Expression profile of MDM-2 proteins in chronic lymphocytic leukemia and their clinical relevance.

The MDM-2 oncoprotein exists in an autoregulatory feedback loop with the tumor suppressor protein p53. Therefore, intracellular levels of these two proteins may play important roles in cell proliferation and tumorigenesis. Several MDM-2 proteins (Mr 35-100 Kd) have been demonstrated in human cell lines. We report here the expression profile of MDM-2 and p53 proteins in 87 cases of chronic lymphocytic leukemia (CLL) as detected by immunoblot analysis. The MDM-2 proteins (p57, p59, p67, and p90) were found to be overexpressed in different combinations in 56/87 (64%) of cases of CLL when compared with normal volunteers. The MDM-2 protein p57 was predominantly overexpressed 46/87 (53%) in CLL. In 22/87 (25%) cases of CLL p57 was overexpressed alone, and in 24/87 (28%) cases it was co-overexpressed with other MDM-2 proteins p59/p67/p90. Six of the 87 cases of CLL showed overexpression of the tumor suppressor protein p53 by immunoblot analysis, and five of those cases also co-overexpress MDM-2 protein p57. No statistically significant correlation of MDM-2 protein overexpression to clinical disease stage and history of previous chemotherapy of CLL patients has been found. However, considering the oncogenic potential of overexpressed MDM-2 proteins, a possible role of MDM-2 proteins in the promotion of CLL disease remains to be evaluated.

Parental sex effects in bipolar affective disorder pedigrees.

A combined linkage analysis was performed on chromosome 18 data and produced modest evidence in three of four data sets for linkage of a susceptibility locus for bipolar disorder to markers on chromosome 18p. All data sets showed a preponderance of females among affected individuals. When this was taken into account, no convincing evidence was obtained for excess transmission from mothers compared to fathers. In addition, there was no evidence for differences in the proportions of affected offspring from affected fathers and mothers. We conclude that these combined data do not support previous suggestions of a maternal effect on transmission of bipolar affective disorder.

Bystander CPR, ventricular fibrillation, and survival in witnessed, unmonitored out-of-hospital cardiac arrest.

STUDY OBJECTIVE: To assess whether bystander CPR (BCPR) on collapse affects initial rhythm and outcome in patients with witnessed, unmonitored out-of-hospital cardiac arrest (OHCA). DESIGN: Prospective cohort study. Student's t test, the chi 2 test, and logistic regression were used for analysis. SETTING: Suburban emergency medical service (EMS) system. PARTICIPANTS: Patients 19 years or older with witnessed OHCA of presumed cardiac origin who experienced cardiac arrest before EMS arrival between July 1989 and July 1993. RESULTS: Of 722 patients who met the entry criteria, 153 received BCPR. Patients who received BCPR were younger than those who did not: 62.5 +/- 15.4 years versus 66.8 +/- 15.1 years (P < .01). We found no differences in basic or advanced life support response intervals or in frequency of AED use. More patients initially had ventricular fibrillation (VF) in the BCPR group: 80.9% versus 61.4% (P < .01). The interval to definitive care for ventricular tachycardia (VT)/VF was longer for the BCPR group (8.59 +/- 5.3 versus 7.45 +/- 4.7 minutes; P < .05). The percentage of patients discharged alive who were initially in VT/VF was higher in the BCPR group: 18.3% versus 8.4% (P < .001). In a multivariate model, BCPR is a significant predictor for VT/VF and live discharge with adjusted ORs of 2.7 (95% CI, 1.7 to 4.4) and 2.4 (95% CI, 1.5 to 4.0), respectively. For those patients in VT/VF, BCPR predicted live discharge from hospital with an adjusted OR of 2.1 (95% CI, 1.2 to 3.6). CONCLUSION: Patients who receive BCPR are more often found in VT/VF and have an increased rate of live discharge, with controls for age and response and definitive care intervals. For VT/VF patients, BCPR is associated with an increased rate of live discharge.

