were site of metastases with extracapsular invasion. In 314 (58.2%) the first level only was involved, in 117 cases (21.7%) metastases were present at the first and second level, whereas in 88 cases (16.3%) all the three levels were sites of metastases. Only 20 cases showed skipping distribution. In 1.5% of the cases the first level was skipped by metastases, in 0.4% the first nodes of the first level are clear the chances that metastatic nodes are present at the second and third levels are negligible. When the nodes at the first level are positive, the chances that metastases are also present at the higher levels are of the order of 40.0%.

Teicoplanin, antibiotics from Actinoplanes teichomyceticus nov. sp. VIII. Opening of the polypeptide chain of teicoplanin aglycone under hydrolytic conditions.

Hydrolysis of teicoplanin (a complex of five closely related factors plus one, more polar component) under selected conditions (acids in a biphasic hydroalcoholic medium) gives the single aglycone with good yields. When the reaction is carried out in homogeneous hydroalcoholic phase the removal of the sugars yields two new compounds. On the basis of fast atom bombardment mass spectra (FAB-MS), acid-base titration, IR, UV and 1H NMR analyses it has been demonstrated that these compounds are two diastereoisomers; they differ from the teicoplanin aglycone in having additional carboxyl and amino groups derived from the hydrolysis of an amide bond. Although the molecular shape of the new aglycones is greatly modified, they still maintain some antibacterial activity which might be correlated with residual binding ability towards the terminal D-alanyl-D-alanine residue of the cell-wall mucopeptides.

Determinations of tissue levels of cefatrizine in blood, lungs and bronchi.

A study was carried out in 12 patients, divided into two groups of 6, to determine tissue levels of cefatrizine in lung (group I) and bronchial secretions (group II) following a single oral dose of 500 mg. In group I, specimens of blood and lung tissue were collected after 2 h from one subgroup of 3 patients and after 3 h from the other subgroup of 3. Average levels were 8.5 and 7.0 mcg/ml for blood and 1.2 and 1.4 mcg/ml for lung tissue respectively. In group II blood and bronchial secretion concentrations were evaluated at the 2nd, 3rd and 6th hours from administration. Average values were 9.1 and 7.7 mcg/ml in blood at 2h and 3h respectively, whereas the average bronchial secretion concentration at the 3rd hour was 10.4 mcg/ml in the first subgroup. In the second subgroup the mean level in blood collected at the 2nd hour was 8.9 mcg/ml, and 2.5 and 4.1 mcg/ml respectively in blood and bronchial secretions at the 6th hour. Cefatrizine levels in bronchial secretions were higher than those in blood at both the 3rd and the 6th hour from administration: this kinetic peculiarity of the drug will doubtless play an important role in the therapeutic efficacy of cefatrizine.

Chemoprevention trial of contralateral breast cancer with fenretinide. Rationale, design, methodology, organization, data management, statistics and accrual.

The Fenretinide (4-HPR) Breast Cancer Study is a randomized multicenter clinical trial originally designed and conducted by the investigators of the Istituto Nazionale Tumori of Milan. The study is sponsored by the National Cancer Institute of Bethesda and by the Italian National Research Council. The trial was designed to evaluate the effectiveness of the synthetic retinoid 4-HPR, at a dose of 200 mg per os every day for 5 years, in reducing the incidence of contralateral breast cancer in a population of patients previously operated on for breast cancer. Between 1987 and 1993, the Istituto Nazionale Tumori of Milan and 9 other collaborating Centers enrolled 2,972 women between the ages of 30 and 70 years who had been previously operated on for T1-T2 N- M0 breast cancer. This paper describes the rationale, design, methodology, organization, data management, statistics and accrual of the participating population.

[Study of fecal antigens against Helicobacter pylori as non-invasive test compared with the 13C-urea breath-test. Personal experience].

The target of this study is the evaluation of two different noninvasive tests: 13C-urea Breath-Test, as gold standard, versus the HpSA test, as new method to research the faecal antigen of Helicobacter Pylori (HP). Thirty patients, affected by dyspeptic symptomatology and never treated before by antibiotic therapy to eradicate the HP, was subjected, by Authors, to this evaluation. This study say that the 13C-urea Breath-test represent the gold standard in the pre-endoscopic research of HP presence concerning specificity and sensibility. However, the HpSA test need more improvement.

[Ciprofloxacin: an alternative oral treatment in respiratory Pseudomonas infection in cystic fibrosis].

20 CF patients, aged from 16.5 to 31.7 years, with chronic pulmonary infection due to Pseudomonas, were included in an open trial to study the efficacy of ciprofloxacin on respiratory exacerbation. Ciprofloxacin was given orally at the dose of 1500 mg/die for ten days. 16 patients concluded the entire treatment with clear clinical improvement, based on a score including 11 parameters. There has also been a significant improvement in the pulmonary function tests, and a tendency of Rx score to decrease. 4 patients interrupted the treatment on the fifth day because of clinical inefficacy. There was no increase of Pseudomonas resistance to ciprofloxacin at the end of the treatment; 30 days after no strain of pseudomonas was found resistant. We observed side-effects in 5 patients, but in no case it was necessary to discontinue the treatment. Ciprofloxacin may be considered as a good alternative to the more established antibiotic strategy in the treatment of Pseudomonas lung exacerbations in CF.
