The authors describe the case of a rare developmental fetal anomaly diagnosed in intrauterine life. Performed ultrasound examination in the 30th week of pregnancy showed in the area of the abdomen of the fetus three ampullar structures, so-called "triple ampulla symptom". These ampullas corresponded to the fetal stomach, the duodenum and a fragment of the small intestine. The suspicion of the innate obstruction of the intestine was confirmed after the birth of the fetus. An attempt of surgical treatment of the diagnosed abnormality was not successful. The child died in its 6th week of life.

The authors have made experiments with cryoablation for menorrhagia in order to find out if cryotherapy is an effective of treatment for this kind of disease. Ten patients aged 35-45 were treated this way. Previously all of them had undergone dilatation and curettage. Pre-operatively the patients were treated with medroxyprogesterone till the endometrium reached the thickness of 4 mm. The cryodevice for the operation was made by a Polish producer "Kriometrum". 0.9% saline was used as an uterine distension medium. There were no operative complications in none of the patients. In the first 24 hour period after the treatment clear vaginal discharge appeared with lasted no longer than 14 days. The first vaginal ultrasonography 2 months after the treatment showed none or only traces of endometrium in all the patients a minimal bleeding of menstrual type appeared in the 7th week after the treatment and it continued till the sixth month of observation. This method of treatment has stopped menorrhagia. For how long-we will have to wait to see. The obvious thing is that endometrial cryotherapy is a cheap, quick and technically very easy treatment.
The authors tried to assess considerably of the cervical electroconisation in the cervical cancer prophylaxis. The 2555 women with cervical erosions were treated by electroconisation. The electroconisations were performed when cytological and colposcopical examinations were unsuspected and conservative treatment in these cases has failed. Histopathological studies after conisation's samples demonstrated in 97% cases paraphysiological cervical changes. In 3% treated women cervical intraepithelial neoplasia was diagnosed. In one case cervical carcinoma was diagnosed in postoperative material. In this study 97% efficiacy of cervical electroconisation was demonstrated. Authors suggest that cervical electroconisation is recommended method in the treatment of cervical paraphysiological changes and plays important role in cervical cancer prophylaxis.
Among micro-organisms infecting vagina whose dominant genus is Gardnerella vaginalis anaerobic bacteria are often present. Well-known damaging tissues and cells features of anaerobic bacteria are taken under consideration as factors which are conductive to generating pathological changes in cervical epithelium. The aim of this study was evaluation of cytological smears (Pap smear, Gram stain and H + E stain) in women with Gardnerella vaginalis infections of vagina and uterine cervix. The research was carried out on 1180 women hospitalised in the prior of fourteen months in Obstetrics-Gynaecological Department of the Municipal Hospital in Kielce. It was proved that characteristic microscopic feature of vagina and uterine cervix Gardnerella vaginalis infection is the absence of lactic acid bacillus, Lactobacillus vaginalis in vaginal contents and a presence of a small number of polynuclear neutrophil leucocytes. Marker cells (clue cells) are easily recognised in microscopic image in vaginal smears stained by Gram's method as well as Papanicolaou method. High sensitivity and specificity of these markers suggest usefulness of cytological smears in diagnosing Gardnerella vaginalis infections.
Among micro-organisms infecting vagina whose dominant genus is GV anaerobic bacteria are often present. There are reports that GV infections of vagina and uterine cervix, apart from their well-known negative role in obstetrics practice can play a role in carcinogenic processes of uterine cervix. The aim of the study was to assess characteristic colposcopy images of cervix in women with Gardnerella vaginalis infection. The research was carried out on 1180 women hospitalised in the period of 14 months. Many observations lead to conclusions that pathognomic clinic feature of Gardnerella vaginalis infection of uterine cervix is visible in colposcope presence of clean, translucent mucus in external cervical os and opaque vaginal contents in the rear vaginal vault. High hydrogen ion concentration in vaginal contents (pH 6.0 and over) correlates with positive "fishy odour test" of this contents. Gardnerella vaginalis infection of vagina and uterine cervix concerns women with existing erosion-type changes of uterine cervix. Visible in colposcopic test restless and "spotted" images visible after Schiller test are pathognomic colposcopic features of Gardnerella vaginalis infection.
Authors presented successful conservative treatment early diagnosed cervical pregnancy. After this procedure abdominal hysterectomy was not necessary.
The authors are presenting an analysis of the prophylactic examinations carried out within the period of seven years among women under the care of the Municipal Health Care Center in Kielce. They are also trying to evaluate the influence such examinations have in forming up the detectability of preneoplastic states and cancer of the uterine cervix. The studies were carried out from 1982-1988. A rule was adopted to repeat the examinations every two years. Owing to organizing difficulties, the first stage of the examinations took three years. The basis of the prophylactic examinations was cytologic examination. The detectability of intraepithelial neoplasm of the uterine cervix (CIN) in the three stages was 0.26%, 0.16%, and 0.07%. It was observed therefore, the gradual drop in the number of the detectable CIN among the population in the duration of the studies. A "shift" of detectable neoplasma changes in the uterine cervix in the direction of a higher percentage of preinvasive cancers and with an early invasion was observed. This shift, beside the stable decrement of the absolute number of detectable cancers is a favourable phenomenon and testifies the great usefulness and the effectiveness of the studies.
In the paper authors consider the problem if Chlamydia trachomatis infection of urogenital tract exerts an influence on postoperative course in women who were operated in the gynaecology department. 186 women have been examined in this study. Using the criteria for selective screening for Chlamydia trachomatis infection proposed by Handsfield, 29 women with higher risk for chlamydial infection were seperated. The personal details have been obtained by standardized questionnaire. Using the immunoenzymatic test "Chlamydiazyme" the incidence of Chlamydia trachomatis infection of the cervical canal was studied in 29 women from groups of higher risk for this infection. No Chlamydia trachomatis antigens were found in this group. These results don't answer the question if Chlamydia trachomatis infection of urogenital tract exerts an influence on postoperative course in women who were operated in gynaecology department, because it is to small trial.
Despite of the great deal of publications on artificial insemination the real effectiveness of the procedure is not clearly estimated so far. This is why in the presented paper authors tried to evaluate the effectiveness of the artificial intrauterine insemination with the abnormal sperm which was laboratory classified as oligo-astheno-teratozoospermia. The sperm which was prepared to the insemination according to the technic enabling to select only the population of the motile spermatozoa. The women were inseminated after the proper hormonal preparation and ultrasound monitoring of ovulation. 153 intrauterine inseminations were performed in 17 women. There was no case where the insemination was performed longer than during 6 consecutive cycles. There was no pregnancy obtained. The authors come to the conclusion that the intrauterine insemination with the abnormal semen of oligo-astheno-teratozoospermia type prepared in accordance with the used in the study technic is not an effective way of treating the infertile couple.
Authors presented the treatment of chronic, recurrent, aggravating inflammation of uterine adnexa currently used in their Ward and performed the task of evaluating the results of the carried out therapy. Material included 99 diseased women, at the age of 18-45, hospitalized at the Ward because of adnexitis chronica exacerbata. It was assumed that the criterion of recovery was regression of all clinical symptoms found by subjective and objective examination at the beginning of the treatment. Combined antibiotic-corticoid therapy was carried out complementary to resorptive treatment with lime and magnesium preparations, with ichthyol, dystreptase and a series of intravenous injection of chlormethine hydrochloride. Good results were achieved as far as regression of objective and subjective symptoms is concerned in 98% of the treated cases. Recurrence of the illness appeared in 6%.