Bystander vs EMS first-responder CPR: initial rhythm and outcome in witnessed nonmonitored out-of-hospital cardiac arrest.

OBJECTIVES: To assess whether outcome and first-monitored rhythm for patients who sustain a witnessed, nonmonitored, out-of-hospital cardiac arrest are associated with on-scene CPR provider group. METHODS: A retrospective, cohort analysis was conducted in a suburban, heterogeneous EMS system. Patients studied were > or = 19 years of age, had had an arrest of presumed cardiac origin between July 1989 and January 1993, had gone into cardiac arrest prior to ALS arrival, and had received CPR on collapse. First-monitored rhythms and survival rates were compared for two patient groups who on collapse either: 1) had received CPR by nonprofessional bystanders (BCPR) or 2) had received CPR by on-scene EMS system first responders (FRCPR). RESULTS: Of 217 cardiac arrest victims, 153 (71%) had received BCPR and 64 (29%) had received FRCPR. The BCPR patients were slightly younger (62.4 vs 68.4 years, p = 0.01) and had slightly shorter ALS response intervals (6.4 vs 7.7 minutes, p = 0.02). There was no difference in BLS response time intervals or automatic external defibrillator (AED) use rates. Naftidrofuryl, a putative activator of neuron survival, stimulates the expression of neurofilament heavy subunit in cultivated spinal cord neurons from chicken.

The effect of naftidrofuryl, a drug used in ischemia for its vasodilator properties and its protective effect on neuronal survival, was investigated on the maturation of cultured chicken spinal cord neurons, focusing on the presence of proteins specific for the developing neuronal cytoskeleton. Although no influence of naftidrofuryl on the rate of growth of neurites was observed, the drug enhanced the relative amount of the high molecular weight neurofilament subunit without affecting the concentration of a microtubule-associated protein, MAP2. These findings suggest that the effect of naftidrofuryl on cultured spinal cord neurons might involve molecular events directly associated with the induction of a mature cytoskeleton architecture, instead of stimulating undifferentiated neurite growth.

Influence of light dark cycles on estradiol-17 beta induced luteinizing hormone patterns of the prepuberal gilt.

Two groups of Yorkshire gilts (110 d of age) were maintained in two light regimens. Both light regimens consisted of 14 h of light and 10 h of darkness, but were 180 degrees out of phase. Gilts in Group 1 received light from 1200 to 0200 and gilts in Group 2 from 2400 to 1400. At approximately 140 d of age each group was divided into four subgroups of eight gilts each (1A, 1B, 1C, 1D or 2A, 2B, 2C, 2D). All gilts were blood sampled at 2-h intervals for 5 d commencing on d 142. The four subgroups received a single injection of estradiol (15 micrograms/kg body weight) on d 143 at either 2400 (A), 0600 (B), 1200 (C), or 1800 (D). For pigs in Groups 1A and 1D, the injection of estradiol coincided with the animals' "subjective day" and the injections given to Groups 1B and 1C with their "subjective night." When estradiol-17 beta (E2) was administered to the gilts during their subjective day the LH profile showed one peak, whereas when E2 was administered during dark hours the profile exhibited two peaks (P < .0001). In Group 2 for which the light cycle was reversed, the well-defined spikes of LH were found to coincide with the injections of estradiol administered during the dark hours. Smaller biphasic peaks of LH occurred when injections of estradiol coincided with the light hours.(ABSTRACT TRUNCATED AT 250 WORDS)

Otomycosis: an experimental evaluation of six antimycotic agents.

