Influence of human immunodeficiency virus infection on cell-mediated immunity in chronic D hepatitis.

To determine whether the abnormalities of cell-mediated immunity described in chronic D hepatitis are associated with hepatitis D virus (HDV) infection or concomitant human immunodeficiency virus (HIV) infection, serologic and tissue hepatitis B virus (HBV) and HDV markers and T lymphocyte subsets were studied in serum samples from 38 patients with chronic D hepatitis, 26 of whom had HIV infection. Patients with chronic D hepatitis and HIV infection had significantly lower peripheral blood T4:T8 ratios resulting from a significant increase in T8+ (suppressor/cytotoxic) cells, while numbers of T lymphocyte subsets were normal in cases with chronic D hepatitis only. HIV+ patients showed an increase in HBV replication (identified by hepatitis B core antigen in liver and hepatitis B e antigen and HBV DNA in serum) and in HDV replication (tissue D antigen and HDV RNA) without evidence of more active liver disease. Probably the immunologic disturbances detected in chronic D hepatitis are secondary to HIV infection, do not contribute to the pathogenesis of liver injury, and are associated with increased viral B and D replication.

[Cluster headache. A clinical study].

The personal experience with cluster headache in 108 patients is reported. Significant clinical findings included a clear male predominance, with a male/female prevalence ratio of 4.7/1, the persistent homolateral character of pain during the attacks, seen in 100% of cases, and the tendency to repeat the attacks on the same side (96%). The cluster headache predominated during spring and the painful crisis, in our experience, had a clear nocturnal predominance. Regarding the signs and symptoms accompanying pain, our series was similar to those in the international literature, except for a smaller prevalence of Horner's sign in our cases. The association with tenderness of the carotid territory ipsilateral to pain is reported here for the first time. The importance of a correct anamnesis of all parameters associated with pain is emphasized in the present study, as in many patients only a detailed investigation permits an adequate clinical definition.

[Hemiparesis-ataxia in meningovascular syphilis].

Two cases of lacunar infarction of "ataxic-hemiparesis" type secondary to luetic vasculitis are presented. The first patient presented meningovasculitis and the second progressive general paralysis. In both cases the ischemic lesion was seen by magnetic resonance and was located at a pontine level in one case and in the corona radiata in the other. Treponemal serology in plasma and CSF was positive in both cases with lymphocytic pleocytosis and high CSF protein levels being observed. Clinical and response to treatment was satisfactory except in the case of general paralysis in whom the impairments existing prior to the cerebral infarction persisted. This study complements recent communications in which neurosyphilis was attributed to the appearance of other lacunar type infarctions (pure motor hemiparesis).