One hundred-and eight-six fungi were isolated from one hundred-and-eighty cases clinically diagnosed as otomycosis. They comprise 59 species of 26 genera of moulds, and 2 genera of yeasts. This large variety of mould isolates provided ample material for in vitro experiments, to evaluate the anti-fungal activity of six antimycotic substances found in the preparations currently used for the treatment of otomycosis. The antifungal activity of clotrimazole and tolnaftate in vitro was evident. For over 94 per cent of the 59 fungus species tested, the MIC was less than 0.1 micrograms/ml, and for 6 per cent it was between 0.4 and 1 microgram/ml. Concerning the other 4 antimycotic substances (iodochlorhydroxyquin, fluonilid, natamycine and polymyxin B sulphate) the MIC ranged from greater than 100 micrograms/ml to 1 microgram/ml for the majority of tested fungi. The clinical observations were mostly in accordance with these findings.

Non-invasive antromycosis: (diagnosis and treatment).

Increasing numbers of cases of chronic maxillary sinusitis are encountered which resist frequent sinus irrigation and treatment of predisposing factors. The antral washouts are of unusual consistencies and colours. The antral washouts of six cases were investigated by culture and microscopic examination and proved to contain fungal colonies. Antroscopy was a valuable asset in diagnosis. One patient had a Caldwell-Luc operation and daily irrigation through an indwelling polythene tube followed by daily instillation of clotrimazole (Canesten). The five patients who refused operation were treated by repeated bi-weekly antral washouts followed by instillation of clotrimazole. Weekly samples of antral secretions were examined by culture and microscopic examination until they were free for four consecutive weeks. We believe that frequent antral irrigation and local instillation of a broad-spectrum antimycotic drug--preferably after debridement--is the treatment of choice for antromycosis.

Aural polypi: a histopathological and histochemical study.

Aural polypi associated with chronic suppurative otitis media have been studied histopathologically and histochemically in 20 patients. The polypi consist of an edematous connective tissue stroma infiltrated by chronic inflammatory cells and numerous blood vessels. The surface is covered by intact stratified squamous epithelium. The histochemical study revealed altered permeability of the blood vessels which seem to be the main pathological background for polyp formation. An enhanced phagocytic activity and increased metabolic activity were found in the epithelium in the chronic inflammatory cells. No glandular activity was found in the polypi.

Tonsillomycosis: a myco-histopathological study.

Tissues from 227 tonsillectomies, mainly in adults, were examined culturally and histopathologically for the presence of fungus. Actinomyces was demonstrated in 69 cases (30.40 per cent) by both methods. Yeasts and moulds were each isolated from 21 cases (9.25 per cent) and mixed isolates were obtained from 7 cases (3.08 per cent). There was no histopathological evidence of these organisms. Negative cultural and histopathological results were obtained in 53 cases (23.35 per cent). Chloramphenicol-resistant bacteria were cultured in 56 cases (25.11 per cent) but were seen histopathologically in only 25 cases (11.01 per cent). In all cases in which Actinomyces or bacteria were present histopathologically, a minimal tissue response indicated that these organisms were saprophytes and not primary pathogens.

Rhinitis medicamentosa: electron microscopic study.

The deleterious effect of decongestant drops on the rabbit respiratory nasal mucosa was studied with the aid of the electron microscope. The cilia were severely damaged, with the appearance of virus-like particles. The epithelium showed characteristic morphological details suggestive of oedematous infiltration. The tunical blood vessels revealed ultrastructural changes of their endothelial lining and basal lamina indicative of increased permeability.

Eucaryotic transposable genetic elements with inverted terminal repeats.

DNA carrying inverted repeats was tested for transposition within the Drosophila genome. Five Bam HI segments containing related inverted repeats were isolated from D. melanogaster and analyzed by electron microscopy and restriction mapping. Southern blot experiments using single-copy flanking sequences as probes allowed the study of DNA arrangements at specific sites in the genomes of five closely related strains. We found that in some genomes the sequences with inverted repeats were present at a particular site, whereas in other genomes they were absent from this site. These results indicated that three of the sequences are transposable genetic elements. In one case we have purified the two corresponding DNA segments, with and without the sequence containing inverted repeats, thereby confirming the mobility of this sequence. These DNA elements were found to be distinct in two ways from copia and others previously described: first, they contain inverted terminal repeats, and second, they have a more heterogeneous construction.
